On a vessel of at least 7.9 meters (26ft) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with through hull fittings below the waterline, a machinery space, a space with a non watertight closure and vessel constructed of wood must have bilge alarms in all spaces below the main deck. Vessel operator is unable to prove bilge alarms. Vessel owner will provide a third deck hand when underway with passengers. The third deck hand will have no other duties other than checking compartments to ensure no water intrusion and to give the alarm in cases of flooding. 46 CFR 182.530

The alcohol testing of each individual must be conducted within 2 hours of when a serious marine incident occurred. Vessel operator did not have a means to test for alcohol on board the vessel during night voyages when testing facilities are closed. Provide a means to test to the satisfaction of the attending MI. 46 CFR 4.06-3 (a)(i)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio Beacon installations, including the requirements for a station license and installation certificates to be issued by the FCC. Vessel does not have an FCC station license or proof of certification of the VHF radio. Vessel currently has two different MMSI numbers on the VHF and the AIS. Correct to the satisfaction of the attending MI. 46 CFR 184.502

The owner, charter, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months. Training shall be logged or documented to be provided to the USCG when requested. Vessel owner will start a log and document training. 46 CFR 185.420

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers. Hoses must be lined commercial fire hoses that conforms to UL 19 or hose that is listed and labeled by an UL recognized by the Commandant as being equivalent. Vessels fwd fire hose has a hole causing it to leak, new hoses has been ordered and is en-route. 46 CFR 181.320 (b)

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers...the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. Although an effective stream was produced, fire pump not capable of meeting requirements.

Each fire hydrant must have a valve installed to allow the firehose to be removed while the fire main is under pressure. Forward fire hydrant valve frozen and not capable of being closed.

Fuel shutoffs for vessel found under wood plank however wood plank hand position made it hard to operate valves in an emergency. 46 CFR 184.620

A voltmeter and ammeter shall be provided for measuring the voltage and current for each generator. DEF: Ammeter is broken. 46CFR183.10-5(g)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria the maximum number of passengers allowed must be indicated by a durable marking of at least 1 INCH numbers and letters at the entranceway to that deck. 46CFR185.602(g)

Vessel is restricted to carrying no more than 84 passengers total, and no more than 6 passengers on the upper deck for trips scheduled for 02 and 04 April, 2017.

Prove weight summary calculations from Marine Safety Center.

KK01 - Replace 2 adult PFD's and carry a total of 136 obboard at all times.

KK02 - Install metal wire supports for all electrical wiring on vessel.

KK03 - Replace foward BII fire extinguisher bracket w/ approved type.

KK04 - Install non conductive mat in front of switch board in bar.

Provide a current Certificate of Documentation to reflect the new owner and operator prior to carrying passengers. When received, Certificate of Documentation must be posted or available on board at all times when the vessel is underway.

Provide an updated Coast Pilot for the area transited and a copy of the Inland Navigation Rules prior to carrying passengers.

Provide proof of enrollment in a drug testing program and make available drug testing records for the crew prior to carrying passengers.

Provide one dual purpose and remove the suicide nozzle on the forward fire station. Fire Nozzle to meet the requirements of 46 CFR 181.320(d).

Provide a first aid kit that is complete and has all the contents as required in 46 CFR 160.041.

Properly mount fire axe in the vicinity of the operating station.

Remove dead end wires in the wheel house console in the vicinity of the magnetic compass.

Replace one inoperable water light IWO the ring buoy on the second deck. Crew provided a new water light that was tested satisfactory.

Repair bilge high level servicing the fuel tank space and demonstrate proper operation prior to carrying passengers.

Provide and post emergency instructions that meet the contents of 46 CFR 185.512.

Repair or replace broken pressure gauge IWO the fire pump.

Ensure all flexible hoses for the raw water exhaust system are within the requirements of 46 CFR 182.720(e)(3)(v). All the flex hoses measured more than the maximum 30 inch limitation.

Remove angel hair to good wood on two I- Beams and associated frames located in the shaft stuffing box for both the port and starboard MDE.

Remove oily water mixture from the bilge below the generator on the port MDE and properly dispose.

Replace rotten wood gussets used as a funnel for the shaft stuffing box drainage system and repair broken PVC piping in association with the drainage system.

Provide metallic fasteners for all overhead wiring in the engine room that meets the requirements of 46 CFR 183.340(b)(4).

Properly mount or remove two gauges on the port and stbd MDE’s. The gauges are wired into the sensors on the engines and the wires are run through the overhead insulation and left hanging. Gauges are for the jacket water discharge temperature.

Make repairs to the RPM indicator gauge located at the operating station for the port MDE.

Submit Fixed CO2 Fire Extinguishing system plans to local OCMI for review and approval.

Make proper repairs to frame #3 at port and starbord ends in forepeak.

Make proper repairs to vertical bulwark support and to associated frame at hull on starboard side.

Provide flame screens at fuel tank vents.

Replace 110 volt duplex outlet cover at pump control in the tank void.

Eliminate dead ended cables in tank void.

Renew wasted carriage bolts in the aft void.

Prove proper operation of fixed bilge system in all spaces.

Make proper repairs to underwater hull where indicated.

Properly install electrical wires and breakers in aft utility locker. Eliminate deadended wires, make proper splices, replace broken swithc covers. Provide fixture cover for light fixture.

Provide detailed legend of breakers purpose at aft breaker box.

Re-new broken cover at duplex outlet in forward storage locker.

Provide floatable line for float light at life ring.

Provide placards at life jacket lockers throughout boat.

Provide placards/signage at fore and aft ladderwells indicating 38 passengers maximum allowed on upper deck.

Replace bilge high level alarm (visual) covering engine room. Bulp is broken and needs repair.

Replace batteries in emergency lighting fixtures and prove proper operation.

Provide CPR certification for at least 50% of crew.
