The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessels management self reported that they are experiencing the beginning of what has been deemed a port stern tube forward seal failure. Mitigation efforts have been put into place and appear to be effective. Make repairs once a return to port has occurs.

Operational readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. During MOB drill for inspection with USCG, rescue boat could not be operated in forward propulsion. ASP was able to repair by the end of the inspection. 46 CFR 199.190(a)

Vessels must be equipped with independently driven fire pumps in accordance with Table 95.10-5(a). During testing of the fire pumps during USCG inspection, port side fire pump gasket failed. Crew made repairs prior to end of inspection. 46 CFR 95.10-5(a)

The minimum instrumentation, alarms, and safety controls required for specific types of systems are listed in Table 62.35-50: Bilge Level – High/Location. During testing for USCG inspection, 06 bilge alarms were inoperable. Crew was able to repair by the end of the inspection. 46 CFR 62.35-50

If the prime mover is a diesel engine, there must be an audible alarm that sounds on…high cooling water temperature. During inspection with USCG, high temperature jacket water temperature was not alarming. Crew swapped out the inoperable thermocouple and demonstrated alarm notification as per revised PSTPs prior to end of inspection. 46 CFR 112.50-1(h)

Each space used by…crew must be fitted with lighting that provides for safe habitable and working environment under normal working conditions. Marine Inspectors found several lights that are out about the vessel. Identify all non-operational lights and make repairs to satisfaction of OCMI. 46 CFR 111.75-15(c)(1)

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. During annual inspection and acceptance of PSTPs, it was found that PSTPs do not include testing or remote operated fire dampers which is part of the fire extinguishing system. Submit revised PSTPs and receive MSC approval prior to unmanned watches implemented. 46 CFR 61.40-6

Vessel authorized a temporary repair utilizing an all-around flashlight until permanent repairs are made. 46 CFR 199.190(a)

The symbols used to identify the aforementioned details shall be in accordance with IMO Assembly resolution A.654(16). Crew stateroom 2-146-2 door does not have sticker for fire damper that is inside. 46 CFR 97.36-1(a)

In general, connections and terminations to all conductors must retain original electrical, mechanical, flame-retarding, and where necessary, fire resisting properties of the cable. Marine inspector found the old wiring from the starboard speed log preamp still in place and with exposed conductors. Remove all dead-end wiring in the associated space to satisfaction of OCMI. 46 CFR 111.60-17(a)

Cylinders must always be secured, and when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. There were several cylinders stowed in various locations throughout the ship that did not fit into their designed braces, did not have their wood bases, and or not secured properly and were next to or nearby the rack, but not in the rack. Inventory all required compressed gas, make racks required and stow cylinders at next maintenance period to satisfaction of OCMI. 46 CFR 147.60(b)(1)

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. During inspection for COI annual, Marine inspectors found the port side water heater has started leaking and is leaving the deck wet and slippery in the space. Make permanent repairs to satisfaction of OCMI. 46 CFR 91.25-50

Vessel's captain reported that the shipboard maintenance system (SAMM) in inoperable. the current SAMM system issue renders the crew unable to effect repairs in the event of an emergency and located necessary replacement parts. SOLAS 1/11(a)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order ready for immediate use. USCG FEACT received a 2692 from the vessel master stating that the fast rescue boat was unable to start. Once the crew placed a temp battery charger on the boat it was able to be started. Root cause was the failure of installed battery charger. Replacement has been ordered.

A self-contained emergency source of electrical power must be provided and the transfer of power must be accomplished in no more than 45 seconds after failure of the normal source of power. The emergency diesel generator's switchboard breaker must be manually charged as automatic charging system is faulty.

The condition of the ship's equipment shall be maintained to ensure the ship will remain fit to proceed to sea without danger to the ship or persons on board. There is an oil leak on the #3 SSDG in way of the turbo, causing accumulation of oil on and around the outside of the turbo. #3 SSDG has been tagged out.

The construction of doors and door frames in "A" class boundaries shall provide resistance to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated. Corrosion has caused the door frame, and adjacent bulkhead, on the starboard anchor windlass room to become detached from the deck. Temporary repair has been made. Permanent repair to be made during January 2024 MTA.

