The Officer in Charge, Marine Inspection must be notified before repairs, alterations or modifications are made, that affect the safety of the vessel. Vessel crew discovered via fire detection system and visually, smoke in the bow thruster room. The smoke was determined to be coming from from a transformer due to a bad over-current protection device (circuit breaker). Repairs to be made to the satisfaction of the attending Coast Guard marine inspector.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made. On 08JAN25 a portion of the above main deck slop tank discharge line broke causing an oil spill. Repairs shall be made to the satisfaction of the attending Coast Guard marine inspector. Submit repair proposal to this office prior to making repairs.

At each COI inspection, the inspector will determine that all fire extinguishing equipment is in suitable condition and conduct any necessary tests to determine the condition of the equipment. During test of the engine room fixed CO2 system, the system failed to discharge CO2 into the engine room. Make effective repairs and arrange for system testing to be witnessed by a Coast Guard Marine Inspector.

Resolution: The vessel's Captain competently maneuvered the ship(from inside port side rear bridge conning station) and the ship's crew were able to retrieve the MOB within approximately 10 minutes from the start of the drill utilizing a portable recovery platform.
