A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Ring buoy waterlight did not have battery. Vessel is restricted to daytime operations only. 46CFR180.70(d)

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Ring buoy water light lanyard was dry rotted and in need of replacement. 46CFR180.70(d)(2)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. One of the life jacket stowage locations was not properly marked. 46CFR180.78.(a)(5)

33 CFR 155.450 - A ship, except a ship of less than 26 feet in length must have a placard of at least five by eight inches, made of durable material fixed in a conspicuous place in each machinery space stating the following... (see 33 CFR 155.450). Vessel did not have a Discharge of Oil Prohibited placard posted.

46 CFR 181.520 - Portable fire extinguishers installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Neither of the vessel's two required portable fire extinguishers were properly mounted.

An adult life jacket must be provided for each person carried on board a vessel. All but 6 adult life jackets found in unserviceable condition. Remove and destroy all unserviceable lifejackets. Vessel restricted to carrying 6 adults maximum until required number of lifejackets are replaced.

A vessel of >26 ft in length, but <65 ft, must carry a minimum of one (1) life buoy of not less than 24 inches in diameter. This ring lifebuoy must have a lifeline and floating waterlight. Ring lifebuoy is wasted and battery for waterlight is expired.

A vessel on a rivers route must carry: three red hand flares and three hand orange smoke signals. A hand red flare may be substituted for any of the hand orange distress signals. Vessel's only distress signals onboard at time of inspection were 4 hand red flares. Vessel limited to short runs <30 minutes from shore.

The owner, managing operator, or a qualified servicing facility as applicable shall conduct the following inspections and tests: for portable fire extinguishers, the inspections, maintenance procedures, and hydrostatic pressure tests required by Chapter 4 of NFPA 10. Portable fire extinguishers overdue for requisite servicing.

The owner, charterer, master, or managing operator shall instruct each crew member at least once every 3 months as to the duties that the crew member is expected to perform in an emergency [185.420]. The master shall conduct sufficient drills to make sure that all crewmembers are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Each abandon ship drill must include...[185.520]. Each fire drill must include [185.524...]. Crew training and drills shall be logged or otherwise documented for review by the Coast Guard upon request and shall include the date of the drill and general description of the drill scenario/topics. No records of crew training/drills available onboard at time of attendance.

Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the Serious Marine Incident within 2 hours of such an incident. No alcohol testing kits available onboard at time of inspection.

Each portable fire extinguisher on a vessel must be of an approved type. The installation and location of portable fire extinguishers must be to the satisfaction of the Officer in Charge, Marine Inspections. 3A:40BC extinguisher missing proper bracket.

The master of a vessel shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The passenger count shall be available to the Coast Guard upon request. Establish means of maintaining current passenger count ashore that meets above requirements.

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03–2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Not all crewmembers presently enrolled in DAPI program/drug testing consortium at time of inspection.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents. Eye wash in first aid kit found to be expired.

Each flare carried to meet this section must be stowed in one of the following: a portable watertight container marked as required by 185.614 of this chapter ("DISTRESS SIGNALS"), carried at the operating station. Flares not stored in proper container.

As appropriate for the intended voyage, a vessel must carry accurate and up-to-date: 2) U.S. Coast Pilot, 3) U.S. Light List, 4) Tide tables, 5) Current tables. Extracts may be provided instead of the complete publications. Publications not up-to-date.

Vessel missing 01 orange smoke flare. Provide 01 orange smoke or 02 red flares.

Vessel does not have means to conduct post-casualty alcohol testing. Provide a means to conduct post-casualty alcohol testing.
