Electric installations on vessels must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. The fuel transfer pump is energized by opening the electrical cabinet and manually pushing the motor’s electrical contact button. Repair to operate as designed by the manufacturer to the satisfaction of the attending marine Inspector. 46 CFR 111.01-1(a)(2)

Table 1 to § 95.50-10(b) indicates the minimum required number and type of extinguisher for each space listed. Extinguishers with larger numerical ratings or multiple letter designations may be used if the extinguishers meet the requirements of the table. Internal combustion or gas turbine propelling machinery spaces 40-B; 1 for each 1,000-brake horsepower; not less than 2 but not more than 6. Vessel is 4,000 brake horsepower and only has two 40-B hand portable fire extinguishers. Provide two additional 40-B hand portable fire extinguishers to comply with the regulation. To the satisfaction of the attending Marine Inspector. 46 CFR 95.50-10(b)

The five (5) Pro Nautic battery chargers are not installed in accordance with the manufacturer’s requirements due to missing overcurrent, creating an electrical hazard. Install the correct amperage overcurrent protection on the output side of the battery charger in as per the manufacturer to the satisfaction of the attending Marine Inspector. 46 CFR 111.01-1(a)(2)

The transfer procedures required by § 155.720 must contain, either in the order listed or by use of a cross-reference index page: A description of each transfer system on the vessel including: A line diagram of the vessel's transfer piping, including the location of each valve, pump, control device, vent, and overflow. Missing the fuel manifold transfer system line diagram. Provide the fuel transfer line diagram to the satisfaction of the attending Marine Inspector. 33 CFR 155.750(a)(2)(i)

Gears, couplings, flywheels, and all rotating machinery capable of injuring personnel must be provided with adequate covers or guards. The starboard steering gear pump coupling protective cover is missing. Install protective cover to the satisfaction of the attending Marine Inspector. 46 CFR 58.01-20

Each forced draft fan induced draft fan, blower of an inert gas system, fuel oil transfer pump, fuel oil unit, fuel oil service pump, and any other fuel oil pumps must have a stop control that is outside of the space containing the pump or fan. Attending Marine Inspector could not witness the fuel pump emergency stop due to a faulty pump controller. Repair fuel transfer pump controller and test the emergency stop in the presence of the attending Marine Inspector. 46 CFR 111.103-9(a)

(a) In General.-A vessel engaged in commercial service that does not carry passengers, shall maintain a video surveillance system. (c) Placement of Video and Audio Surveillance Equipment.- (1) In general.-The owner of a vessel to which this section applies shall install video and audio surveillance equipment aboard the vessel not later than 2 years after enactment of the [name withheld] Coast Guard Authorization Act of 2022, or during the next scheduled drydock, whichever is later. (2) Locations.-Video and audio surveillance equipment shall be placed in passageways on to which doors from staterooms open. Such equipment shall be placed in a manner ensuring the visibility of every door in each such passageway. While reviewing the vessel’s SASH Policy, company removed mattresses to allow for only 09 crew on the vessel. The COI has total persons allowed at 24. Company must install video and audio surveillance equipment in passageways on to which door from staterooms open.

a) Fire detection, manual alarm, and supervised patrol systems are not required except in special cases; but if installed, the systems must meet the applicable requirements of 46 CFR, part 76 of subchapter H (Passenger Vessels) of this chapter. (a) Detectors must be responsive to heat, smoke, or other products of combustion, flame, or any combination of these factors. Smoke detector located in the galley does not work.

Each individual bilge suction shall be fitted with a suitable bilge strainer having an open area of not less than three times at of the suction pipe. In addition a mud box or basket strainer shall be fitted in an accessible position between the bilge suction manifold and the pump. There are no strainers for the bilge suction piping.

Clear instructions must be provided on the vessel that detail the actions each person on board should follow in the event of an emergency. Crew was unable to initially operate bilge suction during test. Need clear instructions for operation and valve alignment.

