If, during an internal structural examination, cargo tank internal examination, or underwater survey, damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. The hatch covers' knife edge is deteriorated on #2 Port recovery oil and is unable to make a gas tight seal. The number #2 port oil recovery tank is limited to carrying grade D and E cargoes until satisfactory repairs are made.

The activation of any detector or manual pull station must cause an audible and visual fire detection alarm signal at the control panel. If the alarm signal has not been acknowledged within 2 minutes, an audible fire alarm must be automatically sounded throughout the crew accommodations and service spaces, control stations, and manned machinery spaces. Vessels installed fire detection system is not sounding within the required time frame when not acknowledged at the control station.

Resolution: Vessel fixed the fire alarm before departing the vessel.
The door is designed to withstand a head of water equivalent to the depth of the sill of the door to the bulk head deck or (3 meters) 10FT whichever is greater. 03 WTD's in enginering space failed chalk test. Doors were adjusted and tested satisfactoritly.

Compliance with the standards established by the American Bureau of Shipping, will be considered as satisfactory evidence of the structural efficiency of the vessel. While conducting training vessels small boat impacted the aft peak mud ballast tank on the stern. Causing a ¾ inch diameter hole three feet above the water line. Vessel will make temp repairs to puncture and submit permanent repair proposal to USCG for approval. Repairs of stern must be approved to the satisfaction of the attending marine inspector.