The condition of the ship's equipment shall be maintained to ensure the ship will remain fit to proceed to sea without danger to the ship or persons on board. Mounting bolt on #3 SSDG duplex fuel filter is loose; power cable in way of P/S auxiliary machinery room exit to be secured/supported; SP107 high voltage -overhead distribution panel is missing 8 bolts in the panel, causing dust and debris to collect inside the panel; multiple non-marine grade extension cords were found servicing equipment, satisfactory permanent electrical connections need to be made; reach rods for multiple ballast tank foot valves (internal) are inoperable; Multiple fire station's hose cabinets have missing or broken latches for securing the doors.

Nonmetallic flexible hose may be used in lube oil, fuel oil, adn fluid power systems in lengths not exceeding 30 inches. All SSDGs are using flexible hoses for diesel fuel drain lines which exceed 30".

Lifebuoys with lights shall be distributed as to be readily available on both sides of the ship. Lifebuoy on port side main deck has been mounted in an improper location, not allowing the berthing port-light to be secured in the open position without trapping the light behind the port-light cover. Cover was found wired open, light pinned behind, not allowing for immediate deployment.

Doors and door frames in "B" class divisions shall have a resistance to fire equivalent to that of the divisions. Door between crew's messroom and recreation room has multiple holes drilled and the door structure is fractured at the latch.

Main propulsion and auxiliary machinery that are automatically or remotely controlled must be provided with an effective primary control and a safety control system. Starboard steering #2 low lube oil alarm is indicating locally but not in the control room; port propulsion field loss indicator alarm is inoperable; port steering room alarm strobe is inoperable.

General arrangement plans shall be permanently exhibited showing clearly the various fire sections enclosed by "A" and "B" class subdivisions. Several spaces on the fire control plan are not properly labeled as "A" boundaries. Initial review indicated the galley and port and starboard fan/anchor windless rooms are indicated as "C" class spaces. A thorough review of the plan and all spaces needs to be conducted, resulting in a revision and submission to the USCG for final review and approval.

Periodic safety test procedures do not have instructions for testing of the emergency diesel generator's safety alarms and shutdowns.

All reasonable steps shall be taken to maintain the radio equipment in efficient working order to ensure compliance with all the functional requirements. Malfunction of the equipment for providing general radiocommunications shall not be considered as making a ship unseaworthy or as a reason delaying ship in ports where repair facilities are not readily available, provided the ship is capable of performing all distress and safety functions. Ship self-reported failure of NAVTEX (part of GMDSS suite) due to corroded exterior antennae connector. Repairs due at next port. Mitigation plan: Safety messages to be received via SAT "C" for NAVAREA 11, and backup means via Japanese official government website for navigational area messages. 74SOLAS(2014)IV/15.8

Each vessel on an international voyage shall carry at least 2 firemen's outfits. Damage Control Fire Team's outfits require servicing and adjustment for communication radios. Satisfactory servicing and synchronization required for radio communication equipment. 46 CFR 96.35-10(b)

Internal combustion engine exhausts shall be suitability insulated from combustible matter. MIs noted ME exhaust insulation replacement overdue in maintenance for stack area. Satisfactory insulation replacement required. 46 CFR 92.05-5(a) Extended to May2023 repair period (VRA)

Enclosed spaces within the vessel shall be properly ventilated and means shall be provided to close all vents & ventilators. MIs noted corrosion by ventilation trunk (aft bridge) and corroded vent closure (starboard deck crane). Satisfactory steel repairs required. 46 CFR 92.15-10(a) Temporary repair, Extended to May2023 repair period (VRA)

At each COI & Periodic inspection, MI shall examine machinery spaces to inspect accumulation of oil which might create a fire hazard. MIs discovered substantial hydraulic oil leak for HPU serving SURTASS emergency winch & controls. Oil leak to be cleaned to prevent fire hazard.

COI inspection shall include inspection of ship's structure & condition of equipment. MIs noted bulkhead corrosion on starboard side bridge wing entrance (ivo searchlight cabling penetration). Satisfactory steel repairs required to be completed. clean/preserve/temporary patch, extended to May2023 VRA 46 CFR 91.25-10

Provide warning of failure of lubricating oil system and other measures to prevent rapid deterioration of propulsion and auxiliary machinery and minimize the fire risks of lubricating oil. Vessel’s #3 MDG lube oil temperature sensor is bad. Replace bad sensor. 46 CFR 58.05-1(a) ABS SVR 2003 Part 4 (4-6-5/5.1.2)

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Per vessel fire plan, fire doors in ECC and its boundary is rated A-60. Fire doors and boundaries in ECC have holes. Rectify fire boundaries in ECC with A-60. Temporary fix (firestop) – new doors on order – extended to May2023 VRA 46 CFR 92.05-1 (a)

Non-conducting deck coverings, such as non-conducting mats or gratings, suitable for the specific switchboard voltage must be installed for personnel protection at the front an rear of the switchboard and must extend the entire length of, and be of sufficient width to suit, the operating space. EDG switchboard insulation mats, front and back, too small for the area. Replace mats with suitable size.

