On vessels subject to STCW, the individual meeting the requirement of this section must also hold an STCW endorsement as Master with the appropriate tonnage for the vessel which he or she is operating, except as noted in § 15.105(g) for vessels on domestic near-coastal voyages. Master isn’t endorsed for STCW on the MMC. 46 CFR 15.805(b)

A Designated Duty Engineer (DDE) endorsement authorizes service as Chief or Assistant Engineer on vessels of less than 500 GT in the following manners: When serving on a vessel to which STCW applies, the appropriate STCW endorsement must also be held. The Chief Engineer did not have the STCW Endorsement of III/2 on their MMC.

On vessels subject to STCW, the individual meeting the requirement of this section must also hold an STCW endorsement as Rating Forming Part of a Navigational Watch (RFPNW) or Able Seafarer-Deck. The OS did not have the STCW Endorsement of II/4 (rating forming part of navigation watch) on their MMC.

After January 1, 2017, all persons employed or engaged onboard vessels to which STCW applies must hold a medical certificate valid for 2 years unless the mariner is under the age of 18, in which case the maximum period of validity will be 1 year. 3 crewmembers had lapsed STCW Medical certificates.

Cargo vessels less than 85 meters (278 feet) in length, with As an alternative to the survival craft requirements of § 199.261(b), (c), or (d), vessels may carry one or more liferafts with an aggregate capacity sufficient to accommodate the total number of persons on board. The liferafts must be supplemented with additional liferafts to bring the total capacity of the liferafts available on each side of the vessel to at least 100 percent of the total number of persons on board. Liferaft capacity does not meet 100% on both sides of the vessel. Vessel is restricted to carrying 25 persons.

Each launching station, or each two adjacent launching stations, must have an embarkation ladder approved under approval series 160.117 or 160.017. There were no way to attach embarkation ladders at the embarkation station for the liferafts as designated on fire safety plan.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. There is an unapproved electrical cord connected to electrical panel “LP-207”.

Failure of an automatic control, remote control, or alarm system must be immediately alarmed in the machinery spaces and at the ECC, if provided. Low start air pressure alarm, pressure sensor is not operational.

The owner, builder, operator, or agent of a vessel of special construction or purpose who believes the vessel cannot fully comply with the Inland Rules light, shape, or sound signal provisions without interference with its special function may apply for a determination that alternative compliance is justified. Vessel has an offcenter mast for it's stern lights which requires a certificate of alternate compliance.

The 1966 international load line certificate certificate shall be the same as set forth in this subpart. Hailing port on load line and tonnage certificates does not match the COD.

The garbage record book must be prepared at the time of the operation and certified as correct by the master or person in charge of the ship. Garbage Record Book entries not signed by master.

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. Incorrect vessel color, persons on board, and lifesaving equipment on EPIRB registration 47 CFR 80.1061 (f)

General arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads, together with particulars of the fire alarms, detecting systems, the sprinkler installation (if any), the fire extinguishing appliances, means of access to different compartments, decks, etc. Fire safety plan is inaccurate to current vessel arrangement for lifesaving appliances.

Licensees of radio stations are required to have current station documents. FCC documentation has the incorrect hailing port. 47 CFR 80.401

Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. A previous deficiency was issued for hydraulic hosing on the A-Frame exceeding 30 inches. Vessel submitted new plans with 45" hoses, MSC approved these new plans, accepting 45". Upon measuring each hose, it was found they are all above the accepted 45". Resubmit plans with current or new arrangement, and install satisfactorily in accordance with approved plans.

Vessels over 180 feet in length which are not passenger vessels, and which operate on international voyages or in ocean, coastwise, or Great Lakes service, must be provided with a direct emergency bilge suction from any pump in the machinery space, except that a required bilge. Vessel does not have emergency bilge suction installed. An emergency bilge suction system must be installed to the satisfaction of the OCMI.

Each Class 1 door must have a quick action closing device operative from both sides of the door. Hinged watertight doors are of the individually dogged design. Quick acting watertight doors must be installed to the satisfaction of the OCM at NEXT DRYDOCK.

- For inspection procedures of marine engineering equipment and systems, see 46 CFR Subchapter F. -The requirements of this section are applicable to those hydraulic and pneumatic systems listed in §58.30–1. - Hose and fittings shall meet the requirements of subpart 56.60 of Subchapter F. - Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. After CG Marine Safety Center's review of submitted drawings and email correspondence MSC replied with a requirement (MSC ltr E1-2500987 dtd March 26, 2025) to install hoses that are less than 30 inches in length. Prove to the satisfaction of the cognizant OCMI that the installed hoses for the fluid power control system used for the "A" frame are not longer than 30 inches.

