A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years; Vessel notified Sector VA their intentions to stop the current dry dock exam to relocate the vessel. Vessel's dry dock exam must be completed prior to carrying passengers. Vessel's owner/operator must request permission from Sector VA to transit outside Sector VA OCMI zone. 46 CFR 176.600(c)(1)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. The alcohol swabs on board expired May 2025. 46 CFR 4.06-15(a)(1), 16c

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Observed two unsecured/improperly secured pressurized cylinders that are used for the soda dispenser. 46 CFR 176.830, 50c

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Several portable fire extinguisher brackets were not approved or no longer properly secured the extinguisher. 46 CFR 181.520, 50c

An adult life jacket must be provided for each person carried on board a vessel. One life jacket was found with the outer layer torn exposing the inner core. Reduced adult carriage to 133. Crew provided a spare adult life jacket, inspected satisfactorily and had proper marking. Cleared 06AUG2025. 46 CFR 180.71(a), 705c

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's stability letter. As per the vessel updated stability letter (H1-2501205) dated April 11, 2025, the vessel is required to install 2,600 pounds of fixed lead ballast ingots located in accordance with the vessel’s ballast plan dated February 11, 2025. 46 CFR 185.315 (a)

When the cognizant OCMI or the Commanding Officer, Marine Safety Center determines, in accordance with § 178.210, that a vessel must have stability details indicated on its Certificate of Inspection or a stability letter, the owner or operator must submit the information listed in paragraph (b) of this section to the Commanding Officer, Marine Safety Center for approval. The Marine Safety Center has requested revised calculations from the owner prior to issuing the stability letter. The current stability letter is not valid. 46 CFR 178.230(a)(2)

46 CFR 10.301(a): The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Vessel’s captain’s medical certificate was not available for inspection onboard.

46 CFR 67.7: Any vessel of at least five net tons which engages coastwise trade must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Vessel’s COD is expired.

46 CFR 184.420(a)(1):As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel’s paper charts were three years out date. Vessel’s electronic charts had not been updated for an unknown length of time.

46 CFR 180.78(a)(5): Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The aft upper deck lifejacket stowage location did not have separation between child and adult jackets.

46 CFR 185.506(a): Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following: (1) The location of emergency exits, (4) The location of the instruction placards for life jackets and other lifesaving devices; (5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master Vessel’s safety orientation script did not include information described in 46 CFR 185.506(1),(4), and (5).

46 CFR 185.604(f): The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The aft upper deck life jacket stowage location was not properly marked indicating number and size.

46 CFR 185.612(a): Complete simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. Each control valve to a distribution line must be marked to indicate the space served. Vessel did not have instructions detailing the operation of the fixed gas fire extinguishing system.

46 CFR 180.70(c): At least one ring life buoy must be fitted with a lifeline. Each lifeline on a ring life buoy must: (1) Be buoyant; (2) Be of at least 18.3 meters (60 feet) in length; (3) Be non-kinking; (4) Have a diameter of at least 7.9 millimeters (5/16 inch); (5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and (6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Vessel did not have a lifebuoy fitted with a lifeline.

46 CFR 185.710: A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel’s first aid kit included expired equipment. Replaced on the spot.

46 CFR 180.68(b)(1): A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter. Vessel’s hand flared were expired. Replaced on the spot.

46CFR185.602(c)(1)&(2): Each vessel must have permanent draft marks at each end of the vessel; or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft.