Each vessel must carry not less than 12 rocket parachute flares approved under approval series 160.136. All 12 rocket flares onboard expire 12/22. Vessel on day light Ops only until new flares arrived onboard. 46 CFR 199.60(c)

Internal combustion engine exhausts shall be suitability insulated from combustible matter. Suspected exhaust leaks & cracks behind lagging for all MDG silencers & exhaust support hangers. Satisfactory investigation and exhaust piping repairs required (if damaged discovered). 46 CFR 92.05-5(a) open work order, extended to May 2023 VRA

Vessels in ocean service shall be fitted with anchors,chains, and equipment in general agreement with the standards established by ABS. MIs noted temporary liquid metal patch (Belzona epoxy) on #2 anchor windlass gear casing. Permanent repair required. 46 CFR 96.07-5(a) temporary metal liquid patch installed, new parts on-board and service order requested, extended to May 2023 VRA

Each self-propelled vessel must have a navigation light indicator panel to control side, masthead, and stern lights. The panel must visually and audibly signal the failure of each light. Panel must have a fused feeder disconnect switch and duplicate light sources for each navigation light. MI noted the following issues: 1. Stern Light (White) - Primary bulb out 2. Range Light - Primary fuse trips when testing switch 3. Port Running Light (Red) - Primary bulb out Satisfactory repairs required.

Vessel's firefighting equipment shall be ready at all times for use and condition maintained. MI noted inlet elbow pipe for Port fire pump suction line leaking. Satisfactory repairs required. 46 CFR 97.15-60(a)

Vessel's firefighting equipment shall be ready at all times for use and condition maintained. During hydrostatic testing of hoses, station 20 fire hose failed. Replaced with new hose. 46 CFR 97.15-60(a)

Where automated systems are provided to replace specific personnel in the control & observation of the engineering plant or reduce overall crew requirements, the safety of the vessel shall be equal to that of a fully attended machinery space; the acceptance of this reduction in engineering personnel is dependent upon the planned maintenance program, inspection, & testing of the automation to ensure continued reliability. Vessel currently has 31 inhibited alarms, with multiple vital system equipment sensors in an error state & requiring manual readings during engineering rounds. Periodically unattended machinery space (PUMS) endorsement to be suspended & vessel shall establish a manned engineering watch, effective immediately. Additional engineers/QMEDs to meet full manned engineering watch to be crewed onboard within 30 days. The manned engineering watch required will be as followed: 1 Chief Engineer, 3 licensed engineers, 3 QMEDs PUMS endorsement & engineering manning reduction will be reinstated upon satisfactory alarm servicing & grooming.

A minimum of two (02) two-way portable radiotelephone apparatus shall be provided for each fire party for fire-fighter's communication. These devices shall be of an explosion-proof type or intrinsically safe. A minimum of 2 devices, compliant with above shall be provided at each fire locker within 30 days. Extended 14 days due to delivery date onboard vessel, estimated 01OCT2021. 74 SOLAS (2014) II-2/10.10.4

The Company should establish procedures to ensure the ship is maintained in conformity with provisions of relevant Class rules and statutory regulations. Required alarm servicing & grooming in Ship's maintenance system is flagged as over 1 year due. Noted several inhibited alarms causing alteration to engineering rounds for verification of sensors impacting vital system automation. Required servicing shall be completed and evidence provided within 30 days. Extended 8 days to 08OCT2021 to align with inport dates, and servicing provider availability. 74 SOLAS (2020)IX/2.1.2 ISM (2018) Part A 10.1

Each alternating-current generator conductor must have a voltmeter to show each phase voltage and bus voltage. Voltmeter for the emergency diesel generator was found to be inoperable. A functioning meter shall be provided for the emergency switchboard within 30 days. Extended to 15OCT2021, estimated delivery 15OCT2021 onboard. 46 CFR 111.30-25(b)(2)(i)(5)

Vital systems that are automatically or remotely controlled must be provided with an alarm systemif instrumentation is not continously monitored for detection of a failure or unsafe condition. Vessel automation system has 38 inhibited alarms. Identify and rectify faults, provide justification for any remaining inhibited alarms.