Each vessel must carry not less than 12 rocket parachute flares approved under approval series 160.136. Vessel outfitted with six hand red hand and six hand orange smoke flares. Vessel restricted from carrying persons other than crew until 12 rocket parachute flares approved under approval series 160.136 are provided. 46 CFR 199.60(c)

Each life raft or group of life rafts must be arranged for float-free launching. The arrangement must ensure that the lifer aft or life rafts, when released and inflated, are not dragged under by the sinking vessel. Port side life raft mounted on the deck under the ladder leading to and from the main deck and 01 deck and the 01 deck overhanging superstructure. Relocate life raft to ensure the arrangement is float-free from any surrounding obstructions. 46 CFR 199.130(c)(7)

All fire hose must be subjected to a test pressure equivalent to the maximum pressure to which they may be subjected in service, but not less than 100 psi. Fire main pressure was recorded to be 80 psi. Provide a test pressure of at least 100 psi to the satisfaction of the the attending MI conduct the fire hose inspection. 46 CFR 91.25-20(a)(4)

Vessel does not have updated notice of intent (NOI), vessel must submit and update NOI.

Vessel has inacurate rudder angle indicator. Ensure rudder angle indicator operates properly.

Lifefloat weak link was not the appropriate size vessel had 11-20 persons instead of 21 or more. Replace weak link with correct capacity.

EPIRB battery and hydrostatic release were both expired. Both need to be replaced.

Vessel has multiple navigation lights in-operable. Replace any in-Op lights.

Multiple engine fuel filters are missing plugs. Replace all missing plugs in all fuel strainers.

Alcohol test strips onboard were expired. Obtain up to date test strips.

Tachometer doesn't work when going ahead on port engine.

Vessel had not conducted an abandon ship drill since 06/22/17. Provide proof of proper logging of conducted drills.

Vessel only had 10 good lifejackets onboard. Restricted to 10 persons onboard until vsl obtains more lifejackets.

Fuel filters missing flame shield. Provide evidence of installed flame sheilds on filters.

Shaft packing leaking in excess. Tighten shaft packing to prevent leaking.

Show proper operation of ground indicator on switchboard.

Doors on back deck to engine room not weathertight. Tighten doors and proove weathertight.

Crew could not prove to MI that they could retrieve somebody from the water, lack of experience and roles and responsibilities.

Lagging missing on exhaust.

Batteries in the engine room and passageway were not properly secured in boxes that could prevent them from moving.

Interlock on switchboard would not work properly because door of switchboard was not properly securied.

Pressure gauges on air recievers were cracked and unreadable.

Provide registration sticker for EPIRB

Vessel was noty keeping any logs as required. To be corrected at next annual inspection.

Epirb monthly checks not being logged. To be cleared at next annual inspection

Port, Stern and anchor lights not working.

Alcohol test strips were expired by 2 years.

SHAFT SPEED INDICATORS INOPERABLE AT BRIDGE CONTROLS. PROVE PROPER OPERATION OF SHAFT TACH. CITE: 46 CFR 113.37-5. DUE: 05DEC2014.

Renew aft air receiver due to wastage behind data plate.

Repair sound power phone ringer from bridge to engine room.

Wire between PORT Engine Room Blower and its control box needs to be replaced with approved marine wiring. Wire sparked when blower was place in on position.

Battery backup for nav equipment and Bridge Emergency equipment not working properly. Show proper operation of emergency power for navigation/General Alarm/PA.

Steering system only reaching 5 degree hard port and 20 degrees hard STBD. Further investigation revealed improper Ram placed in system. Replace ram and demonstrate proper operation of steering gear.

Renew all flexible hases as marked on both main engines, both generators, and the bow thruster engine (approx 12 hoses).

Provide and install shaft guard on the dirty oil pump.

Provide and install new general alarm light in the Engine Room and provide warning placard.
