The pre-engineered (Fireboy) fire suppression system for the engine room did not shut down the engine when a simulation test was conducted by pulling a solenoid wire. 46CFR181.420(a)(3)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. The engine room has two approx 6" diameter plastic tubes running into it used as cable conduit. The ends of the tubes are not gas tight. 46CFR176.830

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. VHF radio did not have the vessels MMSI number programmed in it.
