The sidelights of vessels 20 meters or more in length must be fitted with matte black inboard screens. The inboard screens are not completely matte black.

In a vessel of 12 meters or more in length but less than 50 meters in length the navigation lights must be visible at the following ranges: (i) a mast headlight, 5 miles; (ii) a sidelight, 2 miles. The mast headlight and the sidelights are not visible at the proper intensity.

A vessel of more than 65 feet in length must have navigation lights that meet UL 1104 "Standards for Marine Navigation Lights". The stern light does not meet UL 1104.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Donning placards are not posted.

Each portable fire extinguisher on a vessel must be of an approved type. None of the fire extinguishers on board are fitted with the proper bracket to maintain approval.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew members is expected to perform in an emergency including but not limited to, the emergency instructions listed on the emergency instruction placard. The crew has not been trained on the particular vessel.

Battery chargers must have an ammeter connected in the charging circuit. There is no ammeter installed in the circuit for the battery charger beneath the helm station.

Each lighting fixture and lampholder must be fixed. Light fixture in engine room is not mounted.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Upper deck passenger capacity is not marked.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. 1. Red generator is missing air intake filter and piping. 2. Potable water pressure pump in engine room is not mounted. 3. Overhead insulation is engine room is sagging and missing pieces.

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Electrical penetrations between electrical closet on main deck and engine room are not sealed.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. 1. Sewage connection penetration on starboard side of hull is not watertight. 2. “Y” fitting on starboard side of engine room is open on one side.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Upper deck starboard ring buoy lanyard is wasted.

Each ring life buoy on a vessel must be approved in accordance with § 160.050 in subchapter Q of this chapter. The retro-reflective tape in way of the starboard ring life buoy is not of an approved type.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jacket stowage locker on top deck port side has incorrect number labeled and it shares stowage with adult jackets with no physical divider in place to separate and avoid mistaking. Label proper number of child jackets and divide from adults if using the same locker.

Each nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. A nozzle on a vessel of more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, must: Be of a type approved in accordance with approval series 162.027; or be of a type recognized by the Commandant as being equivalent in performance. Each combination solid stream and water spray firehose nozzle required to be approved under the provisions of this subpart must be of brass or bronze, except for hardware and other incidental parts, which may be of rubber, plastic, or stainless steel, and designed, constructed, tested, and marked in accordance with the requirements of ASTM F 1546 or NFPA 1964. Current fire hose nozzles on vessel are not marked as approved.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems. The red generator on board is missing its intake air filter and rotating belts guard. It also has an oil leak from what appears to be either an oil system pressure or temperature sensor. Replace filter, guards, and repair oil leak.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions; protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; and minimize accidental personnel contact with energized parts. Remedy the following electrical conditions to satisfaction of local OMCI. 1. Top deck aft, GFCI not working as designed. 2. At top deck bar area, remove extension cord acting as permanent cable for a refer and a GFCI under back counter next to refers is not working. 3. Inside of starboard 30 jacket locker on forward top deck, place wire splice into a junction box and remove excessive cable length. 4. In the potable water space, there is a light fixture extinguished in the overhead. 5. In the aft head on main deck, there is a GFCI not working. 6. In the aft galley area, there is a double outlet box needing a cover plate and there is an overhead exhaust fan with no cover and blades exposed. 7. Properly mount light cord beneath main deck bar. 8. There is an open outlet box in the aft main deck accommodation space. 9. Provide an outlet cover for the junction box beneath the galley sink. 10. Properly mount electrical wires in steering space. 11. Provide outlet covers for open outlets beneath second deck bar. 12. Remove dead end wiring in the space under main deck bar. 13. Provide an outlet cover for the electrical outlet in the closet. 14. The shore power cable connection in the main deck switchboard closet is not fully seated in the stuffing tube.

At each inspection for certification all observed unsafe practices must be corrected. The ice machine in the main deck galley is not secured for sea.

At least one ring life buoy must be fitted with a life line which must: be buoyant; be at least 60ft in length; be non-kinking; have a diameter of at least 5/16 inch; have a breaking strength of 5 kilonewtons; be of a dark color if synthetic or of a UV resistant.

The owner of a vessel subject to inspection under 46 U.S.C. 3301 must comply with the Master [name withheld] requirements 46 U.S.C 3106 - Master [name withheld] The vessel does not comply with 46 U.S.C. 3106 in that the vessels bathrooms are accessible to others without a Master Key.

Protective coverings or shields shall be installed to the satisfaction of the local OCMI. -The FWD genset exhaust hose is chaffed on a overhead frame and is not secured properly. -The raw water suction hose for the PORT main engine is chaffing near the engine bed.

The main propulsion machinery installed on each vessel is suitable for the vessel and its operation. The STBD main engine heat exchanger filler neck is broken at the over flow fitting.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire and MOB. The procedures are not posted.

While Conducting Def Check, it was identified that severe pitting is occurring throughout hull plating from the stern tubes aft affecting all areas at or near propellers. Vessel operator to conduct NDT to determine severity and amount of hull wastage develop repair plan and submit to MSU Chicago for approval prior to commencing any repair. Vessel is not authorized to Operate with passengers until New to Zone COI is completed.

