To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in § 176.500(b)(1) and your Certificate of Inspection must be endorsed. The vessel did not complete an annual inspection within the periods specified in § 176.500(b)(1). 46 CFR § 176.502

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. The current DAPI enrollment indicates that the relief master is not enrolled in the DAPI program. The vessel is not permitted to transit on voyages that require a relief crew until all employees are enrolled in the DAPI. 46 CFR 16.201

A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. The public address system was not audible when tested. Static was heard while the master was addressing crew through the PA speaker. 46 CFR 184.610

Each life jacket carried on board the vessel must be marked in accordance with § 185.604 of this chapter. 46 CFR § 180.71 Each life jacket must be marked with Type I retro reflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retro reflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). The crew stenciled the name of the vessel over the retro reflective material on every life jacket onboard. 46 CFR 185.604

All ducts shall be of rigid permanent construction of fire proof material and reasonably gas tight from end to end. The ventilation ducts located next to the bridge have holes in the metal next to the vents which prevents them from being made gas tight. 46 CFR 182.20-45(d)

Auxiliary machinery shall be suitable in type and design for the purpose intended. The aft generator belt was found unattached and torn. 46 CFR 182.10-1

The fire pump shall be self priming and of such size as to discharge an effective stream from a hose connected to the highest outlet. The fire main piping on the discharge side of the pump was found leaking water in the engine room and contained several unauthorized temporary repairs. 46 CFR 181.10-1

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. The fire extinguisher located on the main deck forward is not marked. Standardize the markings for fire extinguishers onboard. One 2-A fire extinguisher shall be installed to protect the 2nd deck. 46 CFR 181.520

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters:"Emergency Exit. Keep Clear." The markings for the emergency exit located forward on the main deck are not visible from a standing position in the accom area. 46 CFR 185.30-15

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Provide proof that the medical kit complies with ISO 18813:2006(E) paragraph 4.12. 46 CFR 184.710

The master shall conduct drills and give instructions as necessary to ensure that all crew members are familiar with their duties. The last logged drill was in August 2023 and the vessel recently operated on a limited coastwise route in February 2024. The crew were not familiar with the procedures for a man overboard recovery drill during a drill that was witnessed by the marine inspector. 46 CFR 185.25-10

46 CFR § 160.027-2; Each life float must meet the requirements in subpart 160.010 of this chapter. 46 CFR § 160.010-4; Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. The material must be in strips at least 50 mm (2 in.) wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. For peripheral body apparatus, each strip must extend completely over the top and bottom surface of the body. For box type apparatus, the strip must extend at least 300 mm (12 in.) inboard from the edge over the top and bottom surface. Each strip must be positioned near the center of the side or end, but so that it is not obscured by any strap. The retro reflective material was found wasted on both life floats. 46 CFR 180.200(a)(2)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). The stenciled name of the vessel is blocking the retro reflective material on the ring buoy. 46 CFR 185.604(i)

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. The vessel has two stowage areas for lifejackets. The lifejackets stowed on the starboard side do not have an approved light. The operator stated that these lifejackets are for limited coastwise voyages to Tinian. The lifejackets without lights are to be removed from the vessel or lights are to be added prior to any overnight voyage or coastwise voyage. 46 CFR 180.75

All vessels with crew members living on board shall be provided with crew accommodations of sufficient size, adequate construction, and with sutiable equipment to provided for the crew. The crew bunk located on the bridge measures approximately 20" wide. A minimum width established by Lounge Arrangements requires at least 24" width. 46 CFR 177.25-1

Gauges to indicate engine cooling water discharge temp and lube oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. The port main engine temp sensor was not operational while the engine was energized. 46 CFR 182.20-5

All vessels must have navigation lights that are in compliance with the applicable Navigation Rules. The anchor light was not operational when tested. 46 CFR 183.420

An electrical installation that has any current carrying conductor intentionally connected directly or indirectly to the metal hull shall be subject to certain additional requirements as determined by the OCMI as per subchapter J. The ground fault detection cannot be verified to be operational by the push test button. Prove that the ground fault detection is operational. 46 CFR 183.10-30

Main distribution panels shall be of the dead front type where voltage exceeds 150 volts AC. The distribution panel on the port side of the bridge is missing a dead front cover for one breaker slot. 46 CFR 183.10-15

Batteries used for engine starting are to be located as close as possible to the engine. The starting battery cable is too long for both of the generators. The battery cable is wrapped around itself 6 times instead of being shortened for the distance between the battery and starter. 46 CFR 183.350

