Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Multiple vent closures used a velcro method to affix the damper to the vent, and they did properly close ventilation. Demonstrate properly operating ventilation closures to the satisfaction of the attending marine inspector. 46 CFR 182.465 (h)

Fire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Dampers used to cover ventilation into the engine compartment made of flammable material. Provide means of securing ventilation which is flame resistant. 46 CFR 177.405 (a)

Each vessel that operates on the high seas, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Provide documentation of EPRIB hydrostatic release servicing or replace to the satisfaction of the attending marine inspector. 46 CFR 180.64

Flexible nonmetallic hose may be used in lube oil, fuel oil and fluid power systems, subject to the following requirements: Flexible hose runs must be visible, easily accessible, protected from mechanical damage, and must not penetrate watertight decks or bulkheads. Nonmetallic hose penetration in a non-water tight bulkhead is susceptible to vibration damage from chaffing. Provide protection from mechanical damage on fuel hose section that penetrates bulkhead to the satisfaction of the attending marine inspector. 46 CFR 182.720(3)(ii)

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. 01 Crew member had not completed preemployment drug testing. Member shall not serve as crew until fully enrolled in vessel's DAPI program. 46 CFR 16.201(a)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Floating water light non operational. Vessel is restricted to daytime operation until rectified to the satisfaction of the attending marine inspector. 46 CFR 180.70 (d)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Vessel could not get underway to perform drills. Conduct man overboard drill to the satisfaction of the attending marine inspector.

At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. Vessel could not get underway to perform drills. Conduct fire drill to the satisfaction of the attending marine inspector.

Portable and semi-portable extinguishers must be inspected in accordance with (IAW) NFPA 10... Portable extinguisher inspections not logged as completed IAW NFPA 10. Inspect and log the inspection of all portable fire extinguishers IAW NFPA 10 to the satisfaction of the attending marine inspector.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Bilge alarm float switches not permanently mounted. Mount in accordance with manufacturer recommendation to the satisfaction of the attending marine inspector.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Replace blade type fuse in the lift circuit with an approved cartridge type fuse to the satisfaction of the attending marine inspector.

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacture's instructions. Bulkhead penetration from engine room to space forward of has a penetration and is not vapor tight. Make space vapor tight to the satisfaction of the attending marine inspector. 46 CFR 181.420(a)(4)

Except on a vessel limited to daytime operations, the compass must be illuminated. Compass light extinguished. Restricted to daytime operation until rectified to the satisfaction of the attending marine inspector. 46 CFR 184.02 (c)

The owner or managing operator must provide satisfactory evidence of the required servicing of the fixed-gas fire extinguishing systems to the marine inspector. The pre-engineered fixed gas fire extinguishing system lapsed the servicing date per manufacturer's instructions. 46 CFR 176.810(b)(2)

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry: Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. One of the following may be substituted for any of the hand orange smoke distress signals required under paragraph (a) or (b) of this section: A hand red flare distress signal approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121. Vessel does not have orange smoke distress signals on board. Route is restricted to Lakes, Bays, and Sounds until rectified. 46 CFR 180.68(a)(2) 46 CFR 180.68(c)(2)(ii)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Spare batteries were being stowed in passenger seating area and fuel cans were being stowed in the forward storage space. 46 CFR 176.830(a)

The owner or managing operator must provide satisfactory evidence of required fixed-gas fire extinguishing system servicing. The engine room fixed-gas system servicing is expired. Additionally, the cylinder is installed on an angle and cannot be determined to have the ability to discharge required contents. Provide proof of valid servicing and proof from the manufacturer that this fixed-gas cylinder can be installed on an angle without a siphon tube. 46 CFR 176.810(b)(2)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. The life ring is wasted and must be replaced. 46 CFR 176.808(b)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The vessel is required to a masthead light forward as per 33 CFR 83.23. 46 CFR 183.420

