USNS EFFECTIVE

All mechanically propelled vessels of 1,600 gross tons and over in ocean or coastwise service must be fitted with a marine radar system for surface navigation. Vessel's X-band and S-band radars were inoperable at the time of inspection due to water intrusion into exterior control unit. 46 CFR 96.25-1

Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. Periodic safety test procedure (PSTP) for Main Diesel Engine Jacket Water High Temp Alarm was unable to be performed as per written procedure due to prior modification to engines. 46 CFR 61.40-10 Extended to 24OCT2026.

Each inflated rescue boat must be repaired and maintained in accordance with the manufacturer's instructions. Starboard side grab line attachments have started losing adhesion to inflated hull. 46 CFR 199.190(g)(4)

The design and arrangement of electric apparatus must afford accessibility to each part as needed to facilitate proper inspection, adjustment, maintenance, or replacement. Stored goods were situated in front of HVAC Feeder Ckt 15-8 P401, preventing access to control unit. 46 CFR 111.01-7

During and after any survey made by the assigning and issuing authority, those items, fittings, etc., which are found to be in an unsatisfactory condition by the surveyor shall be repaired or renewed in order to place the vessel in a satisfactory condition. Void below engineering office is being used for additional storage, but normal arrangement with temporary lighting and grated cover precluded the required closure of scuttle 2-31-2 to comply with load line requirements. 46 CFR 42.09-45

Non-conducting deck coverings, such as non-conducting mats or gratings, suitable for the specific switchboard voltage must be installed for personnel protection at the front and rear of the switchboard and must extend the entire length of, and be of sufficient width to suit, the operating space. Attending MI's noted that existing matting was not providing adequate protection from the switchboard, nor displayed the type approval for this application. Provide a replacement rated for the maximum voltage output of on board systems that is approved for such applications.

Remote alarms. All ECC alarms required by this part must be extended to the engineer's accommodations. Other than fire or flooding this my be accomplished by summarized visual displays. In order to restore PUMS on board the vessel the following must occur. 1. Provide a watch call alarm station in the officers mess near the galley location. On board staterooms were verified on 24 March 2025, however it was noted that the relocation of the Officers mess where the licensed engineers eat is missing a alarm panel display. 2. Complete a trial period to verify the effectiveness and the reliability of bilge arrangement trans alteration that was completed in October of 2024.

During servicing of the steering system it was noted that the port canard was 5 degress difference from the actual rudder position from the bridge rudder angle indicator. ABS Class has issued a finding number and giving the vessel 30 days to correct the issue. The vessel has devleoped a work around procedure and the system is still fully in tact with excpetion to one RAI. No CG 835 was issued as Class is acting on USCG's behalf.

Means shall be provided to control leaks and restrict ignition of combustible materials. MIs noted substantial hydraulic oil leaks for port and starboard anchor windlass HPUs. Leaks shall be cleaned and contained prior to departure. Chief Engineer reported damaged internal O'rings and ship worklist request (SWFL) for the parts over 3 months ago after MIs noted the leaks. SWFL to be submitted to local OCMI and expedited order by company for HPUs to be serviced within 30 days.

STCW engineering officer endorsements are required due to vessel's tonnage (>200GT) and route (Oceans Unlimited). 2AE did not have STCW Officer In Charge Engineering Watch (OICEW) endorsement on MMC. Mariner to submit STCW OICEW endorsement application to USCG NMC within 14 days. Vessel is a gov't vsl & exempt from PSC.

Penetrations of fire rated boundaries shall be kept in good order and maintained to meet appropriate structural fire protection rating. MIs noted improper cabling penetration in the following areas: 1. Improper multi cable transit (MCT) in way of MG Set #1 controls - mixed approval types in a single penetration 2. Improper MCT rocksblock installation in way of AMR forward bulkhead - blocks need to be rebuilt for satisfactory cable size Satisfactory repairs and verification due w/in 30 days.

For vessel having internal combustions engine arranged for air starting, ther are to be two or more air compressors. Vessel Captain reported that start air compressors were not producing the vessels required air presurre. Further investigation and troubleshooting by ships force found thatthe low pressure piston and liner wer badly worn/scored. 46 CFR 62.35-35 ABS SVR 4-2-1 08DEC2023: Modified and extended to 29FEB2024 VRA, ABS verified partial repairs.

