Each lifejacket must be approved under approval series in subpart 160.055 in part 160 of this chapter. The vinyl dip coating of 44 lifejackets were cracked and foam beginning to separate, exposing buoyant material to the environment. Vessel is not to carry passengers until lifejackets have approved repair or are replaced.

The vinyl coating on the life ring had cracks, exposing buoyant materials to the environment. Vessel is not to carry passengers until ring life buoy has approved repair or is replaced.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of ...fixed gas fire extinguishing system to check for excessive corrosion and general condition.
Crew stated they did not know how to test fixed "fireboy" fire extinguishing system in engine space and were unable to test during the inspection. The annual servicing report also stated the indicator alarm "failed".