Exhaust piping shall be gas tight throughout its length. Identified one missing bolt on the exhaust coupling for the forward generator. Identified an unauthorized temporary repair on the exhaust piping located in the lazarette space that is allowing exhaust gas into the space. 46 CFR 182.20-20

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. The SUPER EMERALD COI expired on April 1, 2024. The vessel is not permitted to operate with passengers until the COI is renewed. 46 CFR 176.100(a)

All vessels shall be provided with bilge pumps in accordance with Table 182.25-10(a). The fixed bilge pump and manifold piping system located in the engine room was unable to provide suction from the lazarette space when tested by the crew. 46 CFR 182.25-10

Except as noted in paragraphs (d) and (e) below, each vessel must be equipped with a public address system. On a vessel of not more than 19.8 meters (65 feet) in length, a battery powered bullhorn may serve as the public address system if audible throughout the accommodation spaces of the vessel during normal operating conditions. The bullhorn's batteries are to be continually maintained at a fully charged level by use of a battery charger or other means acceptable to the cognizant OCMI. On a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, a public address system is not required if a public announcement made from operating station without amplification can be heard throughout the accommodation spaces of the vessel during normal operating conditions, to the satisfaction of the cognizant OCMI. The vessel is less than 65 feet and a bullhorn or public announcement cannot be heard on the main deck from the pilot house. Provide a test of the public address system. 46 CFR 184.610

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The medium frequency radio was not operational during the inspection. The vessel is restricted to voyages no further than 20 miles from shore. 46 CFR 184-25-1

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessels with a keel laid date after March 28, 2022, must include plans for the monitoring device(s) within the plan submissions required in 46 CFR 177.202. The Coast Guard will work with the vessel operators to determine a reasonable implementation schedule once the plans are accepted. The monitoring device(s) must: (1) Ensure the wakefulness of the crew in the event that the watchman required in paragraph (a) of this section is unresponsive; (2) Remain operable during the nighttime watch; and (3) Be arranged to ensure proper coverage of the passenger accommodation spaces, common areas, and spaces with potential fire hazards. Submit plans to MSC for the installation of a watchman monitoring device. Contact USCG for verification of plans once approved. 46 CFR 175.110(d) 46 CFR 185.410(b)

The master of a vessel described by 46 CFR 175.110(d) must conduct passenger emergency egress drills from the passengers' assigned overnight accommodation spaces prior to beginning an excursion with new passengers. (1) If the passengers are not assigned an overnight accommodation space, the master of a vessel described by 46 CFR 175.110(d) must conduct passenger emergency egress drills from an accommodation space prior to beginning an excursion with new passengers. (2) For the purposes of this section, excursion includes anytime the vessel gets underway, or anytime passengers remain overnight on the vessel. Conduct a passenger emergency egress drill prior to carrying passengers assigned an overnight accommodation space on an excursion. 46 CFR 175.110(d) 46 CFR 185.507

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. The Jim Buoy life floats were found with gouges in the outer layer exposing the foam. Repair the life floats with an authorized [name withheld] repair kit. 46 CFR 180.175(a)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first aid kit found onboard is not equivalent to a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10). Each first-aid kit must meet the requirements in ISO 18813:2006(E). 46 CFR 184.710

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with this part. Replacement of existing equipment, not including wire or cable, installed on the vessel prior to March 11, 1996 need not comply with the regulations in this part. Cable and wire for power and lighting circuits must: (1) Meet Section 310-13 of NFPA 70 (incorporated by reference; see 46 CFR 175.600) except that asbestos insulated cable and dry location cables may not be used; (2) Be listed by Underwriters Laboratories (UL), as UL Boat or UL Marine cable; or (3) Meet § 111.60-1 in subchapter J of this chapter for cable, and § 111.60-11 in subchapter J of this chapter for wire. The newly installed positive conductor from the battery switch to the generator starter is welding cable on not permitted in the electrical system. The connection to the starter was found with a sharp bend creating resistance and heat. Replace with an approved conductor and install without a sharp bend. 46 CFR 183.115 46 CFR 183.340(d)

Suitable storm rails or hand grabs shall be installed where necessary in all passageways, at deckhouse sides, and at ladders and hatches where passengers or crew might have normal access. The railing on the stairwell leading to the second passenger deck was found fractured. 46 CFR 177.35-5

(g) Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. The new installation of wiring for the sewage pump was found with improper connections using electrical tape only. 46 CFR 183.340(g)

Mattress is to be covered with material that has been treated to give it fire retarded property. Must not contain polyurethane foam. Proved proof of mattres meeting requirments in 16CFR1632.

VSL with more than one passenger deck with overnight accommodation must have a public address system. Show proper operation of the installed PA system.

Repair closing devices on AFT watertight door to passenger deck.