Each vessel certificated to operate on a Limited Coastwise route within three miles of land in warm water must have on board a FCC Type Accepted EPIRB, installed to automatically float free and activate. The EPIRB registration was expired and the hydrostatic release did not have an expiration year marked. Properly register EPIRB and provide it with a valid Hydrostatic release. Vessel is limited to a Lakes, Bays, and Sounds route until corrected. 46 CFR 180.205(e)(3)

Each life jacket must be marked with Type I retro-reflective material. Two adult life jackets retro-reflective material was wasted. Provide appropriate markings to the satisfaction of the attending marine inspector. Passenger count reduced to 25 until resolved. 46 CFR 185.604

The employer shall provide an Employee Assistance Program for all crew members. Vessel did not have on board evidence of informational material, community service hot-line telephone numbers, or company policy regarding drug and alcohol use in the workplace. Provide the above to the satisfaction of the attending MI. 46 CFR 16.401

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels. Vessel did not have on board evidence of a drug testing program. Provide evidence of enrollment with a drug testing consortium. 46 CFR 16.230(a)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The vessel's Safety Radio-telephony Certificate is expired. Provide a valid Safety Radio-telephony Certificate. 46 CFR 184.502

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The vessel's radio operator did not have a radio operator permit on board. Provide valid radio operator permit to the satisfaction of the attending MI. 46 CFR 184.502

Shutoff valves, installed so as to close against the fuel flow, shall be close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank shall be operable from outside the tank compartment. Port fuel shutoff valve doe not completely close. Prove the remote shutoff valve closes completely to the satisfaction of the attending MI. 46 CFR 182.20-40(b)(3)

When the flexible section of a fuel fill pipe is a non-conductor of electricity, the metallic sections of the fill pipe separated thereby shall be joined by a conductor for protection against static spark when filling. Both fuel tank fill pipes are separated by flexible hose without being joined by a conductor. Install proper conductors. 46 CFR 182.20-30(d)

A submersible electric bilge pump may be used as a power bilge pump provided that it meets the requirements in UL 1113. The aft engine space bilge pump is not UL rated. Install correct UL rated pump. 46 CFR 182.520(e)(1)

Each Vessel must have permanent loading marks placed on each side of the vessel forward and aft. The vessel does not have loading marks. Affix loading marks to the vessel in the required locations at next dry dock. 46 CFR 185.602

At each inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Diesel fuel, other various chemicals, and miscellaneous items were observed in the toilet area and Cuddy cabin. Remove all hazardous products, fuels, and chemicals. Additionally, remove all items adding weight not accounted for in the vessel's stability test which are not part of the vessels outfit. 46 CFR 176.830(a)

All fuel tanks must be electrically bonded to a common ground. Bonding wire for stbd fuel tank is damaged. Replace bonding wire to fuel tank. 46 CFR 182.440(b)(4)

Electrical equipment must be installed to protect from electrical hazards. Bonding wires and terminal connections for both port and starboard steering rods have excessive corrosion. Replace or clean up. 46 CFR 183.200

Wire connectors utilized in conjunction with screw type terminal blocks must be of the captive type such as the ring or the flanged spade type. Non-captive connectors on bus bar terminal by steering rods. Replace with approved type. 46 CFR 183.340(k)

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. Mid bilge pump light in auto inoperable. 46 CFR182.520(a)

Each vessel must be equipment with at least two operable portable battery lights. Vessels portable battery lights are inoperable. 46 CFR 183.430

The following spaces must be equipped with a fixed gas fire extinguishing system in compliance with 181.410: A space containing propulsion machinery. Fireboy override function inoperable. Provide a certified technician service report & demonstrate all shutdowns, alarms, and overrides to the attending USCG marine inspector. 46 CFR 181.400 (a)(1)

All vessels must have operable navigation lights in accordance with the Navigation Rules. Masthead light inoperable and not mounted. No anchor light onboard. Install anchor and masthead light that is in compliance with the Navigation Rules. 46 CFR 183.420

