On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each normally unmanned space. The vessel has one port and one starboard bilge high level alarm for three separate spaces in each hull.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Beginning with the January 2025 annual inspection, an annual rigging survey is required, paying special attention to the condition of the stainless steel wires and their terminals. The surveyor noted items in need of repair or replacement.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards including fire, caused by or originating in electrical equipment, and electrical shock. Open ended wiring was noted at the operating station, port side.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. The floating waterlight was found to be inoperable.

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Excessive rust was noted on the forward anchor and chain.

Except on a vessel limited to daytime operations, the compass must be illuminated. The vessel crew could not prove compass illumination.

A vessel must carry adequate and up-to-date U.S. Coast Pilot or similar publication, Coast Guard Light List, and tide tables. U.S. Coast Pilot was not maintained on board and the Coast Guard Light List and tide tables were found to be outdated.

A marine employer shall comply with the requirements of 46 CFR 4.06. The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in a Serious Marine Incident. The vessel could not produce alcohol testing devices for all crewmembers serving on board.

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the OCMI, but not fewer than the minimum number required by table 181.500(b). Vessel needs to replace 01 fire extinguisher.

The connections in a cable splice must be compatible with all other material in the splice and with the materials in the splice. Repair electrical connections behind the fuse panel.

Vessel needs to reinstall the forestay pin spacer in forestay.

*For longitudinal framing support structures (stringers) in both port & starboard sides : Multiple longitudinal framing support inner hull structures (stringers) have been found with dry wood decay exposing its fibers and losing support and strength. Stringers are to be sanded until good wood has been found and inspected by an attending USCG Marine Inspector. Stringers found with superficial dry decay-shall be treated with a wood preserver as recommended in wood repair NVIC 7-95. Stringers found to have excessive wastage (over 25%) shall be verified by an attending USCG Marine Inspector after sanding; and a repair proposal shall be submitted to address the replacement in kind those found with excessive wastage.

Provide a recent sail and rigging survey report, including the sail vessel dimensions. Sail and Rigging survey report shall include the information as displayed in the USCG NVIC 02-16, appendix 3, enclosure 1.

All port and starboard deck hatch covers have been found with wood decay by separation of the wood fibers in their frames and without means of keep close the hatches. Provide a repair proposal to repair or replace, also to include hatch closing devices.

(a) Port Hull, galley forward outboard side, bottom two stringers, scrape clean of all decay material and clear to good wood, treat with a wood preservative solution. (b) Port Hull, galley from aft bulkhead to first ring frame forward, inboard side, tow bottom stringer, scrape clear of all decay material to good wood, treat with wood preservative solution.

An adult life jacket must be provided for each person carried on board a vessel. (01) Adult life jackets have been found to be in a dilapidated state and not fit for service. Until replaced vessel is only allowed 51 adults on board.

A vessel must comply with the applicable requirements for a station license and installation certificates to be issued by the FCC as set forth in 47 CFR part 80. Vessel is required to have a crew member with a valid FFC operators license.

Any vessel of at least 5 net tons which engages in the Exclusive economic zone, must have a Certificate of Documentation bearing valid endorsement appropriate for the activity in which engaged.

Appropriate for the intended voyage, a vessel must carry adequate and up to date charts and publications. Vessel is missing current tides and current tables.

Unless otherwise specified, vessel owners must pay the fees required before inspection services are provided. Provide documentation of payment to www.pay.gov.

An adult life jacket must be provided for each person carried on board a vessel. Adult life jackets have been found to be in a dilapidated state and not fit for service. Until replaced vessel is only allowed 41 passengers and 03 crew on board.

Child sized lifejackets need retroreflective tape re-newed.

Donning plackard required for each manufacture of jacked.

Port hull FWD bulkhead not sealed.

Several dead end wires in the port hull need to be removed or tagged OOC.

Vessel does not have current state registration.

Portable fire extinguishers have not been annually serviced.

Vessels life floats need servicing/repair/replacement or the vessel will be restricted to no more than 3nm.

Vessel's EPIRB hydrostatic release expired.

Vessel's mainsail boom requires a new gooseneck pin. Vessel is restricted from using mainsail until pin is replaced and satisfactorily inspected by attending MI.

As the use of the mainsail is restricted, drills were deferred until mainsail goosneck pin is replaced.

Vessel is required to provide a copy of the Preventative Maintenance Plan for sails and rigging and all associated logs. Copy was not on board at time of inspection.

Vessel does not have valid COD nor valid State registration.

EPIRB registration sticker expired on EPIRB. (Vessel has current paper registration.)

Placed eight (08) ADULT and two (02) CHILD PFD's out of service. Vessel restricted in passenger count until replacement.

Vessel has new life ring that is not properly marked.

First aid kit had missing and expired items.

The wiring for the forward running lights is not marine grade cable and must be replaced with marine grade cable.

Removed unused and dead ended wiring.

Replace multiple broken light fixtures.

Repair broken hinge on STBD aft hatch

Provide proper number of hand flares. The vessel had all expired hand flares onboard.

Remove rudders on port and stbd side and inspect packing. Too much play when pressure applied to rudder.

Replace weak links (2) to proper vessel capacity prior to sailing beyond 3 NM.