Iaw 46 USC 8104(d) - 3 watch system required with max work time of 8 hours. Chief Engineer and 1AE are working 12+ hours a day and splitting a two watch rotation. 1AE openly stated that he was violating STCW work rest hours but was told by the Captain what to write in the reports to not exceed allowances (NMT 13 hours per day). Both engineers stated there is a huge safety concern with lack of rest and missing 2AE and 3AE for over 6 weeks and ongoing. During investigation, 1AE accidentally blacked out the ship by tripping on-line generator breaker - directly related to fatigue and lack of awareness. 46 CFR 15.1111 (e)

Automation systems must perform assigned tasks continuously for the detection of unsafe conditions and not be interrupted from monitoring. During inspection of bilge alarms by OEM for vital system automation, it was uncovered that the system had been rewired to bypass the alarms and trick the system to not alarm. This was across multiple bilge alarms and noted to be a deliberate modification (unauthorized by crew). Weekly bilge alarm testing required, questionable documentation as unsure of timeline for modifications. 46 CFR 62.25(b)

Service pumps shall be fitted with remote controls and protected against tampering. Report from 1AE yielded that power controls from ASW motor controller had been modified to remove electrical safety trips (breaker installed backwards to fail closed and consistently energize controls) - deliberate modification to bypass safety. Satisfactory repairs and further investigation required for additional unauthorized system modifications. 46 CFR 58.01-25

Vital systems that are automatically or remotely controlled must be provided with an effective primary control, and safety control system to ensure adequate and effective operation of the systems. 140+ inhibited alarms, reduced to 40+ by system grooming and engineers but still an active issue (includes ballast system valve actuators and remote controls disabled, FO tank level indicators faulty (manual soundings required, ASW remote pump controls disabled, LO pumps remote controls disabled). 46 CFR 62.25 (a)

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs fire detection and fire alarms systems. Attending IO/MI noted that the Fire Detection panned located in the ECC had numerous active inhibited alarms in addition to active faults preventing the system from operating correctly leaving the ship vulnerable to rapid fire growth. 74SOLAS(2014) II-2/14.2.1.1.2

Starboard ASW motor controller switch has been gutted and removed and is currently being turned on by the knife switch. This alteration was not authorized or approved. The company should establish and maintain procedures to control all documents and data which are relevant to the SMS. The onboard maintenance system “SAMMS” has been inoperable for task direction and tacking for the past 9 months. The company has provided a stand alone laptop, which intermittently works and is only available with signal at the pier. 74SOLAS(2020)IX/3.1 ISM Code (2018) 11.1

Attending IO/MI noted that when entering the starboard ASW pump room there was evident smell of burning electrical components. Further investigation identified that the No.2 ASW pump has a wasted mechanical seal, and the pump is running 10-15 degrees hotter than the adjacent pump verified with a thermal gun. This imminent failure could be further confirmed had the Vibration analysis equipment required to onboard been utilized to compare base readings to compare base readings to current conditions. Replace pump with suitable “In Kind” selection prior to fire or failure. 74SOLAS(2020)I/11(a)

ASW system pressure is substandard preventing operation of the full engineering plant. MI’s noted that one of the ASW check valves that had been removed had the stopping flap removed from it. The main steering gear and rudder stock shall be capable of putting the rudder over from 35 degrees on one side to 30 degrees on the other side with the ship at its deepest seagoing draft and running ahead at maximum ahead service speed in not more than 28 seconds. It was disclosed to attending MI that the Steering canards have a delayed response when in FU and NFU modes. 74SOLAS(2020)II-1/29.3.2

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs structural fire protection including fire resisting divisions, and protection of openings and penetrations in these divisions Self closing fire door leaving the engine room was broken. 74SOLAS(2020)I/11(a)

Piping systems shall be examined under working conditions as required by the marine inspector. Bilge system - issues with system piping and suction - crew has been using portable pumps in voids/confined spaces. Conduct satisfactory suction tests w/ MI witness of deep well spaces & bilge collection pockets. 46 CFR 61.15-15

The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. The onboard testing for fuel oil dilution and vibration analysis have been inhibited by broken or inoperable testing equipment. Attending MI/IO noted that visage or falling ball lube oil viscosity tests for fuel oil dilution were unable to be complete due to broken test units and additionally the vibration analysis testing tool was also inoperable. 74SOLAS(2020)IX/3.1 ISM CODE 10.3

A nonmetallic expansion joint must be replaced 10 years after it has been placed into service if it is located in a system which penetrates the side of the vessel and both the penetration and the nonmetallic expansion joint are located below the deepest load waterline. Vessel needs to establish a tracking and replacement program for all expansion joints located below the water line. Attending MI noted numerous nonmetallic piping installations located in the ASW pump rooms in addition to additional machinery spaces. 46 CFR61.15-12 (b)

