The aggregate capacity of the electric ship's service generating sources required in § 111.10-3 must be sufficient for the ship's service loads. Master reported insufficient service loads on #2 Generator during electrical transfer after routine EDG testing. Identify cause of the casualty, make effective repairs and prove proper operation of the electrical plant to the satisfaction of the CG.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Vessel is experiencing a brake heating issue on B end main engine while B end is forward engine. Effect permanent repairs to the satisfaction of the attending Marine Inspector, prior to carrying passengers.

An inspection of the bridge-to-bridge radio stations on board vessels subject to the Vessel Bridge-to-Bridge Radiotelephone Act must be conducted by the same FCC-licensed technician. The vessel's FCC Bridge-to-Bridge and Safety Radiotelephony Certificates are expired. IAW 47CFR80.59

Provide repair plan to the Coast Guard for review and carry out approved repairs to the satisfaction of the Coast Guard prior to carriage of passengers or cargo.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation shall be IAW subchapter "F" of this chapter. Vessel reported an exhuast leak on the emergency gen. NCDOT must replace flex pipe with in-kind equipment and provide proof of repairs to attending marine inspector.

Crew reported that during a layover dockside in Aurora Terminal, while running a boat drill it was noted the rescue boat went into limp mode. One of the bilge pumps if the emergency power source is its source of power to meet part 56 of this chapter. Emergency bilge pump will start but not take suction while on emergency power. Provide proof to attending MI that emergency bilge pump is taking suction. 46 CFR 112.15-5(d) 46 CFR 56.50-55

The aggregate capacity of the electric ship's service generating source must be sufficient for the ship's service loads. Vsl self reported that both SSDG and EDG could not maintain constant amperage causing fluctuations in electrical services. Vsl must provide generating sources which provide sufficient electrical service for the ship's service load. Vsl is authorized a one time deadhead transit from Aurora, NC to Bayview, NC for repairs. 46 CFR 111.10-4

The master reported that during morning boat checks the "B" end throttle control head was discovered to be leaking air. Trouble shoot, replace the defective equipment, and operational test to the satisfaction of the Coast Guard, prior to the carriage of passengers.

Each life jacket must be marked with type-1 retro-reflective material approved under 46CFR164.018. The arrangement of the retro-reflective material must meet IMO resolution A.658(16). The retro-reflective tape on 18 adult sized life jackets were found wasted. Vessel restricted to 292 passengers.

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. Ground was detected on the main switchboard while the generator was online with load.

Simultaneously with the operation of the transfer means under § 112.20-5, the diesel engine or gas turbine driving the final emergency power source must start automatically with no load on the final emergency power source. During morning daily checks the emergency generator failed to start. Determine cause of the casualty, make effective repairs and prove proper operation to the attending Marine Inspector.

Operating generators must provide continuous and uninterrupted source of power for the ship's service load under normal operational conditions. Master reported high jacket water temperature alarm on #2 generator due to possible faulty sensor. Provide cause of casualty, make effective repairs and prove proper operation to the attending Marine Inspector. If the #2 generator is needed for normal use, constant rounds of the equipment are required with additional monitoring measures.

In general, compliance with the standards established by the American Bureau of Shipping, see Subpart 70.35 of this chapter, will be considered satisfactory evidence of the structural efficiency of the vessel. The maintenance supervisor, reported it was discovered the vessel had structural damage to internal framing on board. Company must carry out temporary welding repairs to the two fractured transverse frames in "A" end steering compartment per submitted repair plan.

The maintenance supervisor, reported it was discovered the vessel has structural damage to internal framing on board. This damage was reportedly identified on three different framed areas in the vicinity of the "A' end steering compartment. The vessel is prohibited from movement until an attending Coast Guard inspector carries out an internal structural examination to their satisfaction and an approved repair plan is provided to the satisfaction of the Coast Guard.

Emergency generator must have audible alarm that sounds on high cooling water temp. Alarm did not sound during test. Prove proper operation.

Automation of vital systems or equipment must be provided on passenger vessels of 100 gross tons. Prove proper operation on reduction gear pressure test IAW PSTP 3.0.

A self priming, power driven fire pump must be installed on each vessel: (iii) Of more than 19.8 meters (65 feet) in length. Vessel notified OCMI office stating fire pump was not operational. Make repairs as needed and provide proof to attending inspector.

