Design verification testing (a) Tests must verify that automated vital systems are designed, constructed, and operate in accordance with all applicable requirements of part 62 of this sub chapter. The tests must be based upon the failure analysis, if required by §62.20-3(b) of this sub chapter, functional performance requirements, and the Periodic Safety tests of §61.40-6. At each inspection for certification and periodic inspection the MI shall conduct such tests and inspections of the main propulsion and auxiliary machinery. Port outboard MDE low lube oil shutdown not working as intended. 46CFR61.20-3

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure the sip in all respect will remain fit to proceed to sea without danger to the ship or persons on board. Every crew member shall participate in at least one abandon ship drill and one fire drill every month. The drills of the crew shall take place within 24 hours of the ship leaving port if more than 25% of the crew have not participated in abandon ship and fire drills onboard the particular ship in the previous month. Vessel fail to perform fire drills in accordance with regulation. Master shall create a non conformity and corrective action plan.

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering-gear compartment. The rudder-angle indicator must be independent of control systems for steering gear. The port rudder angle indicator located at the main steering station on the bridge was not functioning. Repair or replace the port rudder angle indicator and prove operation in the presence of a Coast Guard Marine Inspector.

Annual maintenance on galley rolling A0 fire doors has not been conducted since delivery of vessel. Create maintenance task in your preventive maintenance system to prevent future occurrences.

Maintenance, testing and inspections shall be carried out based on the guidlines developed by the orgaization and in a manner having due regard to ensuring the reliability of fire fighting systems and appliances. Annual drop test/maintence on galley serving line fire door has not been conducted since delivery of vessel. Creat maintenance task in preventive maintance system to prevent future occurrences. 74 SOLAS II-2 Reg 14.2.2.1 , NFPA 80

Repair/Replace damaged SFP insulation on bulkhead aft of CO2 room IWO fan space.

Emergency use ONLY allowed for port L/B until winch brake is inspected and tested by competent authority. Ammend SMS to reflect additional safety measures necessary in the event of needing to use the L/B for emergency.

Replace 7 unapproved portable fire extinguishers with CG Approved ones.

Replace all "Baleysuit" brand Immersion Suits upon return to Valparaiso, Chile. Manufacturer calls for replacement after 10 yrs of age.

Provide proper closure to penetration duct between Baltic Room and Wet lab.

[name withheld] reported stbd outbd main engine governor had been iscovered to be inoperable and in need of repair. Make proper repairs to the satisfaction of the attending USCG Marine Inspector.

USCG Inspectors found water intrusion into foam filled inner hull and subsequent water logging of foam. In consult with vessel operator, condemned rescue boat. Vessel allowed temporary use of Zodiac inflatable boat while awaiting delivery of new fast rescue boat. Vessel to provide documentation to USCG MI demonstrating installation of approved rescue boat.

Third party inspectors found cracks in the carry areas of the winch brakes on both lifeboat winches. Due to non-availability of replacements, authorized temporary repairs after consult with manufacturer while awaiting replacement part manufacture. Vessel to provide USCG MI with documentation showing replacement of brake carry on both winches and completion of successful davit weight test.

Prove 1.5 MAWP of foam piping system.

Prove 1.25 load test on stern "A" frame crane.

Vessel employs non US citizens in unlicensed deck and engineering positions. Current crew list attached in MISLE. As required by 46 USC 8103 vessel must insure all unlicensed crewmembers are citizens on the United States of America.

Prove propeller/shaft alignment problem has been diagnosed and repaired by an approved life boat facility on life boat #1. Vessel is restricted to the dock in a "no sail" status after the due date assigned.

Provide documentation showing two (2) operational SART's on the vessel.

Test newly installed bulkhead between fuel oil tank 4A and 4B in the presence and to the satisfaction of the attending CG Marine Inspector. This item to be cleared prior to the vessel departing port.

Provide USCG MSC approved plans of the newly installed bulkhead separating fuel oil tanks 4A and 4B. This item is to be cleared no later than 24MAR05.

Properly test fuel oil piping installed in conjunction with alterations made to #4 fuel oil tank to the satisfaction of attending USCG Marine Inspector. This item to be cleared prior to departure from port.

Provide ABS approved drawings for deck strengtheners IWO deck where temporary derrick will be installed. This item to be cleared no later than 24MAR05 or at the time when the derrick is actually installed. The derrick is not to be installed until this item is cleared.

Provide necessary stability information and calculations to USCG MSC for determining if an inclining will be required after the derrick is installed. Copy to be sent to USCG Far East Activities. This item is to be cleared no later than 24 March 2005.

Provide final report of permanent ballast installation to USCG Far East Activities. This report should be included in the submittal to clear item 5.

Hello deck safety nets wasted, broken.

Attach IMO number to exterior of hull IAW SOLAS.

Conduct hydrostatic testing of portable / fixed DOT cylinders at an approved facility.

Rudders have severe pitting/wastage. Drain plugs completely disintegrated. Informed by operator that CAPAC system was not operating properly.

Repair fore and aft stern tube bearings, seal boxes and seals to both shafts. Repair damaged fiberglass wrap on the stbd shaft and conduct spark testing both shafts. Conduct NDT at the flange end of both shafts.

Gas free and ready the number 3P/S, 6P/S, 7, 8, skeg void and bow thruster compt. for inspection.

Ready for inspection all starting and control air receivers.

Repair damaged electrical calbe to the #1 SSDG. Cable must be approved marine grade, in kind.

Crop and renew the wasted H2O piping in the engineroom.

General housekeeping. Remove rags from electrical appliances and engines, and cardboard boxes from behind the SSDG's. This is a fire hazard.

Close the 2" penetration btwn the lower aft E/R bulkhead and shaft alley.

Prove proper operation of E/R watertight doors.

Ready sea valves for inspection, prior to undocking.

Crop and renew watsted fire main piping in E/R. Conduct 1.5 MAWP hydro testing when repaired.

Properly support all E/R electrical cable runs.

Properly service vessels liferafts prior to 17 September 1995.

Submit for approval plans to modify the incinerator room fixed fire ext. sys. to correct the problem of free communication between incinerator room and boiler room so that if CO2 is discharged it is contained in the incinerator room.

Submit for CG approval plans detailing the installation of an automatic shutdown device for the emergency diesel generator upon release of the fixed CO2 extinguishing system. System shall be installed after CG approvalin accordance with approved plans.
