Oil filtering equipment shall be of a design approved by the administration and shall be such as will ensure that any oily mixture discharged through the system has an oil content not exceeding 15 PPM. OWS testing requires hose to be installed on discharge line fitting to see visible water that has passed through the 3-way valve for verification purposes. The connection allowed the possibility of discharge of oily mixtures of greater than 15 PPM if a hose was connected to the fitting. One way valve to be installed on test fitting. MARPOL (2017) Annex 1 Reg. 14.6

An alarm system shall be provided indicating any fault requiring attention shall; be capable of sounding an audible alarm in the main machinery room or at the propulsion machinery control position, and indicate visually each separate alarm function. The general alarm visual indicator in the machinery room does not illuminate when the general alarm is activated.

Electrical installations shall be such that electrical services essential for safety will be ensured under various emergency conditions. During operation of the emergency diesel generator, a ground fault was detected on the 120V Bus. In addition, the 24V Bus ground fault indicating bulb was faulty.

The condition of the ship and its equipment shall be maintained with the provision of the present regulations to ensure that the ship in all aspects will remain fit to proceed to sea without danger to the ship or persons on board. The 2" salt water pipe for de-icing that aft sea chest from the D/G #4 overboard is wasted and leaking.

After any survey of the ship, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. An unapproved installation of a diesel generator was loaded onto the starboard side of the main deck. In addition, the generator is not secured properly to prevent shifting while in heavy seas.

STBD engineroom supply dampers did not close completely when tested.

Each emergency generating set shall have a second source of energy provided for an additional three starts within 30 min unless manual starting can be demonstrated to be effective. Vessel's second source of energy for starting the emergency generator was depleted at the time of inspection. SOLAS 74 (2004 Cons Ed) Ch. II-1 Reg 44.2

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the listed items in SOLAS II-2 Reg 15.2.4.1. Fire Control Plan was incorrectly depicting certain fire boundaries and fire doors. SOLAS 74 (2020 Cons Ed) Ch. II-2 Reg 15.2.4.1

Doors required to be self-closing shall not be fitted with hold-back hooks. Engine Control Room door is required to be self-closing and has a hold-back device. SOLAS 74 (1997 Cons Ed) Ch. II-2 Reg 47.2

A fireman's outfit shall consist of personal equipment that includes an axe. All fireman's outfits were lacking the required axe. SOLAS 74 (1997 Cons Ed) Ch. II-2 Reg 17.1.1.5

Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage, settling, or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned. Remote fuel shutoff for the emergency diesel generator was painted over and thus inaccessible and unidentifiable. SOLAS 74 (1997 Cons Ed) Ch. II-2 Reg 15.2.5.

Ships with emergency steering positions shall be at least provided with a telephone or other means of communication for relaying heading information to such positions. Sound powered telephone in propulsion room unable to communicate with bridge. SOLAS 74 (2001 Cons Ed) Ch. V Reg 12(f)

All ships of over 150 gross tonnage, when engaged on international voyages, shall have on board an efficient daylight signaling lamp which shall not be solely dependent upon the ship's main source of electrical power. Daylight signaling lamp does not have a source of power that is independent from the ship's power supply. SOLAS 74 (2001 Cons Ed) Ch. V Reg 11

A line throwing appliance complying with section 7.1 of the LSA Code shall be provided. Line Throwing Appliance consists of only two independent units (two lines and two charges). SOLAS 74 (2020 Cons Ed) Ch. III Reg 18 LSA Code 7.1

Lifejackets and immersion suits hall be so placed as to be readily accessible and their position shall be plainly indicated. Stowage area for lifejackets and immersion suits on the bridge were lacking the required IMO marking/signs. 74 SOLAS (2020 Cons Ed) Ch III Reg 7.2.2 & 32.3.4

Minimum instrumentation, alarms and safety controls listed in Table 62.35-50 shall be in proper working order. Replace inoperable jacket water temperature probe on generator #3.

The SOLAS training manual shall contain instructions and information on the life-saving appliances provided on the ship. The fire-fighting training manual shall detail operation and use of fire-fighting systems and appliances. Insert information into manuals for ship specific appliances (i.e. different types of fixed fire-fighting systems found on board and new rescue boat installation.)

