Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Each vessel must have permanent draft marks at each end of the vessel or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. The vessel did not have any draft marks on its exterior. To be completed prior to next credited dry dock. 46 CFR 185.602 (C)

Every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. The compass did not accurately display the direction that the vessel was pointing. 46 CFR 184.402 (a)

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. There was a singular cable running throughout the length of the vessel with no supports installed. 46 CFR 183.340 (b)(4)

Each ring life buoy with a floating waterlight must have a lanyard securing the waterlight around the body of the ring life buoy. The waterlight must be attached to the lanyard with a corrosion-resistant clip and allow the waterlight to be quickly disconnected from the ring life buoy. The clip was connected to the end of the lifeline instead of secured around the body of the ring life buoy. This was corrected to satisfaction in the presence of the attending Marine Inspector. 46 CFR 180.70(d)

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Service records for fire extinguishers and fixed-gas fire extinguishing system were not available.

An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151, of this chapter. IBA service record was missing.

Except as provided in §§ 80.177 and 80.179, operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit of the class required below. Vessel master does not have a radio operator permit. 47 CFR 80.155.

A vessel of more than 7.9 meters (26 feet) in length, but not more than 19.8 meters (65 feet), must carry a minimum of one life buoy of not less than 610 millimeters (24 inches) in diameter. Life ring has deteriorated to an unusable condition and has a temporary replacement.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing...personnel injury. Rotating shaft must be covered by a machinery guard to prevent injury. Must be completed by 31 January 2023.

Repairs or alteration to the hull machinery, or equipment that affect the safety of the vessel must not be made without approval of the cognizant OCMI, except during an emergency. Vessel was struck by another vessel causing exterior and interior damage while vessel was moored. Vessel is not permitted to carry passengers prior to an MI conducting a damage assessment and internal structural exam to ensure that no damage occurred from mishap.

An adult life jacket must be provided for each person carried on board. 03 life jackets were not in good condition and were removed from service. Vessel is reduced to passenger count of 45 passengers until life jackets are replaced.

Individual power pumps used for separate spaces are to be controlled from a central point and must have a light or other visual means at the operating station to indicate operation. Port bilge pump indicator not operating as installed. 46 CFR 182.520(a)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release had an expiration date of January 2020. 46 CFR 185.740(b)

At each initial and subsequent inspection for certification, operator must have the vessel ready for inspection including the following: Inspection of fixed gas fire extinguishing system. Port engine room halon cylinder is overdue for inspection of the cylinder. Complete required test of cylinder. 46 CFR 176.810

At each initial and subsequent inspection for certification, the operator shall have the vessel ready for inspections including the following: Inspection of the condition of the spars. Hairline fractures were present on forward most part of boom connecting to gooseneck. Third party assessment required to determine suitability of continued service. 46 CFR 176.802

At each initial and subsequent inspection for certification of a vessel, the operator shall have the vessel ready for inspections including the following: Inspection of all railings. Upper course on the forward corner of the starboard rail was fractured throughout. Crop and renew fractured area. 46 CFR 176. 802

Ventilation systems for spaces containing machinery or fuel tanks must comply with the requirements of part 182 of this chapter. Port engine room ventilation ducting had a large fist size hole. Replace ducting. 46 CFR 177.620

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Forward bulkhead in port engine room has small hole that needs to made vapor tight. 46 CFR 177.405(c)

A vessel must carry adequate and up to date charts. Charts onboard were not corrected with the LNM. Provide up to date charts onboard. 46 CFR 184.420(a)(1)

At each initial and subsequent inspection for certification, the operator shall have the vessel ready for inspections including the following: inspection of the condition of the spars, standing rigging, running rigging, blocks, fittings, and sails. PMP expiration dates on cover page overdue and onboard logs do not accurately reflect actual maintenance being done. Update PMP to capture all inspections conducted. 46 CFR 176.802
