It was report the both shaft brakes on both A end and B end were not working. Conduct repairs and test to the satisfaction of attending CG Marine Inspector prior to carrying passengers.

Vessel is allowed to conduct sea trials with no passengers onboard.

The crew self reported finding four fractures of welds in the water tank (A) end compartment in the overhead on the underside of the vehicle deck, during routine checks. Company Representative reported they took the vessel out of service and they intend to order marine chemist and coordinate welders for repairs. Provide repair plan to the Coast Guard for review and carry out approved repairs to the satisfaction of the Coast Guard prior to carriage of passengers or cargo.