Bilge must be cleared of trash, debris and accumulation of oil. Removed rugs, plastic bags and debris form engine room bilge.

Clean and prime (seal) identified interior bottom plate that are susceptible to corrosion.

Remove all identified dead end wires AFT of the Wheel House.

Master shall conduct drills and given instructions to ensure crew members are familiar with their duties. Proved proof of drills conducted.

Life preserver storage shall be marked at least 1" letter " LIFE PRESERVER ADULT and CHILD" with total numbers. [name withheld] preserver storage.

VSL with ocean or coastwise routes must have approved light on life preservers. Install missing lights on CHILD life preservers.

Repair inoperable public address system.

Ring life buoy shall be in good and serviceable conditions. Replace damaged grab line attached to ring life buoy.

Overnight accommodation for passenger must be fitted with independent modular smoke detection an d alarm unit in compliance with 181.450. Install missing smoke detectors.

VSL shall be equipped with suitable number of portable battery lights. Provide operational portable battery lights.

VSL must have adequate emergency lighting fitted in passenger accommodations. Repair and demonstrate proper operations of the emergency lights.

Escape hatches and emergency exist shall be marked both side at least 1" letter "EMERGENCY EXIT, KEEP CLEAR". Mark emergency exit for accommodation space.

Repair wasted and holed starboard engine room ventilation ducts.

Remove temporary installed portable ceiling lights in engine room and repair existing light fixtures.

Repair oil leaks on starboard propulsion engine turbo charger.

Replace missing electrical sensors and reconnect electrical wires - starboard reduction gear.

Make proper electrical wire connection to all identified dead end electrical wires / disconnected electrical wires.

Install missing protective cover on auxiliary drive belts for both Generators.

Repair damaged side of electrical panel on #1 generator.

Repair salt water leak above alternator on #1 generator.

Replace inoperable fuel shutdown solenoid on #2 generator.

Remove gasoline containers, paints and portable generator from accommodation area.

Clean and prime all interior bottom hull plate that were identified as areas with exfoliation corrosion.

Connect lose recirculation breather hose from turbo charger to crankcase.

Repair and show proper operations of emergency fuel shut off remote control valves.

Replace the modified screw on salt water pump cover with in kind screw.

Vessel required to have 6 red and 6 orange flares or substitutes with 12 red flares.

Install missing life jacket lights on 3 child PFDs.

Install missing tags on portable CO2 fire fighting extinguishers.

Mooring lines were found stored in front of the emergency egress exit door.

Fire and abandon ship drills were not documented regularly.

Smoke detectors in the accomodation space and female head did not have batteries installed.

Recommended passenger safety orientation be printed and include discussions of fire and emergency egress.

Power strips were used with household small appliances without grounding pole & conductor.

Life float must be in serviceable conditions. Repair damaged JIMBUOY life float or replace with Coast Guard approved life saving equipment.

Found dead ends wires under control station in wheel house. Remove dead end wires.

Show proper operations of the SSB radio.

Repair inoperable lights in passenger cabin.

Relocate stern light away from light range obstruction.

Electrical equipment must be install and maintain to prevent electreical hazard to crew as require by 46CFR183.200. Remove non marine grade type battery charger.

Exhaust gas must not leak from piping as required by 46CFR182.430. Repair exhaust leak on port / STBD propulsion engine exhaust pipes.

Small battery must be protected from falling as required by 46CFR183.354. Relocate generator battery from the engine foundation to prevent falling.

Back up generator, "A", taken off line due to mechanical failure. Repair to as original or replace in kind.

All type I lifejackets must be properly marked with VSL name as required by 46CFR185.604. 23 new type I lifejackets not marked. Properly marke all new lifejackets with vsl name.

Ring Life Buoy must be marked with vsl name as required by 46 CFR 185.604. Faded vsl name on life buoy. Re- stencil vsl name on ring life buoy.

Fuel filter must be must be installed with steel bowl as required by 46 CFR 182.455. Port racor fuel filter is missing steel bowl. Install CG/manufacture approved steel bowl on fuel filter.

Fuel shutoff for engines must be in operating condition at all time as required by 46 CFR 182.200. Found remote operating fuel shutoff valve on Alpha generator not in operating condition. Show proper operation of shutoff valve to attending MI.

Fuel tank must have means to determining amount of fuel in tank as required by 46 CFR 182.445. Found modified fuel tank sounding tube. Remove or install proper marine type marine fuel gauge.

Part of exhaust pipes between exhaust manifold and cooling water injection must be insulated as required by 46 CFR 182.425. Found Bravo generator missing exhaust pipe lagging. Install missing lagging.