ISM Code requirements were not met onboard and multiple violations observed: 6.2.1 - The company shall ensure that each ship is manned with qualified seafarers iaw national and international requirements. The ship has been without a 2AE and 3AE for over 6 weeks and not in compliance with STCW work rest hours for licensed engineering officers. 7 - Company shall establish procedures, plans and instructions for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. No record book entries or concise instructions from engineering department with logging sewage discarges/transfers to retention barges. No instructions available for transfer procedures or pollution prevention instructions. 9.1 - The safety management system should include procedures ensuring that non-conformities, accidents, and hazardous situations are reported to the company, investigated, and analyzed with the objective of improving safety and pollution preventions. Ship received reports from the chief engineer o/a 16JUN2023 concerning the potential discharge of raw sewage while at the pier due to failures within the MSD system for treatment. No further report was made to the local OCMI or MSC until 12SEP2023 via whistleblower. Reports were submitted to company DPA but little action were taken for prompt and thorough investigation by operator. 10.1 - The company should establish procedures to ensure that the ship is maintained in conformity with the relevant rules and regulations. Ship had several unauthorized modifications to safety systems and engineering equipment (MSD, Bilge alarms, salt water cooling pumps) that were uncovered by OEMs during technical support and servicing. Technicians were unable to determine length of time that systems were modified and safeties disabled. 10.3 - The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. These measures should include the regular testing of equipment. Several items were noted in direct contrast to this requirement: 1. During inspection of bilge alarms by OEM for vital system automation, it was uncovered that the system had been rewired to bypass the alarms and trick the system to not alarm. This was across multiple bilge alarms and noted to be a deliberate modification (unauthorized by crew). Weekly bilge alarm testing required, questionable documentation as unsure of timeline for modifications. 2. Attending IO/MI noted that the Fire Detection panned located in the ECC had numerous active inhibited alarms in addition to active faults preventing the system from operating correctly leaving the ship vulnerable to rapid fire growth. None of the crew could state how long the alarm had been active or sounding. 12.3 - The company should periodically evaluate the effectiveness of the SMS. Multitude of SMS failures observed onboard w/ little to no internal documentation.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required is maintained. Vessel Master reported an intermittent fire alarm fault. 46 CFR 95.05-1 (a) 46 CFR 76.27-5 (b)

All ships of 300 gross tonnage and upwards... be fitted with speed and distance measuring device, or other means, to indicate speed and distance through water. Vessel Master reported intermittent failure of the doppler speed log. 1974 SOLAS (2014) Reg. V/2.3.4

Exhaust system piping and components shall be independently supported to minimize failure from vibration, shock, expansion and contraction. Found #4 MDG exhaust support broken. 46CFR 58.03-1 ABYC P-1-1.5.6 50c

Fire detecting systems must be installed in the locations indicated by Table 76.05-1(a). Found INOP smoke detector in STBD Motor room. 46CFR95.05, 76.05-1

Liferafts must be arranged for float-free launching. Port/STBD liferaft station strap setup for center raft is dependent of outboard raft hydrostatic release for float-free launching. 46CFR199.130 10c

The exhaust system shall be gas-tight to the hull interior. Found #4 MDG exhaust piping joint separated and leaking into the space. 46CFR 58.03-1, ABYC P-1-1.5.1 10c

Bolts and studs must extend completely through the nuts. Loose bolts were found on Port Steering ram attachment. Torque bolts to OEM specifications. 46 CFR 56.25-20 10c

Lifesaving appliance must be in working order and ready for immediate use. Found FRB unable to start. Corroded connection lead to fuse failure. 46CFR199.70 10c

During operational test of oily water separator (OWS), unit could not produce effluent under 15 ppm. Prove proper operation of OWS prior to use.

Make temporary repairs IAW RO standards, submit repair proposal and effect repairs for cracked bulkhead IWO door frame in space above stbd pump room.

Correct rudder angle helm/actual differential for both steering gear units to alarm within 5 degrees difference.

Investigate and remove jumpers inside "Emergency shutdown relay panel" inside ECR.

here the emergency source of electrical power is an accumulator battery, it shall be capable of carrying the emergency electrical load without recharging while maintaining the voltage of the battery throughout the discharge period within 12% above or below the nominal voltage. Primary source of EDG starting, accumulator batteries are inoperable and EDG cannot start. Replace/repair primary source of EDG starting source prior to departure of current port.

