Except as provided in paragraph (a)(3) of this section, no person may operate an oceangoing ship of 400 GT and above but less than 10,000 GT, excluding a ship that carries ballast water in its fuel oil tanks, unless it is fitted with approved 15 ppm oil-filtering equipment for the processing of oily mixtures from bilges or fuel oil tank ballast. Vessel could not prove proper operation of oily water separator due to faulty regulator. Vessel to prove operation to satisfaction of marine inspector prior to using oily water separator. 33 CFR 155.360(a)

Machinery driving forced-draft and induced-draft fans, fuel-oil transfer pumps, fuel-oil unit and service pumps, and similar fuel-oil pumps must be fitted with remote controls from a readily accessible position outside the space concerned so that the fans of pumps may be stopped in case of a fire in the compartment in which they are located. The controls must be suitably protected against accidental operation and agaist tampering and must be suitably marked. Vessel's remote stops at the manifold are inoperable. Prove operation to satisfaction of marine inspector. 46 CFR 58.01-25

No person may operate an oceangoing ship of 400 GT and above but less than 10,000 GT, excluding a ship that carries ballast water in its fuel oil tanks, unless it is fitted with approved 15 parts per million oily-water separator (OWS) equipment for the processing of oily mixtures from bilges or fuel oil tank ballast. OWS polishing pack bypass is not in compliance with the manufacture's intended design for processing oily water. Prior to usage of OWS, ensure compliance with manufacture's design to the satisfaction of the attending marine inspector. 33 CFR 155.360(a)(1)

Except as noted in paragraph (c) of this section, all enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. Preventative maintenance is needed for weather deck ventilation closures, goose necks, and wingnuts to ensure they are greased and exercised for proper closure. 46 CFR 92.15-10(a)

Vessels required by 46 CFR 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: At least one light in each space where a person may be maintaining…or transiting, such as public spaces, work spaces, etc. Prove operation of emergency light in forward bosun locker.

Vessels required by 46 CFR 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: Provide and prove operation of emergency lighting in passageways to allow passengers and crew to find their way to open decks and to survival craft, muster stations, and embarkation stations with all watertight doors and fire doors closed.

The purpose of this part is to ensure a dependable, independent, and dedicated emergency power source with sufficient capacity to supply those services that are necessary for the safety of the passengers, crew and other persons in an emergency. Replace frayed battery cable for Emergency Radio Batteries with new to as original.

Each fireman's outfit must consist of one self-contained breathing apparatus, one lifeline with a belt or a suitable harness, one flashlight, one flame safety lamp, one rigid helmet, boots and gloves, protective clothing, and one fire ax. In lieu of the flame safety lamp, vessels may carry an oxygen depletion meter which is listed by a Coast Guard recognized independent laboratory as intrinsically safe. The firefighting team’s radio used for communication are not intrinsically safe.

Fire control plans showing for each deck the control stations, the various fire sections enclosed by fire resisting bulkheads, the arrangement of the alarm and extinguishing systems, the fire extinguishers, means of access to different compartments and decks and the ventilation system including location of ventilation shutdowns, positions of dampers and the numbers identifying each system. The fire control plan did not list the locations of all the smoke and heat detectors.

Each vessel much carry lifebuoys approved under approval series 160.150. Markings must be marked in block capital letters with name of vessel. Lifebuoy are fitted with non-kinking line for self-igniting light and retroreflective material under approval series 164.018. Found all lifebuoys with markings of vessel name not properly affixed, self-igniting light not properly arranged to lifebuoy, and retroreflective tape no longer serviceable.

Lifesaving appliances, instructions to passengers, and stowage locations must be marked in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Replace faded placards for port and starboard life rafts with new.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. In addition, the manufacturer’s instructions for onboard maintenance of lifesaving appliances must be onboard the vessel. The following must be provided for each appliance. See subparagraphs (i) thru (vii): (i) Checklists for use when carrying out the inspections required under paragraph (e) of this section, (ii) maintenance and repair instructions,…(vii). No proof of periodic testing of immersion suits and closures on board. Conduct tests and inspections of immersions suits and submit records to USCG for review. See USCG NVIC 1-08, guidelines for testing and shipboard inspection of immersion suits.

Each circuit directory must (ii) A description of the load of each circuit; and (iii) The rating or setting of the overcurrent protective device for each circuit. Multiple circuits marked “spare” were in the “On” position. Verify rating or setting of the overcurrent protective device for each circuit, identify/re-label “spares” w/equip. in use, and update electrical cardholder in Electrical Panels P425 and P235.

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. It was reported by vessel's master on 11DEC2022 that the starboard main engine is leaking oil from the expansion covers and oil level dipstick tube. Per master, vessel is returning to Guam with an arrival date of 14DEC2022. Identify cause of leak and effect permanent repairs/tests to the satisfaction of the attending Class Surveyor. Submit copy of Class report attesting to repairs and tests completed to USCG FM/Sector Guam prior to vessel's departure from Guam [name withheld].

Each cargo vessel must carry at least one rescue boat. Each rescue boat must be approved under approval series 160.156. SEAWARD EXPLORER not carrying a USCG approved rescue boat. Per 46 CFR 199.640 (table, Column g) when in coastwise service, an alternative to the rescue boat requirement in § 199.262(a), vessels may carry a motor-propelled workboat or a launch that meets all the embarkation, launching, and recovery arrangement requirements in § 199.160 (b) through (f).

Vessel must undergo two ISE's within five years as per the table in 46 CFR 91.40-3a. The 04 voids, two on each side of vessel, remain to be inspected by a Coast Guard Marine Inspector. Must be completed within 90 days.

Submit Fire Control Plan for approval.