For vessels required to meet the damage stability standards of subpart H of this chapter, when Class 1 doors are installed below a deck the molded line of which at its lowest point at side is less than 7 feet (2.14 meters) above the deepest load line, an indicator light for each door which warns when the door is open must be installed on the bridge. Currently installed class 1 watertight doors below the main deck do not have indicators installed. Install watertight door indicators on subdivision watertight doors at or before the scheduled 2025 drydock. Installation and testing must be performed to the satisfaction of the attending Coast Guard Marine Inspector.

At each inspection for certification, the marine inspector shall conduct inspections of main propulsion and auxilliary machinery as he feels necessary to check safe operation. While performing underway trials for drills, bow thruster was not operating properly. Repair bow thruster, conduct underway testing, and perform underway drills prior to issuance of COI.

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. During routine ballast operation post COI and hull inspection with ABS, noticed tank levels between the forepeak and #1 starboard ballast tank equalizing unusually. Shifted all forepeak ballast into #1 starboard and identified a small leak in the bulkhead between #1 starboard and the forepeak ballast tank. Provide repair proposal to MSU Lake Charles for approval and make permanent repairs to forepeak/#1 starboard ballast bulkhead.

Except as provided in 46 CFR 15.725, no vessel may be navigated unless it has in its service and onboard the crew complement required by the Certificate of Inspection (COI). The vessel's COI allows for an able seaman (AB) to be replaced by a specially trained ordinary seaman (OS) who has a letter of qualification as provided in NVIC 3-83. At the time of inspection, and up to this date, a letter of qualification could not be provided to the marine inspector for the OS assigned to the vessel who is replacing an AB. Provide letter of qualification for assigned OS or make appropriate crew changes to meet minimum safe manning requirements as required by the COI. Prior to the vessel getting underway a complete crew list must be submitted to this office for review. The crew list must include each mariner's name and MMC reference number.

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the seaworthiness of the vessel. While fast to the dock and under ideal waterway and weather conditions, attending marine inspectors (MI) observed an inadequate man-overboard recovery operation using a water filled "Oscar". Upon completion of the drill, MIs discussed the drill with the crew and found the designated AB running the rescue boat had not had recent hands on training/operation of the rescue boat and the OS had only on one occasion handled the recovery cradle. Both situations were presenting a safety concern for the drill. MIs recommended responsible crewmembers (based on station bill requirements) be afforded training opportunities prior to MIs returning to witness another drill. Perform satisfactory man overboard recovery drill to the satisfaction of attending MI.

At each inspection for certification and periodic inspection the Marine Inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as he feels necessary to check safe operation. Starboard main engine inboard turbo charger is leaking oil from turbine side and the Port main engine outboard turbo charger is leaking oil from turbine side. Repair both main diesel engines to the satisfaction of the attending Marine Inspector. 46 CFR 61.20-3(a)

In sleeping accommodations each berth must have a light. Marine Inspector identified 07 berths with inoperable bed lighting. Crew must go through all berthing, identify, and repair lighting to the satisfaction of the attending Marine Inspector. 46 CFR 92.20-45

Each vessel must carry lifebuoys approved under approval series 160.050/150 with retro-reflective material. Marine Inspector identified cracked and severely weathered retro-reflective material on 02 ring buoys on each bridge wing. Crew must go through all ring buoys and repair retro-reflective material to the satisfaction of the attending Marine Inspector. 46 CFR 199.70(a)

Each hand portable fire extinguisher shall be marked with a number and the location where stowed shall be marked with a corresponding number at least 1/2 inch. Identified 04 portable extinguishers without a corresponding number to their stowed location. Crew must go through all portable fire extinguishers and number them according to their stowage location number to the satisfaction of the attending Marine Inspector. 46 CFR 97.37-23

No person may operate an ocean going ship of 400GT and above unless it is fitted with an approved 15 PPM oily water separating equipment. Vessel 15PPM content meter is inoperable, the internal battery has exceeded its life and is not serviceable.

Operational tests and checks of all safety and limit controls must be conducted. Vessels thermal fluid heater could not be tested due to a malfunction in the electronics.

Prove proper operation of emergency light. 4 Lights would not operate upon loss of ships power, 1) GALLEY 2) WORKSHOP 3) STEERING GEAR ROOM 4) ENGINEROOM PORT ESCAPE.

Life rafts must be in a float free arrangement. Port and stbd life rafts were tied to a cleat.

Repair or replace recovered oil tanks PV valves. PV valve currently sticks open position when testing manually.

Master could not locate hazardous locations plan.

The time delay device must be pneumatically operated and have an accuracy of -0/+20 percent of the rated time delay period throughout the operating temperature range and range of delay settings. The time delay failed to meet the time requirement for the following spaces: oil heater room and oily water separator room. Crew demonstrated the ability to egress the protected spaces meeting the current time delay. Master must replace signs stating the current time delay and current time delay and conduct egress training with crew. 46 CFR 95.16-45