Stability information (stability details indicated on the Certificate of Inspection, a stability letter), is required on certain vessels. Enough stability information, including stability calculations and assumptions made to use them, must be provided to allow the master to be able to determine operating guidelines, loading restrictions, and ensure compliance with the applicable intact and damage stability regulations of this chapter. The unrecorded weight change of the vessel has led to a light ship displacement exceeding 3%, with a total estimated weight change of 20% since the last stability test. Accordingly, the parameters under which this stability letter was issued has been altered; therefore, the current stability letter is invalid. 46 CFR 178.210(a)

There must be a means of accurately determining the amount of fuel in each fuel tank either by sounding, through a separate sounding pipe or a fill pipe, or by an installed marine type fuel gauge. Vessel has no means of determining the level of fuel in tank. 46CFR182.445(b)

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or Vessel did not have non-combustible trash cans on board. 46CFR177.405(f)

Must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. Collision bulkhead has a penetration that is not watertight. 46CFR179.310(a)(2)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Through-hull opening on the port side shell of steering space with no pipe, cap, or valve in place. 46CFR177.100

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). A second power pump is an acceptable alternative to a hand pump if it is supplied by a source of power independent of the first power bilge pump. No portable hand bilge pump or second power pump on board. 46CFR182.520

Capable of moving the rudder from 35 degrees on one side to 30 degrees on the other side in not more than 28 seconds with the vessel moving ahead at maximum service speed. Rudder only went to 25 degrees to port. 46CFR182.610(a)(3)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. No FCC Safety Radiotelephony Certificate on board. 47CFR80.903

The Bridge-to-Bridge Act and the regulations of this part apply to the following vessels in the navigable waters of the United States: (a) Every power-driven vessel of 20 meters or over in length while navigating. No FCC bridge to bridge certificate on board. 47CFR80.1001(a)

A vessel of 12 meters or more in length shall be provided with a whistle, a vessel of 20 meters or more in length shall be provided with a bell in addition to a whistle. Bells and gongs shall be made of corrosion-resistant material and designed to give clear tone. The diameter of the mouth of the bell shall be not less than 300 mm for vessels of 20 meters or more in length. No bell was on board. 33CFR83.33(a)

Except as otherwise provided in this section every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. (b) The following vessels need not be fitted with a compass: (1) A vessel on a rivers route; (2) A non-self propelled vessel; and (3) A vessel operating on short restricted routes on lakes, bays, and sounds. (c) Except on a vessel limited to daytime operations, the compass must be illuminated. Compass light inoperative. 46CFR184.402(c)

Each hatch cover must— (1) Have securing devices; and (2) Be attached to the hatch frame or coaming by hinges, captive chains, or to other devices to prevent its loss. Forward hatch cover has no securing device. 46CFR171.124(b)(2)

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical outlets missing covers or not installed correctly in the womens head, galley and upper deck. As well as lighting that was hanging by its wires in galley,upper deck, and compartment three. Ventilation motor in mens head missing cover. 46CFR183.200

All cable and wire must: (1) Have stranded copper conductors with sufficient current carrying capacity for the circuit in which they are used; (2) Be installed in a manner to avoid or reduce interference with radio reception and compass indication; (3) Be protected from the weather; (4) Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Multiple areas in engine room and steering compartment where electrical wires were supported with plastic zip ties. 46CFR183.340

All fuel tanks must be electrically bonded to a common ground. Fuel tank is not bonded or grounded. 46CFR182.440(b)(4)

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Such hose must overlap metallic pipe ends at the least 11/2 times the pipe diameter and must be secured at each end by clamps. The flexible section must be accessible and as near the upper end of the fill pipe as practicable. When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. Fuel fill pipe/hose not joined by a conductor. 46CFR182.445(g)

Each device must— (1) Be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow toxic substances to escape to the interior of the vessel; (2) Be vented or provided with a means to prevent an explosion or over pressurization as a result of an accumulation of gases. At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Portable air compressor in compartment two is not secured to anything. 46CFR176.830(a)

Each ring life buoy on a vessel must: (1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150; (2) Be readily accessible; (3) Be stowed in a way that it can be rapidly cast loose. The forward life ring buoy was not stowed in a location. 46CFR180.70(a)(3)

In accordance with CG-543 Policy Letter 11-03 (Dated April 8, 2011), submit certification information to Sector Miami OCMI, with a copy to the MSC, certifying compliance with the new Final Rule regarding Assumed Average Weight Per Person (AAWPP) of 185 lbs. MSC will review and verify the information provided and issue an addendum to the existing stability letter or a new stability letter.
