Mechanically properly "S" vessels which are ferry vessels or which carry more than 49 passengers shall comply with the provisions of Subchapter F of this chapter. Non-metallic flexible hoses were installed on fire main and bilge systems. 46 CFR 182.40-5

Inspect weather decks. Top deck on starboard side has area of de-lamination. Repair at next dry dock.

All machinery and equipment must be installed and maintained. Starboard main jacket water elbow to head tank had unapproved repairs to piping.

Inspection of all accessible parts of the exterior and interior of the hull. Fore peak deck access cover was seized shut in open position.

Each heater must be so constructed and installed as to prevent contact with combustible materials such as towels and clothing. Removed 2 portable space heaters from pilot house.

Upon activation of the fixed gas fire extinguishing system shutdown pressure switch, the main diesel engines secured, but the generator stayed running. 46 CFR 410(b)(10): A device must be provided to automatically shut down… engines that draw intake air from the protected space… Modify system to shut down all machinery in the engine room upon activation of the fire suppression system.

No means of alcohol testing were onboard the vessel or at the office. 46 CFR 4.06-15(a)(1): The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel… Purchase alcohol test strips or provide proof that alcohol breath testing can be completed within 2 hours of SMI.

A vessel to which 46 CFR Subchapter T applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. The vessel's COI is expired. Vessel must remain dockside until Renewal Inspection has been completed to the satisfaction of the marine inspector.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Install guard on all rotating machinery.

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Make through blkhd penetrations watertight.

Except as noted in paragraphs (d) and (e) below, each vessel must be equipped with a public address system. A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. Fix broken speakers for the PA system.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Reattach wire into junction box.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Provide a new first-aid kit with unexpired items.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator permit. Operator was unable to produce radio operator permit. Show proof of radio operator permit. CFR not listed: 47 CFR 80.159

Inspection and testing of portable extinguishers must be in accordance with NFPA 10, monthly inspections may be carried out by owner, operator, or designated crew. Vessel did not maintain records of monthly inspections. Maintain portable fire extinguishers IAW NFPA 10.

A vessel must carry a first aid kit with items as approved in 46 CFR 160.041. MI found expired aspirin and no ammonia inhalants. Outfit the first aid kit IAW 46 CFR 160.041.

Each connection or terminal end must be insulated and secured within an enclosure. MI found multiple dead end wires. Insulate and enclose.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels. Owner/operator did not have a random testing program that met the requirements of 46 CFR 16.230. Enroll an person serving on board that performs in a safety sensitive position in to the random testing program that meets 46 CFR 16.230.

All machinery installed on each vessel is suitable and maintained as to afford adequate protection against fire. Repair leaking fuel cutoff valve. 46 CFR 182.01-5 (Cite not listed in MISLE)

Wiring points and splices shall be mechanically secured and made in junction boxes. Secure dead end wires into junction box or tape ends located under bridge console and a/c unit. 46 CFR 183.05-50 (Cite not listed in MISLE)

Penetrations in watertight bulkheads must have means to make them watertight. Seal penetrations on forward engine room bulkhead.

A vessel must comply with any applicable requirements for any radio. Program MMSI number in VHF radio.

Alcohol testing of each individual must be conducted within 2 hours of a serious marine incident. Provide means to conduct alcohol test. 46 CFR 4.06-3 (Cite not listed in MISLE)

All vessels shall be drydocked and subject to inspection. The items listed a) through e) shall be be completed before the vessel is placed back in the water. All items shall be inspected by a marine inspector before receiving credit for drydock. a) Damage to FRP on port side amidships near water line exposing chop strand. Provide a repair proposal. b) Damage to FRP directly above port shaft, fwd of intermediate strut, prior repair is failing. Provide a repair proposal. c) FRP/coating on aft end of keel is failing. Provide a repair proposal. d) Various small areas of coating failure noted throughout the hull. Re-coat all exposed FRP. e) Make through-hull valves in fwd fuel tank space available for inspection. f) Base of railing stanchion at aft stbd corner cracked all the way through. Repair/replace railing. g) Aft-most 6 inches of seating bench on weather deck, main deck port side, is broken. Repair/replace bench. h) Engine room battery terminal connections found made with wing nuts. Properly secure all connections with permanent-type fasteners. i) Port diesel engine flywheel cover is corroded/missing and no longer provides adequate protection to personnel. Replace cover with suitable machinery guard. j) 40psi hydro-pneumatic tank in engine room fwd stbd corner is severely corroded. Provide new suitable tank. k) Overhead light fixture in engine room near fwd bulkhead is disassembled. Properly install light fixture.

The owner or managing operator shall make the vessel available for dry-dock and internal structural examinations at the appropriate intervals. Vessel must complete a credit for dry-dock and internal structural examination prior to the carriage of passengers.

Liquid level gauges shall be of heat resistant materials, adequately protected from mechanical damage, and provided at the tank connections with devices which will automatically close in the event of rupture of the gauge or gauge lines. Fuel tank gauge was plastic. 46CFR182.20-25(a)(5)

A vessel on a lakes, bays, and sounds route must carry three hand red flare distress signals. Vessels hand red flares were expired.

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Life buoy not labeled.

Except as allowed by paragraphs (b) and (c) of this section, before getting underway on a voyage or as soon as practicable thereafter, the master of the vessel shall ensure that suitable public announcements are made informing the passengers of safety topics in 185.506. Vessel did not conduct or have procedures onboard for passenger safety orientation.

Vessel must carry up to date charts, U.S. Coast Pilot, CG Light List, Tide and Current tables. Vessel's chart was not up to date and all other publications were not onboard.

Wiring joints and splices shall be mechanically secure and made in junction boxes. Wiring connections under helm console were not mechanically secure or made in junction box. 46CFR183.05-50(b)(1)

Every vessel subject to Part III of Title III of the communications act must have a detailed inspection of the radio installation by an FCC licensed technician IAW 47 CFR 80.59 once every 5 years. If the ship passes the inspection, the technician will issue a communications safety act certificate. Vessel could not provide the original certificate.

No Marine employer shall engage or employ any individual to sere as a crewmember unless the individual passes a chemical test for dangerous drugs for that employee. Marine employer did not have record of captain and crewmembers pre-employment testing.

Master shall conduct sufficient drills to make sure all crew members are familiar with their duties. Master did not have drill logs. Provide copies of drill logs.

Vessel must comply with applicable requirements for any radio installation as required by 47 CFR part 80. Master does not have radio operator permit. Provide copy of permit.

The starboard bridge wing buoy needs replacement.

The retroreflective tape on (26) lifejackets needs to be replaced.

The vessel name needs to be printed on (3) lifejackets.

The posted lifejacket donning instructions are for a model of jacket other than what is onboard. Replace posted placards with proper model.

The starboard side fire hydrant was spraying water at the connection. Repair or replace.

Repair potable water leak at the pipe/pump housing.

Cap multiple dead-end wires at the operating system that were improperly covered with electrical tape.

Station license expired in 2015. New license must be obtained.

Vessel must post lifejacket donning placard for different style PFD.

Provide updated magnetic compass deviation table.

Multiple deadend wires found in forward compartment and underneath helm operating station. Wires must be removed or capped. 46 CFR 183.01-5

Replace reflective tape on lifejackets.

Provide valid FCC Safety Radiotelephony Certificate. *Vessel limited to 1,000 feet from shore until corrected. 47 CFR 80.903

Vessel must have Coast Pilot and tide tables on board.