The statutes relating to inspection of public vessels are primarily contained in 46 U.S.C. 2101(24) and 2109. a. 46 U.S.C. 2101(24) defines a public vessel as one that "is owned, or demise chartered, and operated by the United States Government or a government of a foreign country; and…is not engaged in commercial service." The THOMAS G. THOMPSON is owned by the United States Navy (Office of Naval Research), operated by the University of Washington, and is not engaged in commercial service. U.S. Navy and National Oceanic and Atmospheric Administration (NOAA) vessels as defined in 46 U.S.C. 2101(24) operated for oceanographic research are not required to be inspected and certificated by the Coast Guard. However, such vessels may be inspected and certificated upon request by the parent agency and upon an “interagency agreement to this effect.” Without an interagency agreement, the Coast Guard does not have authority to conduct inspections, or casualty investigations on the THOMAS G. THOMPSON. You are required to "establish" an interagency agreement between the Office of Naval Research (ONR) and the United States Coast Guard no later than December 22, 2022, when vessel's Certificate of Inspection expires. Failure to establish an interagency inspection agreement with the Coast Guard prior to the Certificate of Inspection(COI) expiring will result in the vessel being deactivated in the Coast Guard's database. In addition, Coast Guard Sectors Puget Sound and Columbia River will not conduct COI renewal inspections without an interagency inspection agreement. The THOMAS G. THOMPSON does not need a COI to operate as a Government owned Oceanographic Research Vessel. Failure to establish an inspection agreement will not limit vessel operations. It is our understanding that the Office of Naval Research owns several research vessels that are operating in a similar manner. It would be beneficial to enter all ONR owned research vessels under the same interagency agreement. If you disagree with this decision you may ask for reconsideration, or appeal this decision in accordance with 46 CFR 1.03-20.

If you disagree with this decision you may ask for reconsideration, or appeal this decision in accordance with 46 CFR 1.03-20.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Vessel reported two of four main engines inoperable. Proceed directly to berth, propose repair plan, and make satisfactory repairs to the satisfaction of a CG marine inspector prior to departure.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. # 2 ships service generator reported inoperable due to mechanical failure in the electrical section. Parts have been ordered with a long lead time. Complete repairs at next dockside availability in Seattle scheduled for 02 OCT 2021, but not later than 31 OCT 2021. Vessel may operate in accordance with the document titled, (Additional Information on Electrical Load Analysis for R/V Thomas G. Thompson), Submitted to the Coast Guard by the vessel operator on 08 AUG 2021.

Vent outlets from all tanks which may emit flammable or combustible vapors must be fitted with a single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh. The clear area through the mesh must not be less than the internal unobstructed area of the required pipe. Required to ensure the flame screen on the 6C FO pipe is properly secured at the opening of the pipe. 46 CFR 56.50-85 (a) (8)

The integrity of a stairtower shall be maintained by “A” class doors at every level. The door shall be of the self-closing type. Holdback hooks will not be permitted. However, magnetic holdbacks operated from the bridge or other suitable remote control positions are acceptable. Noticed at least 4 fire doors (2 in the galley, 1 in the stairwell, and 1 near the engineroom) that did not self close. Test all fire doors throughout the vessel and ensure they are all operating properly and self closing. 46 CFR 190.17-10 (d) (4)

Equip each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe during oil transfer operations with a portable container of at least a 5 U.S. gallon capacity. Require a mechanical means of closing each drain and scupper in the container or enclosed deck area required by this section. Required to locate a means to close the drain of the aft containment. 33 CFR 155.320

Satisfactory means permanently attached, shall be provided for closing the openings of the air pipes. Required to provide means to close the pipes leading from the aft weather deck to the wire room. Remove the wooden plug from the smaller pipe. Need to make both pipe openings capable of being watertight. 46 CFR 42.15-45 46 CFR 42.15-50

Means of escape shall be maintained in a safe condition, adequate for emergency situations. The escape hatch leading to the weather deck on the aft stbd side needs to be fixed so that is can be quickly opened from both sides in the event of an emergency. 46 CFR 190.10 SOLAS II-2/ Reg 13.1

On behalf of the United States of America, the American Bureau of Shipping is authorized to issue or reissue the appropriate load line certificates. The required load line certificate issued to a specific vessel shall certify to the correctness of the load line marks assigned and marked on the vessel and compliance with authorized conditions, restrictions, and/or exemptions, if any. Noticed that the load lines marked on the hull of the vessel do not match the load line certificate. Required to rectify the issue with ABS. 46 CFR 42.07-35

Approved copy of PSTPs not on board, vessel has developed PSTPs and is working with Sector Puget Sound and MSC on approval and implementation. Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. 46 CFR 61.40-6

Periodic safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. All ships shall have a minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter´s communication shall be carried on board. Those two two-way radiotelephoneapparatus shall be of an explosion-proof type or intrinsically safe. The radios onboard weren't intrinsically safe.

It is the duty of the owner, master, or person in charge to have performed at least once in every 12 months the tests and inspections of all hand portable, semiportable fire extinguishers, and fixed fire extinguishing systems on board, as described in Tables 91.25-20(a)(1) and 91.25-20(a)(2) in § 91.25-20 of this subchapter. CO2 bottles were not weighed during annual inspection.

All electrical installations in hazardous locations must comply with the general requirements of section 33 of IEEE 45-1998. Lighting fixtures in the E-Gen room, over the workbench area in ER, and over the number 3 SSDG covers were missing or inadequate.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. The general service/ fire pump's seal was leaking.

Gyro Compas Repeaters for Bridge wing consoles were inopperable. Durring DDX gyrocompass repeaters were replaced with defective units. New units are on order and will be installed at first availability.
