One wheel foam extinguisher (45hr), one fixed dry powder (40hr), and one foam applicator were beyond the required annual inspections date as per the vessels SMS. The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In addition to the deficiencies below, a Liberian flag inspector attended 1-hour prior to the start of PSC exam & cited 02 deficiencies including inoperable smoke detectors in the engine room and oil leakage and accumulation in the machinery spaces, and it was determined the vessels crew knew of a faulty heat detector testing appliance and stated that a new requisition was submitted. The crew was not able to provide the requisition or any evidence of purchase, as required by the ISM Code and the vessel’s SMS. The function of the detection system shall be periodically tested to the satisfaction of the Administration by means of equipment producing hot air at the appropriate temperature. Vessel was found to be using an open flame torch to test heat detectors, which contradicted SMS procedures and manufacturer instruction and may have damaged vital fire detection equipment rendering it incapable of detecting a fire. 74 SOLAS (20) II-2/14.2.1.2. 30 ABC

Oil fuel pipes, which if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 l and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. The quick closing line handle that is located outside of the EDG space broke during testing not allowing the generator to be shut off remotely in case of fire. 74 SOLAS (97) II-2/15.2.5. - 30 ABC

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. 01 portable rolling CO2 applicator found to be covered with oil, and 01 foam applicator found with a kinked hose thus rendering them inoperable or potentially ineffective. The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts. Vessel’s generator shafts were found to have exposed rotating machinery which could lead to injury as well as damages to loose rubber hoses near the exposed area. 74 SOLAS (04) II-1/26.1 - 17AC

Class A fire boundary doors leading to engine space and steering gear room are fitted with 2 kinds of hold backs: lines and metal hooks. Unauthorized hold backs on Class A boundaries pose severe threat to required containment of shipboard fires. 74 SOLAS (97) II-2/47.2 - 17 AC

The arrangements for the storage, distribution and utilization of other flammable oils employed under pressure in power transmission systems, control and activating systems and heating systems shall be such as to ensure the safety of the ship and persons on board. Steering gear piping was found to be leaking hydraulic oil at a flange above the steering drive motor, creating a hazard. 74 SOLAS (97) II-2/15.4 - 17AC

PSCO IDENTIFIED A RAW WATER PIPE IN THE ENGINE ROOM BENEATH THE DECKPLATE WITH A HOLE IN THE SIDE OF THE PIPS THAT WAS SPRAYING A CONSTANT STREAM OF WATER INTO THE BILGE.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Port side lifeboat lifeline was missing one bolt. Creating an opening in the hull.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. The overhead insulation in the main engine space near the air tanks was damaged. 40AC
