Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Identified designated E lights were inoperable. Identified there was no E light in the forward gear locker on the car deck and within the steering compartment. Repair E lights and demonstrate operation to attending Marine Inspector(MI).

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable. Unable to verify satisfactory operation of automatic fire dampers. Install inspection ports or provide proof of satisfactory damper operation to attending MI.

Each vessel must be provided with a visual/audible alarm at the operating station to indicate a high water level in each space with a non-watertight closure , such as a space with a non-watertight hatch on the main deck. Cardeck flooding alarms found inoperable. Repair cardeck alarms and demonstrate operation to attending MI.

A vessel must carry adequate and up to date charts. Master was unable to show when the electronic charts were last updated. Demonstrate that charts have been updated in the last 6 months. 46 CFR 121.420

Generators operating in parallel must meet the standards of 46 CFR 111.12-11(f). Each generator arranged for parallel operation must have reverse-power. The #2 Generator reverse power relay failed to operate. Repair or replace the relay and until such time, parallel operation is not permitted. Demonstrate proper operation to attending CG Marine Inspector.

A door must be capable of operation from either side by one person. The door from the galley to the weather deck required excessive force to open/close and dog down. Ensure the hinges and dogs are able to operate effectively and secure closed to meet the requirements for an A class fire boundary. 46 CFR 116.435(b)(2) & (c)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The lever to deploy the MES is located well above the weather deck and requires crew to stand on a ladder in order to deploy the system. This arrangement puts the crew member well over the railing height and in danger of falling overboard in rough weather. Within 30 days, provide a plan to mitigate the risks currently associated with deploying the MES or a plan to replace the current system. Subchapter K requires a marine evacuation system used for embarkation of survival craft to meet 46 CFR 160.175 standards. That standard is no longer listed in the CFR. Without clear guidance on the requirements for the MES, the OCMI will attempt to use good marine practice proposed by the operator to address the above safety issue. 46 CFR 115.800(a) 46 CFR 117.150(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of th hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations: Currently the number 3 generator inoperable directly affecting the ability to use the ships bow thruster. Make repairs to the generator to the satisfaction of the attending CG Marine Inspector.

Every person assigned duty as officer in charge of navigational watch or engineering watch, or duty as ratings forming part of a navigational or engineering watch muct receive a minimum of 10 hours of rest in any 24 hour period. The hours of rest required may be divided into no more than two periods in any 24 hour period, one of which must be at least 6 hours in length. All crew members exceeded their working hours without rest. The master shall ensure crew members receive adequate rest prior to returning to passenger operations. 46 CFR 15.1111

Vessel must carry at least one operating rescue boat in the cradle. Repair the passenger grab lines and demonstrate lowering, running and recovery of the rescue boat. 46 CFR 117.210

Prior to carrying passengers demonstrate a man overboard and abandon ship drill to the satisfaction of attending CG Marine Inspector. 46 CFR 122.520

In accordance with 46 CFR 118.400 (e)(1) your vessel is required a smoke actuated fire detecting system of a type approved by the Commandant that is installed in accordance with 46 CFR 76.35. While conducting tests it was noted that your silence actuating button was not functioning properly on your fire detecting system. Make repairs within the 90days.

Prior to carrying of passengers repair the #1 steering system and demonstrate to attending CG Marine Inspector. In addition, provide technicians report.

The #2 Steering system motor control when in local does not allow the pump to swing to the port, when in emergency mode. However, the steering system functions in all modes from the bridge including emergency mode with both pumps. Repair emergency local steering for #2 pump and demonstrate to the satisfaction of attending marine inspector.

Repair the exhaust fan shut-down device and re-test the suppression system to the satisfaction of attending Coast Guard Marine Inspector.

Repair oil content meter, prove proper operation of OWS.

Replace anti-freeze in sprinkler system.

Make sprinkler tank open for inspection.

Prove proper operation of #1 steering system in follow up mode. Repair to be completed no later than 23 January 2010.

Replace/install overboard discharge valve in fire main system.

Conduct hydrostatic test on both ships heating boilers.

Repair weather deck talk stations to audible condition.

Adjust port weather door in forward viewing area to prevent from sticking.

Governor actuator plug was missing its retainer clip and vibrated loose causing generator prime mover shut down. Vessel shall make repairs to plug to ensure it is unable to vibrate free.

Remove all flammable paints, solvents, etc from boatswains (bosun) compartment.

Provide elevator certification and post as required. System shall be locked out until item cleared by attending MI.

Prior to operation of stove have area clean and ready for inspection. Cleanliness must be to the satisfaction of the attending CGMI.

Flush system until discharged fluid is meets system required Propylene glycol level as required in system design as well as NFPA 25. Flush shall be sufficient to eliminate any corrosion/ foreign matter in branch lines.

Identify required system maintenance and testing procedures, with demonstration of implement schedule as required by manufacture and NFPA 25.

Upon temporary removal of one of your MEC unit vessels total person count shall be limited to 120 persons until completion of annual servicing & replacement of unit on vessel.
