All escape hatches and other emergency exits shall be marked on both sides using at least 1 inch letters: "Emergency Exit, Keep Clear", unless such markings are deemed unnecessary by the Officer in Charge, Marine Inspection. Port and starboard emergency exit doors on the slots machine deck did not have visible emergency exit signage due to slot machine obstruction. 46 CFR 185.30-15(a)

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Port main diesel engine shaft seal had a constant stream leak. 46 CFR 182.01-5(a)

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: (1) Where the internals of the vessel are completely concealed, sections of the lining or ceiling may be removed or the parts otherwise probed or exposed so that the marine inspector may satisfy himself as to the condition of the hull structure. Fixed ballast under the deck in the galley was inaccessible for inspection due to laminate and flooring. 46 CFR 176.25-5(a)(1)

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Large build up of grease was found in the ventilation ducts for the oven hood in the galley. 46 CFR 176.25-50(b)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. Marine inspector observed fire pump pressure gauge not working as designed, and could not verify required PSI. 46 CFR 181.10-1(c)

On a vessel on which the number of passengers permitted on the upper decks is limited by stability criteria... the maximum number of passengers allowed on the upper decks must be indicated by a durable marking of numbers and letters at least one inch in height at the entranceway to each such deck. Marine inspector observed that the 2nd deck did not have a marking indicating the maximum allowable passengers on that deck. 46 CFR 185.30-3(e)

Flexible hose shall be fabricated with an inner tube and a cover of synthetic rubber or other suitable material reinforced with wire braid. The hose cover shall be adjudged self-extinguishing as per ASTM D1692, and the hose assembly be capable of withstanding free burning gasoline for 2 1/2 minutes. Identified fuel oil supply lines exterior shielding missing and metallic sheathing wasted for the port and stbd main diesel engines and center generator. 46 CFR 182.20-40(a)(2)(ii)

Identified partially collapsed raw water supply non-metallic hose from the sea strainer to the raw water pump on starboard main diesel engine. Additionally, hose is missing two hose clamps. 46 CFR 182.20-10(b)(1)

Each inflatable liferaft, inflatable buoyant apparatus, life float, and buoyant apparatus on the vessel on March 11, 1996, may be used to meet the requirements of this part for these survival craft as long as the survival craft is continued in use on the vessel, and is in good and serviceable condition. MI observed 03 of 04 lifefloats with tears, gouges, and water pockets. 46 CFR 180.15(b)

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. MI observed cable penetration located above water tight door in the engine room that was not sealed with multiple wires passing through. 46 CFR 179.320(c)

Examine all accessible parts of the exterior and interior of the hull and the watertight bulkheads… MI observed water leaking through a hole located in the aft Starboard engine room overhead outboard side. It appears from deck above that it is coming from A/C unit. 46 CFR 176.25-5

A weathertight door must be provided for each opening located in a deck house or companionway. MI observed a quarter inch gap where doors sealing edge meets frame on both port and starboard main deck aft. 46 CFR 179.360(d)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] observed a door on the 01 deck secured close with a waterline to a wash station. Alteration was made without notification to the [name withheld] removed the water line and made door operational. 46 CFR 176.700(a)

An adult life jacket must be provided for each person carried on board a vessel. MI observed 168 adult life jackets and COI requires 170 onboard. Two adult life jackets were added bringing total to 170 adult life jackets correcting deficiency on the spot. 46 CFR 180.71(a)

A vessel...may not be operated without having on board a valid U.S. Coast Guard Certification of Inspection. Vessel's COI expired on 15JUN21 and a COI must be issued prior to the carriage of passengers.

Each EPIRB must be tested monthly and logged or otherwise documented. The operator has not logged the monthly testing of the EPIRB. 46 CFR 185.728

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts. The chart found onboard was last updated in 2017. Update the chart using LNM or purchase a new chart. 46 CFR 184.420

As appropriate for the intended voyage, a vessel must carry a U.S. Coast Pilot. Provide a 2021 Coast Pilot for the intended voyage. 46 CFR 184.420

Each vessel certificated to operate on a limited coastwise route in warm water must be provided with life floats of an aggregate capacity that will accommodate 50% of the total number of persons permitted onboard. One (22) person lifefloat was found damaged. Three (22) person lifefloats are onboard in serviceable condition. Total number of persons permitted onboard reduced by 38. 46 CFR 180.205(c)

All unfired pressure vessels shall be Coast Guard approved and installed to the satisfaction of the OCMI in accordance with subchapter F. The pressure relief device for the 200 psi MAWP pressure vessel in the engine room does not have markings to indicate the relief pressure. Replace the pressure relief device with one under 200 psi. 46 CFR 182.10-5 (d)

Auxiliary machinery shall be suitable in type and design for the purpose intended. Threaded plug in the hydraulic tank located in the engine room was found leaking due to improper installation. Re-install plug to prevent tank from leaking. 46 CFR 182.10-1

All electrical installations shall be to the satisfaction of the [name withheld] identified a battery charger connected to engine starting batteries on the starboard side and 24 volt batteries on the port side. Installation of the battery charger was incorrect. Adjustments to the charger for temperature compensation and battery type were not made prior to installation. The batteries were found overcharged and leaking water from the battery. Hydrogen gas was witnessed off gassing from the battery due to overcharge creating an explosive atmosphere in the engine room. Corrosive vapors and liquid from the batteries have destroyed the battery tray. Install battery charger in accordance with the manufacturer manual. Replace battery trays under batteries. 46 CFR 183.01-15

Conductors shall be protected from overcurrent in accordance with their current carrying capacity using a UL 489 circuit breaker or UL listed fuse (class CC, G, H, J, K, RK1, RK5, and T) in accordance with 46 CFR 183.05-35 and 46 CFR 183.380 (L). The conductor installed between the battery charger and 12V/24V batteries in the port/stbd engine room does not have an overcurrent device installed. Install overcurrent protection on the conductor between the battery charger and batteries in accordance with the conductor's current carrying capacity. 46 CFR 183.05-50(C) AND 46 CFR 183.380 (L)

A self-propelled vessel of 65 feet or more in length, engaged in commercial service shall have an AIS Class A or B device installed on board. AIS was not found installed on board. 33 CFR 164.46(b)(2)(ii)

A suitable rescue boat shall be required except when a rescue platform, of a design approved by the OCMI, is installed to the satisfaction of the OCMI. The rescue platform landing shall be constructed of expanded metal mesh to reduce the effect of wave action. 46 CFR 180.10-35 and Local USCG St. PETE OCMI policy

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: EMERGENCY EXIT, KEEP CLEAR. Identified the emergency exit for the engine space located on the port side without appropriate signage on the main deck. Passengers or crew may unintentionally block this escape from above. Place appropriate signage above the emergency escape from the engine space. 46 CFR 185.30-15