Installed fire equipment and its installation shall meet the requirements of this part. ECR fire door is holed from previous hardware installations.

The general construction of the vessel shall be such to minimize fire hazards. Area behind ECR switchboards are not clear and being used for storage.

Periodic safety test procedure documents approved by the CG must be retained aboard vessel/ Submit PSTPs to MSC for final CG approval.

Each receptacle outlet must be compatible with the voltage and current of the circuit in which it is installed. Remove daisy chain power strip in ECR.

Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulations. In addition, it shall be provided with alarm arrangements to indicate when this level cannot be maintained. The system shall also be provided with arrangements to ensure that any discharge of oily. (Cont) mixtures is automatically stopped when the oil content of effluent exceeds 15PPM. Vessels OWS can not produce effluent below 15 PPM, even when drawing suction from sea chest. OCM P/S has been altered, OCM reading did not change from 0.0. Have OCMs re-calibrated, provide accurate system diagram, conduct 20 min operational test to the satisfaction of attending USCG Marine Inspector and ABS Marine Surveyor. MARPOL 74/78 Annex I/14.7, 33CFR 155.380 17ac

All power actuated valves required in emergency to vessel's machinery, maintain stability, and to operate bilge and firemain system must have a manual means of operations. remote valve controls must be accessible under service condition. STBD side ballast main deck reach rod 6-16-1 was found seized and inoperable. 46CFR 56.50-1(g ), 50c

An audible and visible alarm must activate in the pilothouse upon failure of the electric power to the control system of any steering gear. Conducted sat steering gear test. However, STBD steering gear pump 1 has active alarm of main power fail that cannot be reset. 46CFR 58.25-25 50c *Update 9/3/2019* Received email that steering was repaired, however it has not been witnessed by ABS or the Coast Guard. Extended deficiency to 20Sep2019.

Ceilings, linings, and insulation, shall be of approved incombustible materials. General workshop 2-108-1 A-60 ceiling has insulation removed from outboard rear corner. Renew with approved insulation. 46CFR92.07-10, NVIC 9-97, CH-1 , 50c

Provide some means of fire/smoke resistant sealant to all MCT and cable penetrations in A-60 boundaries to the satisfaction of Class or CG.

Provide a plan to [name withheld], which addresses properly approved sealing of cable penetrations throughout the A-60 spaces. Multiple MCTs and pentration openings found with mixed systems, such as Rox Blox and Fire Stop, or multiple types/brands of sealers used. Plan should provide types of approved systems to be used and a reasonable timeline for installation.

When placed in autopilot, the Starboard Steering System produces an unusual noise and excessive temperatures. Troubleshoot and make permanent repairs IAW manufacturers recommended repair procedures to the satisfaction of the attending USCG Marine Inspector.

Properly repair steering gear to the satisfaction of the OCMI and attending classification society surveyor.

Found numerous cracks on 01 deck adjacent to Frame 74. Provide repair plan and repair all areas identified in accordance with an ABS/USCG approved plan to the satisfaction of the OCMI.

New LSA Mod maintenance and training requirements are not identified in the vessel’s Safety Management System (SMS). Correct to the satisfaction of the OCMI.

Found numerous weathertight doors and hatches did not close properly or had missing / improper gasket material. Repair to the satisfaction of the OCMI.

Found numerous fire doors that did not close properly or were missing closure devices. Correct to the satisfaction of the OCMI.

The Periodic Safety Test Procedures (PSTP) specific test procedure for “Under Frequency” did not perform as written. Correct to the satisfaction of the OCMI.

Reevaluate existing structural modification for Frame 74 and perform a structural stress analysis by a certified naval architect or structural engineer that identifies structural issues found onboard vessel following Frame 74 Modification. Correct to the satisfaction of the OCMI.

Damage to port side shell at frame 56. Repair to satisfaction of OCMI prior to departure.

Make necessary repairs to Starboard Steering Gear rudder. Repairs and operational testing to be completed to the satisfaction of OCMI prior to departure from next port of call (Subic Bay, Phillippines).

