The hydraulic power pack that actuates the isolation valves was leaking excessive amounts of hydraulic fluid at the directional control valve.

In meeting these requirements, the company should ensure that: any non-conformity is reported, with its possible cause, if known. The hydraulic hand start for the secondary start of the emergency generator was found to have a major hydraulic leak. Upon review of monthly and weekly maintenance log, the deficiency had not been reported in accordance with SMS procedure.