Make permanant repairs to the fast respinse boat ignition swith.

Fast rescue boat (FRB) propulsion shaft bearing failed, limiting the top speed of the FRB. Repair or replace thrust bearing.

Crack was discovered on a longitudinal weld seam on bulkhead 166, lower inboard longitudinal, forward of the port steering gear room. Make permanent repairs IAW ABS steel repair procedures to the satisfaction of the attending USCG Marine Inspector or ABS Marine Surveyor.

Rescue boat davit hydraulic ram leaks excessively. Replace or make permanent repairs IAW manufacturers recommended repair procedures to the satisfaction of the attending USCG Marine Inspector.

Replace running lights showing faults on the navigation panel.

Stbd rudder angle indicator on stbd bridge wing is inop and needs replacement.

Post placards for both new davit launching life raft systems showing launching procedures.

All weather deck scuttles should open and close freely.

Replace wasted pilot ladder shackle.

Paint locker exterior light switch need to be replaced.

Port bridge repeater reads different from main heading inside pilothouse.

Stencil new Halon/CO2 release pull station for HPU/Array room located on fantail.

Replace missing wingnuts on weatherdeck for several ventilation closures.

Conduct operational test of OWS in port pump room (tank was empty).

Conduct operational test of OWS in stbd pump room (solenoid valve on order).

Ensure all life raft painter lines are secured to the weak links.

Ensure stuffing tubes are replaced for steering oumps in both steering rooms.

New installation of davit-launch liferafts requires an inspection of lifesaving arrangements for certification that includes proper condition and operation of survival craft launching appliances. Complete 10 percent overload test in the presence of ABS or USCG.

New installation of davit-launch liferafts requires an on-load release hook test. Complete required tests in the presence of ABS or USCG. Towing strain test required by LSA Part 2, Regulation 6.2.7 can be delayed until a test liferaft is made available.

New installation of davit-launch liferafts requires an inspection of lifesaving arrangements for certification that includes proper condition and operation of survival craft launching appliances. Complete abandonship drill, launching a davit-launch raft of comparable size and capacity in the presence of USCG.

While onboard, discovered four locations on upper deck, aft of the engineroom casing stack space that showed signs of buckling, temporary repairs, and doublers installed to cover cracks in the superstructure. Make proper permanent repairs to the satisfaction of the attending USCG inspector.

New installation of davit-launch liferafts installation and steel structure modification requires approved plans be submitted to ABS and USCG. Provide approved plans to the attending marine inspector.

New installation davit-launch launching appliances must be Coast Guard approved. Provide Coast Guard Approval Certificate for the launching appliances to the attending marine inspector.

Towing strain test required by LSA Part 2, Regulation 6.2.7 can be delayed until a test liferaft is made available.

EDG tripped off-line during a 2-hour load test. Must be repaired to the satisfaction of Class and USCG prior to departure. SOLAS 2004 Cons. II-I/44.2

Provide repair proposal for existing cracks located at Frame 90, starboard side entrance to Pump Room.

Provide repair proposal for existing crack located on the deck on the 01 level, Frame 74.

Found two oily water holding tanks on the IOPP SOVC that may not be identified in the proper section. ABS to determine if tanks are either required equioment for controlling oil discharge from machinery spaces or are a means of retention and disposal of oil residues.

Properly label "General Alarm - go to your station" at General Alarm located on the fantail.

Make permanent repairs to the fuel leak located on #4 SSDG at the fuel-oil return valve.

Make proper repairs to the lifebuoy quick release arrangement located on the port side bridge wing.

EDG failed to start due to faulty start batteries.

Vessel reports ships whistle is inoperable. Vessel shall repair or replace ships whistle and provide report to FEACT prior to departure from next port of call. To the Satisfaction of Coast Guard Activities Far East.

Vessels S-band (3GHz) is inoperable. Vessel shall repair or replace vessels S-band radar prior to departure from next port. To the satisfaction of Coast Guard Activities Far East.

1. Several inspection and multiple engineering analyses have identified high stress areas susceptible to structural failure throughout the T-AGOS Class. Crowley and Military Sealift Command shall consolidate engineering studies and develop systematic approach to permanently repair areas of structural concern. All repair plans shall be submitted for approval to the satisfaction of the OCMI.

2. Develop a structural monitoring plan for all identified areas of high stress/concern. All high stress areas shall be made available for inspection at each CG annual and Class Survey.

Temporary repairs shall be made to the satisfaction of ABS and [name withheld] analysis and permanent repairs shall be completed by the next for-credit dry.
