Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 03 Marlin Type 1 life jackets installed on vessel (Model 320RT for Adults and Model 321RT for Child size) have a malfunctioning spring-loaded tab on the snap hook of the safety strap. The spring-loaded tabs fail to spring back,(return to closed position) after depressing to secure snap hook in place. Renewal required for all affected Lot #'s. Vessel has 157 serviceable adult life jackets on board, therefore passenger count shall be reduced to 147 passengers + 03 required crew and 7 other persons in crew, IAW vessel COI. Provide 03 additional, serviceable Type 1 life jackets. 46 CFR 176.808(b)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. Starboard side fixed bilge system non-operational at forward compartment due to stuck check valve at suction foot. New bronze check valve installed and operationally tested during inspection to the satisfaction of the attending Marine Inspector. No further action required. 46CFR182.25-1(a) "old T"

Each portable fire extinguisher must be of the approved type. 03 portable fire extinguisher showing rust in body of extinguisher. Replace 03 portable fire extinguishers. 46 CFR 181.500

Vessel over 26ft must have visual and audible indicator for high water level at the operating station for each machinery space. Starboard Laz bilge high water audible and visual indicator not operational. Provide visual and audible indicator at operating station for Stbd laz. 46 CFR 182.530

Each life jacket must have retroreflective material. Some jackets had worn retroreflective materiel. Raplace work retro tape and provide new tape. 46 CFR 185.604(h)

Equipment installed must be suitable for the vessel and its intended purpose. Port side steering hydraulic hose rubber outer covering wasted. Replace wasted hose with approved J-1942 hose. 46 CFR 182.100

Machinery installed must be suitable for the vessel and its intended purpose. Both port and starboard side engines have coolant leaks. Make repairs to engine to secure coolant leaks. 46 CFR 182.100

Equipment installed must be suitable for the vessel and its intended purpose. Windlass not operational. Provide operational windlass. Complete at next drydock. 46 CFR 182.100

Vessel does not currently meet the requirements of Work Instruction 31. Mandated crew posted on COI may not enter the water or act as lifeguard until policy and procedure provided. Provide written procedure. Work Instruction 31

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel.

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Provide/Update Company Drug Policy to reflect current changes to DAPI program (i.e., Provide DER information, EAP Information, Hot-line, etc.).

Starboard side diesel fuel vent missing 30 x 30 mesh screen. Provide 30 x 30 mesh screen.

Starboard side engine room raw water pipe union leaking at through hull. Make repair to piping.

Port side engine room generator exhaust has temporary repair where leak is located. Make permanent repairs to gen exhaust.

Port side engine room main engine exhaust has temporary repair where leak is located. Make permanent repairs to main engine exhaust.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances including the operational test of the means provided for pumping bilges. While conducting a seatrial of the vessel CG inspectors identified a leaking hose connection between the electric bilge pump and the overboard valve in the port engine compartment. The installed hose in incorrect for the application and is causing leaks at both connection points, however the vessel has an installed fixed bilge pump within the space for dewatering. The vessel is required to keep the overboard valve secured to prevent water ingress until the correct hose has been installed and the leak is secured as approved by an attending marine inspector.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. Bilge pumps were found unsecured. Secure all bilge pumps to the deck.46CFR182.25-1 “Old T”

Provide EPIRB NOAA registration documentation.

Provide 02 adult type 1 life jackets.

Fire main leaking at hose connection on both port and starboard side. Make repairs to ensure no leaks from fire main.

Rudder post flange wasted where bolts secure to tighten packing. Renew port and starboard rudder post flange to new condition. Correct at the next drydock.

Port bulkhead from engine-room to laz has penetration. Make bulkhead watertight. Correct at the next drydock.

Port side storage space aft bulkhead near outboard hull has penetration. Make bulkhead watertight. COrrect at the next drydock.

Port sewage space and fore peak bilge line holed. Make piping operational with no holes.

Emergency fuel shutoff valves missing for port and starboard side. Provide fuel shutoff valves.

1 15 person life float was unsatisfactory in accordance with 46 CFR 176.808. Renew in accordance with 46 CFR 180.200.

Radio Safety Certificate was expired. Provide current certificate in accordance with 46 CFR 184.502.

17 Adult lifejackets failed inspection.