A tree became lodged in the vessels rudder/propellar impacting the vessel's ability to manuver. A visual inspection or underwater survey documenting condition of the vessel rudders and or propellars. A underway operational test of the vessel's ability to safely manuever. All inspections and test will be to the satisfaction of the attending marine inspector.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing personnel injury. Port engine wet exhaust piping leaking from fiberglass elbow. Repair elbow utilizing a fire retardant FRP system.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire. Starboard engine #2 fuel nozzle leaking fuel. Repair leak as required.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing machinery failure. Starboard engine mounts are loose allowing the starboard engine to shift approximately 1" when shifting from forward to aft. Repair engine mounts as required.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing personnel injury or machinery failure. Starboard engine belt guard loose allowing guard to come in contact with moving parts. Repair as required.

Gages to indicate cooling water discharge temperature and lubricating, oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Starboard engine lube oil pressure gauge not operating at operators station. Repair as required.

Electrical equipment on a vessel must be installed and maintained to protect the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead end battery cable found by starboard engine. Remove dead end cable as required.

A marine employer shall comply with the requirements of §4.06 in subchapter A of this chapter. The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel is not equipped with alcohol test equipment required. Provide test equipment as required.

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel missing station license. Provide station license as required.

Each individual referred to in paragraph (a) of this section must hold an MMD or MMC that serves as identification, with an appropriate endorsement for the position in which the seaman serves, and the MMD or MMC, along with a valid medical certificate. Vessel Master was unable to present a valid medical certificate. Provide a medical certificate as required.

Severe side shell corrosion and steel wastage identified behind the starboard rub rail IWO the gangway. Severe corrosion and steel wastage identified on the main weatherdeck above the forepeak and #2 void. Severe corrosion and steel wastage identified on the main deck throughout the vessel affecting all other voids and spaces. Severe corrosion and steel wastage identified to the port, starboard and stern bulkheads above the sheer line. Crop out to good steel all areas identified by the attending CG Marine Inspector and renew to original condition. Conduct satisfactory NDT of all welded areas exposed to the exterior of the vessel in the presence of the attending CG Marine Inspector.

2 windows on main deck in passenger accommodation area discovered with cracks that pose a hazard to passengers and crew. One port side amidships, and one starboard side forward of amidships. Both windows shall be renewed prior to departure

Fixed bilge de-watering system inoperative, crew not properly trained in system function. Prove proper operation of bilge de-watering system in the presence of the attending CG [name withheld] label all associated lines and valves at manifold, train crew in proper operation and post operational instructions.

Fire drills unsatisfactory and no documented evidence of prior training available. Crew performance did follow written procedures. Crews were unaware of responsibilities, hesitant, or demonstrated inability to act. Perform fire drills and properly document to the satisfaction of the attending CG MI.

Master and Crew observed using cellular phones for internal and emergency communications. Observed operations were intermittent and/or delayed creating uncertain tranfer of information and at times no communications. Provide proper and adequate internal communications to the satisfaction of the attending CG MI.

Emergency instructions do not contain current or accurate contact information. Revise written emergency instructions to reflect current reporting and modification information.

No proof of Management Information System requirements under the Chemical Testing program. Provide proof of data reporting to the MIS Data Collection program.

New installation of the Type III Marine Sanitation Device, and associated piping was installed without prior approval from the cognizant [name withheld] system appears to be two times total capacity than that observed during last inspection. Submit replacement plan to include total weight changes and materials used for review and approval.

Lifejacket location markings throughout vessel are not readily visible. Clearly mark all lifejacket locations in accordance with IMO Resolution A.760(18) or 46 CFR 185.604(f).

Port and Starboard propulsion engines have loose or missing fasteners on the forward rotating g machinery guards. Properly fasten machinery guards.

Watertight electrical enclosure in #2 void has cable inserted directly into box without clamp or strain relief. Install cable into box with appropriate cable fitting as identified by the attending CG MI.

Electrical connection air compressor has exposed conductors. Properly secure electrical supply cable to the satisfaction of the attending CG MI.

Electrical outlet cover in the #2 void is not properly installed. Properly secure electrical outlet cover in the #2 void as identified by the attending CG MI.

No portable lighting at the machinery space access. Provide adequate portable lighting to the satiscation of the attending CG MI.

Open piping in watertight bulkhead, port side upper, in forward engine room. Properly close open piping to the satisfaction of the attending CG MI.

No proof vessel is in compliance with the authorization to discharge under the National Pollutant Discharge Elimination System, small Vessel General Permit (sVGP). Provide documented proof of compliance.

Crop and Renew wasted main deck and transverse deck beam identified by the attending [name withheld] approved plan.

Starboard Aft Mooring Cleat Mount damaged from previous event. Make permanent repairs to cleat and mounting surface to the satisfaction of the attending CG MI.

Submit repair proposal to crop out and renew wasted main deck and transverse deck beam in #3 void IVO MSD as identified by the attending CG MI.
