A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Each first-aid kit must meet the requirements in ISO 18813:2006(E) paragraph 4.12. A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel's fire buckets were 3.5 gallons. //46 CFR 181.610//

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Letters and numbers must be at least 50 millimeters (2 inches) high. Vessel did not have proper amount of PFDs marked. 46CFR185.604(f)

For a life float - approved under approval series 160.027. Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. The material must be in strips at least 50 mm (2 in.) wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. For peripheral body apparatus, each strip must extend completely over the top and bottom surface of the body. For box type apparatus, the strip must extend at least 300 mm (12 in.) inboard from the edge over the top and bottom surface. Each strip must be positioned near the center of the side or end, but so that it is not obscured by any strap. 01 life float had unapproved reflective tape. 46CFR180.200(a)(2)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. MI condemned 06 child and 05 adult life jackets. 46CFR180.175(a)

As appropriate for the intended voyage, a vessel must carry adequate and up-to date: Coast Guard Light List: Light List is outdated.

Vessels bilge alarm wire's are in the bilge and not in a junction box, vessel must place connection as high as possible in a junction box.

Vessel was missing its Coast Pilot and had an outdated chart.

All three float lights onboard had expired batteries.

EPIRB had an expired hydrostatic release unit.

Owner/Operator failed to submit stability certification compliance by 01DEC11. Issued CG-835 requiring vessel operator to submit new calculations for stability based on the assumed average weight per person of 185 pounds and in accordance with 46 CFR 178.320 amended by FR vol. 75, No. 239, USCG-2007-0030. OCMI did not reduce passenger count as COI restrictions place them in compliance.
