HEAVENLY DAYS

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Crew member was not proficient with MOB recovery. MI administered training on the spot and Master followed up with additional instruction. 46 CFR 185.520(a)

A bilge pipe in a vessel of not more than 19.8 meters (65 feet) in length must be not less than 25 millimeters (1 inch) nominal pipe size. A bilge pipe in a vessel of more than 19.8 meters (65 feet) in length must be not less than 40 millimeters (1.5 inches) nominal pipe size. A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Hand bilge pump line was partially air bound due to incorrect fitting size. Replace fitting with appropriate size. 46 CFR 182.510(b)

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must:(1) Be buoyant;(2) Be of at least 18.3 meters (60 feet) in length;(3) Be non-kinking;(4) Have a diameter of at least 7.9 millimeters (5/16 inch);(5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and(6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Lifeline was worn, replace with new. 46 CFR 180.70(c)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Stbd fuel tank COV partially wasted, replace. 182.455(b)(4)

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Bow anchor plate was wasted, replace. 46 CFR 184.300

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Sailing catamaran ran aground and suffered hull damage to stbd hull. Temporary repair was applied and water intrusion was controlled. Provide plan for transit to Subbase Drydock. Provide full damage assessment report and submit with repair proposal to USCG for review and plan approval prior to making permanent repairs. Sea trials may be required at the discretion of the attending MI. 46 CFR 176.700

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. HEAVENLY DAYS suffered damage to the starboard rudder adversely affecting the maneuverability of the vessel. Complete all necessary repairs, provide a report addressing the scope of work completed and provide any associates invoices/ receipts. Upon receipt of these documents, marine inspectors will schedule a follow-up exam to conduct sea trials on the HEAVENLY DAYS. 46 CFR 176.700(a) 46 CFR 176.700(d)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. All inspected sailing vessel owner/operators should have a written Preventive Maintenance Plan (PMP) that specifically addresses their vessel’s sail rigging equipment and components. Vsl manager was not able to provide PMP IAW NVIC 2-16 and there is not an indication of PMP implementation. 46 CFR 176.802(a)(3)

Third party rigging survey dated 12SEP2023 noted "Ugly surface corrosion without evidence of leaking" on chain plate. O/O polished chain plate for re-inspection. MI's noted evidence of water intrusion on port chain plate due to corrosion IWO fasteners. During re-inspection, MI's noted corrosion IWO lower Fore-stay chain plait/lug where it is embedded into cross member. Provide repair proposal from qualified third party rigger. 46 CFR 176.802(a)(3)

Forestay chainplate and port/starboard shroud chainplates must be removed and replaced due to the age of the vessel combined with no documented history of inspection/replacement since vessel construction circa 1991.

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of the vessel. MI's observed sea water seeping through the lower deck of the port fwd void space in the vicinity of the waterline. Vessel must determine the source of the water ingress and ensure the watertight envelope is not compromised in that compartment. 46CFR 177.10-1

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments under all practical conditions. Starboard fixed hand bilge pump wasn't permanently attached to allow interconnection of the pump to the bilge piping of each individual space. Bilge portable hand pump was also missing. 46CFR 182.25-1 46CFR 182.25-10(d)

On or before March 11, 1999, an existing vessel must comply with the bilge high level alarm requirements in § 182.530. Port and Stbd fwd voids did not have visual and audible alarms installed. Vessel shall install audible and visual alarms for void with non-watertight closure on the main deck. 46CFR 182.115(d)

On or before March 11, 1999, an existing vessel must comply with the bilge high level alarm requirements in § 182.530. Stbd head bilge alarm did not provide audible or visual alarm at the operating station. Vessel shall make repairs as necessary. 46CFR 182.115(d)

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. MI discovered that all bilge suction points were missing foot strainers. Vessel shall install strainers as required. 46CFR 182.25-5(b)

At each initial and subsequent inspection for certification, the marine inspector shall inspect all lifesaving equipment to determine its serviceability. Attending MI’s noted that 31 of the adult life jackets were unserviceable. 46CFR 176.25-22(a)

Each ring life buoy must be of an approved type. Ring life buoy did not have any approval markings or approval stamps. MI observed that ring life buoy had unsatisfactory repair that allowed water intrusion. Retro reflective material was faded, peeling off, and vessel’s name was not permanently marked. Vessel shall replace ring life w/ USCG approved type. 46CFR 180.30-1(a)

All life preservers shall be marked with the vessel’s name. The required child’s life preservers were not marked with the vessel’s name. 46 CFR180.25-15

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel crew did not log EPRIB monthly tests. 46CFR 185.728

Flexible nonmetallic hose may be used in lube oil, fuel oil and fluid power systems, subject to the following requirements:(B) Flexible fuel hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Attending MI’s noted that several of the hose clamps were wasted or absent from the fuel filters and flexible fuel hoses. 46CFR 182.720(e)(3)

Fuel lines shall be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal straps with no sharp edges. Attending MI’s noted that the control arm that links the port and starboard rudder was making contact with the fuel supply lines to the vessels outboard engines. Re-route fuel hoses away from pinch and chafe points of the steering components. 46CFR 182.14-40(b)(2)

Main steering apparatus. All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. Attending MI’s noted that the bolt and nut retaining the steering linkage to the port outboard engine was insufficient. Replace with adequate bolt/nut combination. 46CFR 182.30-1(a)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. MI’s noted that the ladder accessing the starboard forward void was missing mounting hardware; which could hinder escape from the space or cause and injury while entering. 46CFR 177.01-5

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. MI observed multiple incomplete kits with wet contents. 46CFR 184.710

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel could not produce evidence of satisfactory fire drills being conducted. 46CFR 185.524(f)

Inspection shall be in accordance with standards acceptable to the OCMI as good marine practice. Lines that secure forward trampolines to the hull are worn and frayed. 46CFR 176.25-1

Electrical installations and equipment should be installed so that they will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment and reasonably safe from electrical shock of personnel. Electrical cables for both outboard motors were dragging in the water because it was not properly secured to the hull. 46CFR 183.01-5

Vents must be designed and constructed to minimize clogging by the contents of the tank. Sewage tank vent was broken off in the fwd port void space. 33CFR 159.61

If a vessel operates on exposed or partially protected waters, an opening port light is not permitted below the weather deck unless it has an inside, hinged dead cover. Port light in forward void is missing a lens and dead cover. 46CFR 171.119

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either a pressure type connector on each conductor; a solder lug on each conductor; a splice made with a pressure type connector to a flexible lead or conductor; or a splice that is soldered, brazed, or welded to a flexible lead or conductor. Sewage tank discharge pump wiring connections were connected by twist caps. 46 CFR 183.01-15(a) 46 CFR 183.340(i)(1)

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. The COI was not posted. 46 CFR 176.302

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Electrical wire in stbd storage head space was utilized to hang extra lifejackets, putting strain on the wire. 46CFR 176.800(a)

Whenever damage to hull plating or structural members that may affect the seaworthiness of a vessel is suspected, the OCMI may conduct an internal structural exam in any affected space and may require the vessel to be drydocked or taken out of service to assess the extent of the damage and to affect repairs. Master notified USCG that the vessel struck a submerged object and or grounded which caused a loss of steerage and caused restricted maneuverability. Provide underwater damage survey to OCMI or attend drydock for USCG hull inspection prior to operating with passengers.