Vessel is required to carry radio equipment to comply with the requirements are specified in 47 CFR part 80. Integrate VHF radio with GPS and conduct marine radio check IAW USCG MSIB 20-20.

Design verification and periodic test procedure documents approved by the Coast Guard must be retained aboard the the vessel. Approved documents were not onboard during COI inspection.

Each vessel must carry a lifejacket for each person onboard and in addition at least 10% of child sized life jackets. MI discovered 46 adult sized life jackets and 01 child sized life jackets unserviceable. Vessel is restricted to 259 passengers.

Classification: 02 CO2 fire extinguishers IVO of the port and starboard emergency access not properly labeled with classification.

Vessels crew discovered the #2 SSDG would not secure during morning engineering checks. Vessel needs to further investigate and conduct repairs and ensure full operational test are satisfactory as per manufactures specs.

Employers must ensure that they and their crewmembers meet the requirements of 46 CFR 16. Crewmembers are unsure who the designated employee representative is and has not received drug awareness training. Company's policy or policy statement were not onboard.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. During man overboard drill it was identified that the rescue boat engine throttle was inoperative.

Cargo nets hooks missing 3 pins on A and B ends.

Fire extinguisher mount #9 was loose.

Drink machine bracket missing bolt and loose. Properly secure.

Rescue boat fire extinguisher wasted.

Recues boat davit controls getting stuck in the closed position when swing vsl overboard. The master must manual disengage controls locally at davit.

#12 fire ext. discharge hose cracking, replace with new.

"Crews" first aid kid meds expired.

Prove 50% of crew CPR first aid. 7 crewman checked for CPR first aid card and only one was vaild.

CO2 bottle hose for e-gen loose.

A and B end shafts need to be adjuested to proper drips per min as per manufactures spec.

Deck hatchs were leaking on A and B end of LAZ.

Renew wasted lashings IWO all liferafts.

Renew rubber latch IWO 01 deck life jacket stowage box.

Lubricate EDG door (very hard to operate)

Rescue boat lifting shackles wasted, replace with new.

A-END R/W PVC piping union leaking in shaft alley.

Painter line grommet missing for liferaft casing.

B-end lifejacket markings missing or damaged.

Identified 3" hole caused by wastage in sewage supply line to MSD. Make permanent repairs to the satisfaction of the attending marine inspector.

Vessel experienced temporary blackout and subsequently left alarm panel illuminated post casualty.

Vessel reported (2) pin hole leaks in fire main.

Pin hole leak in fire main.

Pin-hole leak in fire main found directly above main engine (B-end).

Electric Starter not engaging on A End of vessel, A Main will not start.

Identify and correct the ground in the main switchboard.

Properly label the motor controller in the engine room IWO the base of the ladder.

Prove proper operation of the #1 bilge pump pressure gauge.

Attending inspectors noted wastage on bottom of WTD leading into E/R. CG-835 issued to repair/replace WTD at next drydock period.

Attending inspectors noted that the gasket in the watertight door between the E/R to fuel tank space was showing signs of deterioration. CG-835 issued to replace NLT 20Jul2012.

Attending inspectors noted that the "B" End MDE F/O Strainer assembly (RACOR) was missing it's flame impingiment bowl assembly.

Attending inspectors noted that the Chief Engineer on board vessel reported >10% Fuel Oil Dilution on "B" end MDE. Engineers have been troubleshooting engine with negative results. Chief Engineer will continue to monitor FOD and change oil as per manufacturers instructions.

Repair grab lines on life ring "B" end Stbd side and "B" end Pilothouse deck.

Repair rub rail ond Gel Coating on Rescue boat Port side.

Replace two (2) missing child life jackets on "B" end passenger deck and one (1) missing Child life jacket on "B" end deck box.

Prior to the daily operation of the vessel, The Chief Engineer will conduct a daily inspections of both end Steering quadrant's and log results in the engineering log. This daily inspection will continue until proper repairs have been completed and inspected by the attending Marine Inspector.

Renew rudder bearings on both the A & B end due to excessive play.

Repair four watertight doors from engine room Fore and Aft. All doors are dragging due to worn washers on the hinges.

Renew automatic heat detector due to mechanical damage (dented dome). The system is still fully operational, but concerns regarding the teperature actuation point require renewal.
