Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel owner could not produce log book for monthly EPIRB test.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 2 1/2 gallon buckets. Buckets on board were 5 gallons.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:Charts of large enough scale to make safe navigation possible. Vessel operator could not provide information when electronic charts were last updated.

Gauges to indicate engine revolutions per minute (RPM) The gauges must be readily visible at the operating station. STBD engine RPM is not working properly.

When a marine employer determines that a casualty or incident is, or is likely to become, an SMI, the marine employer must ensure that the following alcohol and drug testing is conducted:Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the SMI.

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Ventilation louvers to engine room are being blocked from closing with wire.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include— (1) Closing each valve leading to an overboard discharge and removing the handle; (2) Padlocking each valve leading to an overboard discharge in the closed position; or (3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Sewage holding tank overboard discharge found closed but without locking device. 33 CFR 159.7(c)

For vessels other than those covered in paragraphs (b) and (c) of this section, the name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. Vessel name was not marked on the port or starboard side.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Vessel is missing original COD.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Master did not provide a marine operator permit. 47 CFR80.159(e)(1)

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:Date of the drill and training; and General description of the drill scenario and training topics. Documentation of drills and training was not available for review.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspections of fire extinguishers not completed and logged.

Glass or other glazing material used in windows at the operating station must have a light transmission of not less than 70 percent according to Test 2 of ANSI Z 26.1 (incorporated by reference, see 46 CFR 175.600) and must comply with Test 15 of ANSI Z 26.1 for Class I Optical Deviation. Port side window of operating station sun damaged and highly fractured. 46 CFR 177.1030(b)

Watertight hatches must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT HATCH—KEEP CLOSED”. All watertight hatches were found with no labels

Stations in the maritime service must be licensed by the FCC either individually or by fleet. FCC Station license is from previous owner is no longer valid.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles were constructed of plastic.

The discharge ends of fuel tank vent pipes must terminate on the hull exterior as high above the waterline as practicable and remote from any hull openings, or they must terminate in U-bends as high above the weather deck as practicable and as far as practicable from openings into any enclosed spaces. Vent pipes terminating on the hull exterior must be installed or equipped to prevent the accidental contamination of the fuel by water under normal operating conditions. Fuel tank vent is arranged to discharge to the interior of the vessel over hatches. Re-route vents to discharge to the exterior of the hull.

Except on a vessel limited to daytime operations, the compass must be illuminated. Vessel compass is not illuminated

A hatch cover must be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Lanyard securing port engine room hatch to vessel broken.

Each small battery installation must be protected from falling objects. Battery cover for port engine is not capable of staying on. Make modifications to cover to account for battery wires and close properly.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Starboard engine lubricating oil pressure gauge not working.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Markings for life jacket stowage worn and unreadable.

A hatch cover must have securing devices. Port/Stbd engine room hatch to vessel are ceased and unable to be secured.

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in § 176.500(b)(1) [annual inspection within 3 months before or after each anniversary date] and your Certificate of Inspection must be endorsed. Vessel did not conduct annual inspection for 2023. 46 CFR 176.502

Crew training shall be logged or otherwise documented for review by the COast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. No documented drills or EPIRB testing documented. Document at least one of each drill/training/testing w/i one month. 46 CFR 185.420(d)

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in 176.500(b)(1) [annual inspection within 3 months before or after each anniversary date] and your Certificate of Inspection must be endorsed. Vessel is outside of three month window for Inspection and is not permitted to operate until annual inspection has been completed. 46CFR176.502

Each life jacket must be marked with Type I retro reflective material approved in 46 CFR 164.018. Replace all retro reflective tape from life jackets and life ring. 46 CFR 185.604 (h)

The stowage locations of the personal flotation devices carried in addition to life jackets under §180.72, must be separate from the life jackets, and such as not to be easily confused with that of the life jackets. Mark each life jacket storage container with the type and quantity of stored life jackets. Ensure child are separated from adult. 46 CFR 180.78 (5)(b)

Six hand red flare distress signals approved in accordance with §160.021 and Six hand orange smoke distress signals approved in accordance with §160.037. Ensure the vessel has either 6 red flares and 6 orange smoke or a total of 12 red flares. 46 CFR 180.68 (a)(b)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Replace the current EPIRB hydrostatic release unit. 46 CFR 185.740 (2)(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date. Ensure current charts, light list, navigation rules, tide tables are on board at all times. 46 CFR 184.420

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Ensure first aid on board contains required items and instructions IAW 46 CFR 160.041. 46 CFR 184.710

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). Ensure Port Side bilge pump is fully operational. 46 CFR 182.520 (a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Ensure Starboard side high water alarm is fully operational. 46 CFR 182.530 (a)

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Make penetrations from both forward compartments going aft watertight. 46 CFR 179.320 (c)

A hatch exposed to the weather must be watertight. Ensure all hatches are made watertight to include replacing gaskets when necessary. 46 CFR 179.360 (a)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel is past due for drydock inspection. 46CFR176.600(c)(1)

Make repairs to identified areas on hull.

Demonstrate means to make all ahtches watertight. 02 forward, 01 port aft.

Replace expired hydrostatic release on EPIRB prior to operating beyond 3 miles from shore.

Properly attach float-free link to life float painter and vessel prior to carrying passengers beyond 3 miles from shore when carrying an approved EPIRB.

Bond metallic sections of port and starboard fuel fill and vent lines to prevent static build up.

Install metal supports on wiring and cables throughout vessel at intervals no greater than 24" or in accordance with 14.h of ABYC E-8.

Secure Bomar watertight hatches to hatch frames with captive chains prior.
