Vessel expierenced an equipment failure between the engine order telegraph and the main engine rpm output while docking. 74SOLAS(20)I/II(a)

Resolution: Chief Engineer replaced start air valve for cylinder one on the main diesel engine. DNV Class surveyor witnessed start/stop of main diesel engine satisfactorily 12 times and submitted class report.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed tape over electrical breaker in engine control room switchboard. When electrican removed tape, it exposed open contacts with live wires.
