Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. Ship cannot provide a space categorization plan or clarification for the following spaces used for storage: lobby area between cryoconverter and watertight door 12, and sewage land greywater treatment room. Spaces were found with storage of combustable material and no fixed firefighting. 74 SOLAS 81 amend II-2/ 26.2.2

Every oil fuel pipe which if damaged would allow oil to escape from a storage tank shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire. The pneumatic remote shut off valve on the emergency generator fuel oil tank was not capable of remote operation. 74 SOLAS 97 II-2/15.2

The crew bled off the pressure from the fresh water supply tank, the salt water pump should have actuated at 6 kgf/cm^2 but it did not. The salt water pump control panel was not set-up for automatic operation.
