The company should establish procedures for the implementation of corrective action, including measures intended to prevent recurrent. Additionally in meeting these requirements, the company should ensure that any non-conformity is reported, with its possible cause, if known. As documented in the vessel's safety management system [NON CONFORMANCE HANDLING CIM-PP12-4.3-WI] a non-conformance can be issued due to underperformance of purchased goods [in this case, gasket P/N 90810-0063 107]. Upon request by PSCO, no documentation pertaining to a raised non-conformity could be provided by the crew. Upon statements by engineering crew [chief engineer, second engineer] an unapproved rubber packing gasket had been utilized [in lieu of designed manufactured part] since August 2017 on all cylinders, and most recently replaced with a new unapproved rubber gasket in September 2017 [for the #1 cylinder]. 74SOLAS(14CONS)IX3.1 ISM CODE 9.2/ISM CODE 10.2.2 50AC
