The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Vessel's has 377 gauges with expired calibrations (6-Feb-2026).Vessel has 6 gauges 'rejected' or with 'no gauge'. Calibrate and/or install all identified gauges in accordance with DD/ROH scope of work and provide satisfactory evidence of calibration results to the attending MI.

When cylinders of Class 2 (compressed gas) materials being transported by vessel are stowed in a vertical position they must be stowed in a block and cribbed or boxed-in with suitable sound lumber and the box or crib dunnaged to provide clearance from a steel deck at least 10 cm (3.9 inches) off any metal deck. Pressure receptacles in the box or crib must be braced to prevent any shifting of the pressure receptacles. The box or crib (gas rack) must be securely chocked and lashed to prevent shifting in any direction. Vessel had 8 compressed gas R-134 refrigerant DOT cylinders stowed in the AMR space where 4 where outside of brackets and braced via a ratchet strap. Install gas rack to accommodate all 8 cylinders permanent stowage during next DD/ROH.

Master has reported that FRB a faulty house battery has rendered the FRB inoperable, that there is a corroded gear selector switch, and an inaccurate battery volt meter. Persons designated by the owner of the vessel must conduct all tests and the Design Verification and Periodic Safety tests (PSTPs) must be witnessed by the Coast Guard. After approval by USCG (Marine Safety Center), demonstrate PSTPs for attending Marine Inspector. Due by 04JUN2025. 46 CFR 61.40-1(b)

The SMS should be effectively implemented and include specific measures aimed at promoting the reliability of equipment and systems. During in-service inspection, the following objective evidence indicates that the start air system may be improperly configured and/or not effectively maintained: - System operating at 195 psi rather than design pressure of 250 psi - 4000 hour overall overdue - Missing spare parts that were ordered in Oct2019 but not received - Five inhibited alarms You are directed to complete an internal SMS audit within the next 30 days to include a root cause analysis and implement corrective actions to prevent recurrence. The effectiveness of any corrective actions will be assessed at the next regulatory scheduled external audit.

Condition of the ship shall be maintained fit to proceed to sea without danger to the ship or persons on board. Vessel’s air compressors are 2 years overdue for maintenance overhaul. Parts ordered by vessel in October 2019, company never processed order. Vessel marked overhaul completed June 2020 in SAMMs system with note deferring until drydock. Per company procedure maintenance can only be deferred up to one year. Additionally, Hours meters were installed February 2020 not in accordance with OEM and non/standardized. After any survey of the ship under regulations 7, 8, 9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. Following a report from the Master to USCG FEACT about inoperable Emergency Generator found during regular scheduled maintenance, verified the Emergency Generator does not start automatically and does not take electrical load in the event of an emergency. Vessel must have an emergency source of power with a capacity to supply all required loads.

Gyro-repeaters located in the port and starboard steering spaces are inoperative. Each vessel must have an illuminated repeater at the main steering stand.

Installed bilge-pumping arrangements in both port and starboard pump rooms are not approved or annotated on the vessel’s approved bilge piping diagram. Update and seek approval for the update that includes installed pump and hose arrangements currently in use.

Engineroom emergency fire dampers for stbd side exhaust fan failed to actuate. Means shall be provided to secure dampers in the event fixed firefighting systems are energized.

Following a scheduled USCG inspection & proper repair procedures, the crack identified at Frame 56 stbd side-shell in fuel tank 6-40-2 started leaking seawater. Most probable cause was the redistribution of stresses on bulkhead 40 & surrounding area after scheduled insert on bulkhead 56 was cut out & removed. Previous CG-835 issued on 13APR2017 allowing temporary repair to this area is vacated.

Submit repair proposal to ABS and MSC for the permanent repair strategy for the cracked weld in fuel tank 6-40-2.

ABS and vessel crew discovered a cracked weld at Frame 56 port side-shell, which does not appear to penetrate thru the heart of the weld. Make temporary repairs to the satisfaction of [name withheld] may correct on behalf of the OCMI.

Vessel crew discovered the dual-seal on the stbd propulsion shaft had failed. Replace seal at next scheduled credit dry-dock. Notify Activities Far East, Japan of scheduled repair and request USCG attendance to witness repairs.

Multiple areas of fractures in way of fuel tanks, bulkheads and structural members have been identified on USNS LOYAL. The vessel is directed to complete permanent repairs to the satisfaction of the cognizant OCMI. Prior to departure from MRA period and not later than 21 March, 2017.

1. P.A. system transmission is broken and intermittently inoperable. Repair P.A. system to full functionality to ensure shipboard announcements can be heard clearly throughout vessel.

Port and Stbd life-boat searchlights are inoperable. Stbd life-boat canopy light is inoperable. Repair/Replace searchlights and Stbd canopy light.

3. Installed Jotron EPIRB Hydrostatic release is not marked with Coast Guard or SOLAS approvals. Replace hydrostatic release with approved Hammar H2OE hydrostatic release or an approved equivalent that is both CG and SOLAS approved.

Main Port/Stbd Rudder angle indicators and bridge wing repeaters are off by 5-6 degrees. Repair rudder angle indicators to reflect correct rudder angle.

5. Port Side Aft life-boat embarkation light is inoperable. Repair/replace Port Side Aft life-boat embarkation light to ensure proper operation.

6. Stbd Side Aft Work/L.B. Embarkation light is inoperable. Repair/replace Stbd Side Aft Work/LB embarkation light to ensure proper operation.

7. Galley Range Guard Fixed Fire extinguishing system appears to be leaking and gauge is indicating it needs to be re-charged. Have servicing technician inspect and recharge fire extinguishing agent bottle.

8. Fire Hose Station Enclosure latches for stations #24 and #30 are broken and Enclosures are secured with line. Repair/replace enclosure latches so that enclosures can be secured as designed

9. 19 Life-jackets were found deteriorated and holed to the point that they are no longer serviceable. Ensure adequate serviceable life-jackets are available for all persons onboard in accordance with your Certificate of Inspection.

Several inspection and multiple engineering analyses have identified high stress areas susceptible to structural failure throughout the T-AGOS Class. Crowley and Military Sealift Command shall consolidate engineering studies and develop systematic approach to permanently repair areas of structural concern. All repair plans shall be submitted for approval to the satisfaction of the OCMI.

2. Develop a structural monitoring plan for all identified areas of high stress/concern. All high stress areas shall be made available for inspection at each CG annual and Class Survey.