Make permanent repairs to starboard main propulsion motor. Repairs and operational testing to be completed to the satisfaction of the OCMI prior to departure from next port of call (Subic Bay, Phillippines)

Found fixed piping, portable hose arrangement, and air diaphragm pump in OWS space used to transfer bilge water from various voids & spaces throughout vessel (both hulls) to Oily Water Holding Tanks. Fixed piping and portable hose arrangement are not on any approved piping plan. Make all modified piping as original. Remove all unapproved piping and hose arrangements. Submit to ABS,USCG and MSC.

New installation of davit-launch liferafts requires an inspection of lifesaving arrangements for certification that includes proper condition and operation of survival craft launching appliances. Complete 10 percent overload test in the presence of ABS or USCG.

New installation of davit-launch liferafts requires an on-load release hook test. Complete required tests in the presence of ABS or USCG.

All fuel tanks and ballast tanks must be inspected for fractures and cracks at brackets associated with house support structure by an approved ACS. All issues found by attending ACS must be corrected to their satisfaction.

New installation of davit-launch liferafts requires an inspection of lifesaving arrangements for certification that includes proper condition and operation of survival craft launching appliances. Complete abandonship drill, launching a davit-launch raft of comparable size and capacity in the presence of USCG.

Towing strain test required by LSA Part 2, Regulation 6.2.7 can be delayed until a test liferaft is made available.

After investigating fuel oil contamination inside SWB tanks: 6-16-2. 6-56-1 and 6-124-2; vsl identified 2 fractures between FO tanks 6-56-2 and 6-40-2. Vsl shall inspect all FO and SWB tanks that could have been a source of contamination. Permanent repairs shall be approved and affected to the satisfaction of the OCMI prior to departure from port. Temporary repairs will not be authorized.

Provide repair proposal for fuel oil piping leak in ballast tank 6-108-2.

Make permanent repairs to fuel oil piping leak in ballast tank 6-108-2.

Properly dispose of contaminated ballast water from tank 6-108-2 in compliance with local laws and USCG regulations.

Pressure switch for the automatic shutdown of #2 and #3 propulsion generators associated with the Halon primary firefighting system is inoperable. This deficiency was found during the annual servicing by Global Fire and Safety on 28JUL2016. Coast Guard was not notified of this issue.

Primary lifesaving falls for the port side lifeboat show visible damage in the form of “fishhooks” and a broken strand. (7 strands per wire, 35 wires for the falls).

Emergency shutdown remote pull cable located outside the engineroom for #3 Main Propulsion Generator is badly frayed.

Stbd winch motor controller for stbd lifeboat / rescue boat davit will not hoist lifeboat / rescue boat to the correct position due to a faulty limit switch in the winch drum. Unable to unload an injured crewmember in the event of a personnel casualty.

Periodic Safety Test Procedures (PSTP) for reverse power relay test and power limiting procedures do not adequately describe actual procedures necessary to complete test for Engineering reduced manning. Power limiting test caused a complete blackout of the vessel. Reverse power relay test requires a split load pierside and does not allow independent control of each engine on the EasyGen System.

Found two oily water holding tanks on the IOPP SOVC may not be identified in the proper section (Section 2 and 3). ABS to determine if tanks are either required equipment for controlling oil discharge from machinery spaces or are a means of retention and disposal of oil residues.

Permanently repair or replace S/W piping to reefer condenser located in refrigeration machinery space.

Permanently repair or replace valve on S/W cooling discharge piping located in the stbd pumproom.

Prove proper operation of both port and stbd OWS in the presence of USCG Marine Inspector or ABS Surveyor.

Repair fuel leak on #2 Main Propulsion Generator on #8 cylinder.

Make permanent to General Alarm located on the fantail.

Make proper repairs to the emergency switchboard breaker to the satisfaction of the OCMI.

Make temporary repairs to the pencil-sized hole located between the oily water holding tank and the adjacent fuel tank, including striker plate, to the satisfaction of the OCMI.

Make permanent repairs to the oily water holding tank bottom and adjacent fuel tank overhead, including striker plate, to the satisfaction of the OCMI.

Vessels lower steering cylinder rod has detached from the rod eye in the tiller assembly of port steering system. Make necessary repairs to port steering system to restore full system capability. Prior to departure from next port of call. To the satisfaction of attending ABS Surveyor and FEACT.
