All detectors shall be of a type such that they can be tested for correct operation and restored to normal surveillance without the renewal of any component. Vessel's fire control panel has two faults. When spot checked, smoke detectors on the 06 deck did not function. Rectify to the satisfaction of RO. 74 SOLAS(97) II-2/13.1.13

The owner of an applicable vessel “shall provide clear and conspicuous signs on board the vessel notifying the crew of the presence of video and audio surveillance equipment... the owner of an applicable vessel or employer of a seafarer “shall provide training for all individuals employed by the owner or employer for the purpose of responding to incidents of sexual assault or sexual harassment[.]” This training shall ensure individuals (A) “retain audio and visual records and other evidence in an objective manner. Signage of surveillance and crew training is yet to be implemented on recently installed. Install signage and provide training to the satisfaction of attending Coast Guard Marine Inspector. 46 USC 4901 CVC Policy ltr 23-05 MSIB 13-23 CH3

Vessel Captain reported Port and Starboard Rudder Horns Flooded with salt water. Normally these spaces are empty and air tight. Seals/rings have possibly failed. Report discrepancy to RO.

Vessel is restricted to the carriage of 69 personnel until the lifeboat gripes/pendants are replaced to the satisfaction of the attending OEM technician. Currently the fore and aft gripes/pendants have oversized D-rings that do not allow for the unimpeded launching of the required survival craft for the full crew compliment shown on the vessel's COI. Vessel is restricted from the carriage of cargo until this deficiency has been cleared.

Vessel is currently outfitted with a non-type approved switch over/central processing unit as part of the gyrocompass' heading management and monitoring system. This switch over/central processing unit is identified as a BBG-DRP-422 Digital Repeater Panel. As per SOLAS V, Regulation 18, all navigation systems and equipment must be type approved by the Administration.

Fire caulk an dinsulation is missing on fwd bulkhead of ECR at longitudinals in overhead and in cable run. Bulkhead is required to be an A-60 boundary. Rating of bulkhead must be restored to A-60. SOLAS (84) II-2/Reg 44.1

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have a mechanical means of closing each drain and cupper in the container. Two contaiments were missing plugs. 33 CFR 155.310(a)(3)

If a survival craft is receovered by any means using power, safety devices must be provided that automatically cut off power before the davit arms or falls reach the stops. (In lifeboat #4, the aftermost limit switch- for the after fall was inoperable.) SOLAS (2004) III/Reg 20.1

A life jacket shall be provided for every person on board the ship. Three lifejackets were unserviceable. SOLAS (2014) III/Reg 7.2.1

Cable MCTs found wasted along the tank top deck on the port side near pump room.

Vent screen to #2 F/O service tank was clogged with dust and feathers, needs to be cleaned or replaced.

2 life rings found in need of repair or replacement. 1 with damaged coating allowing waterlogging of foam and the second needed new retro tape.

Containment boxes on foc's'le and stbd 01 deck needed mechanical plugs that fit properly.

Fire/smoke detectors found covered or covers hanging via sensor wires.

The piping systems shall be of a construction adequate for the service in which they are intended and shall be so installed. Pressurized air line piping in incinerator room for incinerator blower had air leak. Permanently repair air line piping.

Leaking lube oil and fuel oil valves found at #1 starboard forward manifold.

Soft patch found on oily waste piping in JP5 pump room. Crop/renew wasted piping. Pressure test to satisfaction of attending MI/RO.

Under fill connections and vents there must be a fixed container or enclosed deck area with mechanical means of closing. Small discharge containment found wasted/holed at starboard cargo manifold just forward of shore power connection. Crop/renew wasted areas.

Multiple small discharge containment wells for vents found without mechanical drain plugs. Provide mechanical plugs for all small discharge containments throughout vessel. 33CFR 155.310 & .320 2003 ABS Supp III-12

The master shall not be constrained from executing a decision which is necessary to maintain the safety/security of the ship. Properly install lock on emergency escape from steering gear flat to ensure emergency egress and security are maintained. SOLAS 74am (14 Cons Ed)XI-2 Reg 8.1