The OCMI may require the vessel to get underway. Get underway in presence of marine inspector to conduct drills. 46 CFR 176.404(a)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space bilge subject to flooding from sea water piping within the space. The visual alarm for the high water level indicator was inoperable. Repair the audible alarm to the satisfaction of the Marine Inspector.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing LAT/LONG input for DSC operation. Provide proof of LAT/LONG input into VHF radio to the satisfaction of the Marine Inspector. 47CFR80.905

A submersible electric bilge pump may be used as a power bilge pump provided that the pump is permanently mounted. The forward engine room bilge pump was not permanently mounted to the bilge. Permanently mount the bilge pump.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Life jackets were not separated. Properly separate and mark adult and child size life jacket stowage.

Each floating waterlight installed on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The floating waterlight was permanently attached to the ring buoy. Attach the light to the ring buoy with a corrosion-resistant clip.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. The vessel’s ring life buoy is missing Type II retroreflective material. Adhere retroreflective material on the ring buoy IAW 46CFR185.604.

A submersible electric bilge pump may be used as a power bilge pump provided that the pump is listed by an independent laboratory as meeting the requirements in UL 1113. The 02 installed engine room bilge pumps are not UL rated. Replace the installed bilge pumps with the UL rated type.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The fuel filter servicing the Port Main Engine was leaking. Repair fuel filter to the satisfaction of the Marine Inspector.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's port and starboard fuel shutoff valves were not operable. Prove proper operation of fuel shutoff valves to satisfaction of attending Marine Inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Battery for EPIRB expired. Renew EPIRB battery IAW manufacturer’s specifications.

All cable and wire must be installed with metal supports spaced not more than 24 inches apart. Multiple wires unsupported in compartment forward of engine room. 46 CFR 183.340 (b)(4)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found in multiple compartments to include fwd of engine room, and coming off the battery charger. 46 CFR 183.200 (c)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Two engine mounts on the port engine have failed. The forward inboard and aft outboard. 46 CFR 182.100 46 CFR 182.100

Cable and wire for power and lighting circuits myst: Be listed by Underwriters Laboratories (UL) as UL Boat or UL Marine Cable or other standards listed by 46 CFR 183.340(d). Vessels winch uses SAE automotive cable not meeting the requirements of 46 CFR 183.340(d). 46 CFR 183.340(d)

Electrical equipment exposed to the weather must be watertight. Vessels winch cable has connection that is not watertight. 46 CFR 183.210 (b)

If the batteries are not adjacent to a distribution panel or switchboard that distributes power to the lighting, motor, and appliance circuits, the battery lead must have a fuse in series as close as practicable to the battery. Vessels winch battery installation has no circuit protection. 46 CFR 183.350 (g)

Each small battery must installed in a well ventilated space and protected from falling objects. Vessels battery box covers have cut out for terminal connections. Provide protection for exposed terminals from falling objects. 46 CFR 183.354 (b)

Overcurrent protection must be provided for each ungrounded conductor. Vessels bilge pumps and high water alarms do not have overcurrent protection installed. 46 CFR 183.380(a)

Each life jacket must be marked with type I retro reflective material approved in 46 CFR 164.018; Vessel had 03 child life jackets with unusuable retro reflective material. Add new retro reflective tape or replace life jacket.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Annual maintenance of operating station portable extinguisher was not completed. Replace or complete annual servicing.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge; (2) An audible alarm that sounds upon discharge. Audible alarm and visual indicator were not working. Prove proper operation of alarm and light for fireboy.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Midships bilge pump visual indicator is not working. Prove proper operation of visual indicator.

Provide proof of current FCC Radiotelephony Certificate IAW 46 CFR 80. Onboard certificate expired in May 2017.

Attach lifeline and floating waterlight to ringbuoy IAW 46 CFR 180.70.

Log monthly tests of EPIRB IAW 46 CFR 185.728. Tests were last logged in DEC 2017.

Provide proof of annual servicing of E/R fixed fire extinguishing system IAW 46 CFR 181.420. Last service was April 2017.
