Each buoyant apparatus shall be fitted with a life line and shall be equipped with a water light and painter. Two (02) water lights connected to life floats were non-functioning.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Machinery Guards on Air compressors are wasted and not secure.

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port #2 Engine jacket water pipe leaking. #2 Port Engine junction box was damaged.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Crack present in galley space lighting fixture. Lacked inline junction box leading to the stern light and two (02) deck lights. #2 Port Engine junction box was damaged.

Oceans T boats on warm water route required 100% life float. 02 life floats were found to have wasted life lines as the pennants were frayed and able to be torn from the life line. Vessel is restricted to 72 passengers until the life floats are repaired.

Gears, couplings, flywheels, and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Guards were not installed on rotating machinery belts on all 6 mains.

Protection must be provided at such locations where persons or equipment might come in contact with an exhaust pipe. #6 main exhaust and all main engine turbos had no insulation/ protection installed.

Lifefloat lifeline and pendants must be as furnished by the manufacturer with the approved life float. Lifeline and pendants were dry rotted from sun exposure. Passenger count reduced by 36 to 60 until replaced.

Lifefloat painter must be stowed so that it may run out freely; painter was not installed to allow the stbd side lifefloats to float free.

Lifefloat light must be as approved under approval series 161.010; battery in port side waterlight for lifefloat was dead.

Ring life buoy on a vessel must be approved in accordance with 160.050; grab line was missing beckets, so line was slack and loose on pilot house ringbuoy

Each ring life buoy must be marked with Type II retroreflective material; stbd side ring life buoy retro tape was peeling, needs replacement.

Engine exhaust piping must be properly supported by non-combustible hangars or blocks; #3 engine exhaust piping, #5 MDE piping were each missing bolts

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station." Multiple jacket water discharge temperature gages were found to be inoperable. Vessel shall fix or acquire temperature gauges and clear discrepancy with marine inspector.

Each overnight accommodation space on a vessel with overnight accommodations for passengers must be fitted with an independent modular smoke detection and alarm unit in compliance with §181.450." Two modular smoke detectors found to be inoperable. Smoke detector operability shall be demonstrated to marine inspector.

Where pressure-indicating gauges are used, they shall be chosen to be compatible with the pressure to be indicated. The size of the visual display, the fineness of graduations, and the orientation of the display will be considered. Both of the A/C receivers pressure gauges were inoperable in their ability to determine the pressure. Fix or acquire pressure gauges and demonstrate operability to marine inspector.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment and reasonably safe from electrical shock of personnel." MI noted the following electrical hazards; 1. Electrical connection made outside junction box for port A/C compressor. 2. Both port & starboard side MDE starter batteries were exposed and not in protective boxes/covers. 3. Starboard A/C cutoff switch was exposed. 4. Overhead light was exposed in engine room 5. High voltage dead end wires exposed in engine room from previous fire damage and system replacement. 6. Electrical control box mount missing for FWD gen. All items above shall addressed and corrected by the master within 14 days.

Switchboards shall be fitted with driphoods and non-conducting hand rails. Non-conducting mats or grating shall be provided on deck in front and rear of switchboards. Inadequate electric matting found in from of switchboard. Matting must be ASTM Type II. Vessel shall correct deficiency and clear the deficiency with the marine inspector.

The intent of this part is to insure that the scantlings, material, fastenings, workmanship, and arrangement of each vessel are such as to provide for safe operation in accordance with the terms of its certificate of inspection. Primary consideration shall be given to the provision of a seaworthy hull. Bulkhead penetration was open on the FWD bulkhead in aux space. Clear deficiency with marine inspector.

The intent of this part is to insure that the main propulsion machinery installed on each vessel in suitable for the vessel and its operation and that the auxiliary machinery is suitable for the vessel and its operation and that the auxiliary machinery is suitable for the purpose intended. It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. No. 3 main engine lube oil plug was broken and the port generator jacket water head tank hoses were wasted. Port generator Murphy switch needs to be reinstalled. These deficiencies shall be corrected and cleared with the marine inspector.

The Rules concerning shapes shall be complied with by day. Vessel did not have the necessary day shapes to comply. Obtain two black day shape balls and clear deficiency with marine inspector.

Batteries in water-lights on life-rings and life-floats were expired as of August 2016. VSL replaced batteries during the inspection. 46 CFR 180.70(d) / 180.175

Machinery Guards on MDE 4 and 6 are missing. Guards shall be in place while machinery is running for safety of crew. Install guards in accordance with manufacture instructions. 46 CFR 177.960

AFFF is expired. Product has a shelf life for 20 years. Replace Bottles with non expired product. 49 CFR 176.315

Shaft seals on # 3 and # 6 MDE are leaking. Secure leak on shaft seals. 46 CFR 179.350

Have space certified safe for worker by a certified Marine Chemist and do not to clean or move anything until inspected by a USCG Marine Inspector.

Submit Repair to local OCMI for approval and make repairs to the satisfaction attending USCG Marine Inspector.

During preparations for abandon ship damage, to a life float and the line connecting 1 of the floats parted.

Replace 2 paddles lost during preparations for abandon ship.

Provide boathooks, ships medicien chest and medical aid at sea book, and line throwing gun blank cartridges IAW 33CFR143.405 Prior to operation as stand by vessel.

#1 and #6 Shaft seals are leaking raw water excessively, approximately 1 gallon per hour each.

Port outboard MDE fuel supply valve leaking. Repair or renew supply valve.

Backing nuts not provided on shaft collars. Provide backing nuts.

Interlock bar was removed from switchboard panel. Provide properly installed interlock.

Port Fuel Return Valve leaking. Repair or replace.

Properly outfit all fuel vents with flame screens.

Vsl not properly outfitted with Fuel transfer procedures and fuel manifold diagram. Provide transfer procedures and diagram for vsl and diagram at transfer station.
