The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. 1) Repair broken bottom panel on self-closing Class "A-60” door, No. 1-05-2, to the satisfaction of Class. 2) Replace missing firestops in Probe Shop overhead and forward bulkhead with cable penetrations to the satisfaction of Class.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The 2JV and 2JZ sound powered phone circuits were not operational in motor room #1.

The general emergency alarm system shall be audible throughout all the accommodation and normal crew working spaces. Vessel’s General Alarms (GAs) were tested inoperable in the following locations listed below. Repair and prove operation to the satisfaction of Class: 1) Inside Hospital Space – both GAs inside the space. 2) 01 Level , stbd side bulkhead in way of bulkhead marked with frame number 69. 3) Above water dispenser machine in [name withheld]. 4) Next to WTD 01-20-01 Deck Dept. Sorbent Locker. 5) Stbd side exterior bulkhead forward of Probe Shop on main deck. 6) GA bell and light in JP5 cargo pump room. 7) GA at Motor Room No. 2’s entrance.

Each circuit directory must (ii) A description of the load of each circuit; and (iii) The rating or setting of the overcurrent protective device for each circuit. The breakers in the following electrical panels were not labled with the name of equipment in use: 1) Crew’s Laundry Power Panel #03-99-2: Breaker Nos. 6, 7, 9 and 10. 2) Crew’s Laundry Power Panel #02-96-2: Breaker Nos. 3, 4, 7, and 8,

Tanks of a tankship must be equipped with an intrinsically safe high-level alarm and tank overfill alarm which is independent of the cargo gauging system that activates a visible and audible alarm where cargo transfer is controlled and in the cargo deck area. Cargo Tanks #9P and #8S high/overfill alarms were tested and found inoperable. Prior to cargo to loading cargo, repair and prove operation of the cargo tank level alarms.

The electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency; For a period of 18 h, emergency lighting: In the machinery spaces and main generating stations including their control positions; in all control stations, machinery control rooms, and at each main and emergency switchboard. Several emergency lights were extinguished throughout the machinery space.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. The flex hose from the hot start to the emergency generator block was in need of servicing.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. MI discovered actuation hoses for cylinders 41483 and 61521 in Halon Room No. 1-60-1 were not reconnected after being serviced. Reconnect actuation hoses in accordance with manufacturer’s specifications for immediate use.

Each completed operation shall be signed for and dated by the officer or officers in Charge. Missing entries in vessel’s Oil Cargo Record Book Part II for “(O) Additional operational procedures and general remarks” of tests completed by the vessel crew on the ODME in manual mode. Entries must be made in the ship’s Oil Cargo Record Book Part II after completion of each test and signed/dated by the officer.

A lifejacket complying with the requirements of paragraph 2.2.1 or 2.2.2 of the Code shall be provided for every person on board the ship and, in addition: if the adult lifejackets provide are not designed to fit persons weighing up to 140kg and with a chest girth of up to 1,750 mm, a sufficient number of suitable accessories shall be available on board to allow them to be secured to such persons. The following persons require a suitably sized lifejacket to be issued to them: [name withheld], [name withheld], OS – [name withheld], AB-[name withheld] and Communications Officer.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Replace deteriorated and sun damaged grab lines on ring buoy at STA# 21 01-110 and ring buoy at STBD rescue boat with new to as original.

A training manual complying with the requirements of paragraph 3 shall be provided in each crew mess and recreation room or in each crew cabin. Replace missing SOLAS Training manual in crew mess.

Under MARPOL Annex V, ships of 400 gross tonnage and above, or those carrying 15 or more persons engaged in international voyages, must maintain a Garbage Record Book. The book must be kept for a period of two years after the last entry. Each entry, detailing garbage disposal or incineration, must be signed by the officer in charge of the operation and by the master of the ship. Several completed pages in vessel’s Garbage Record Book were not countersigned by the master.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. The reach rod for the fuel oil settling tank drain, FO-VL-052 was disconnected and not operational.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea. 1) Replace missing signs for secondary means of escape in vessel’s cargo pump room to the satisfaction of Class. Note: IMO Resolution A.11116(30) while not explicitly mandatory for pre-2019 ships, can be used as a guide for developing escape route plans and signage; IMO Resolution IMO Resolution A.654(16) applies to vessels constructed before 2000. 2) Replace unapproved straight arrows for egress/exit on the bulkheads in the Motor Rooms with IMO approved directional arrows that are photo-luminescent, self-adhesive vinyl.

Before being assigned to shipboard duties, all persons employed on a seagoing ship, other than passengers, shall receive familiarization training in personal survival techniques or receive sufficient information and instruction, taking account of the guidance given in part B. Crewmember in SCO position (Supply Officer) did not have required STCW endorsements or proof of training.

The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. If the signals have not received attention within 2 min, an audible alarm shall be automatically sounded throughout the crew accommodation and service spaces, control stations and machinery spaces of Category A. Vessel had a new fire detection system installed during the previous drydock period. MI observed the testing of manually operated call points and one smoke detector (Zone 16, Detector 120) that went directly to the general alarm, bypassing the 2 minute acknowledgment period.

Tanks of a tankship must be equipped with an intrinsically safe high-level alarm and tank overfill alarm which is independent of the cargo gauging system that activates a visible and audible alarm where cargo transfer is controlled and in the cargo deck area. The tank level gauging system is out of calibration providing inaccurate tank level reading for cargo tanks and ballast tanks. Ship is gas-free with empty cargo tanks but console in CCR shows innage in COT 10P/S of 2.6 feet, COT 9P of 8.3 feet, COT 9C of 10 feet, COT 4P/S of 1.4 feet, and COT 2C of 2.2 feet (discrepancy is measured in feet with one foot equaling 100 or 1000 barrels per foot depending on tank, with largest variance being ten feet or 10,000 bbls/420,000 gallons, with total discrepancy equaling 11,710 bbls/491,820 gallons). 74 SOLAS (2001) II-2/59.1.6 and 46 CFR 39.2007(a) & (d)

The company should establish procedures, plans and instructions for key shipboard operations. Bunkering manifolds on port and starboard sides of Tank Deck do not follow SMS procedures on color-coding or labeling transfer piping systems. Paint colors do not match piping systems. No labeling is present indicating the service or product for piping at either manifold. Two valves painted yellow on the port bunker manifold appear to have corrosion and are marked with spray paint.

Flammable liquid lockers shall be protected by an appropriate fire-extinguishing arrangement approved by the Administration. Cargo Storeroom, “Cargo Strm (Lube Oil) 01-27-1,” serves as storage for combustible sundries and flammable liquids in 55-gallon drums, 20-liter pails, and quart containers on the 01 Deck, starboard side in the Forward Truck Tunnel and is labeled as “Hazmat Stowage Area.” The space has no fixed fire-fighting arrangement. Per current inventory, there is 10,000+ gallons of flammable liquids being stored in the space. Similar space on port side of Forward Truck Tunnel, “FWD Cargo Strm (Flam Liq & Misc) 01-24-2,” is a smaller space and contains smaller quantities of identical flammable liquid products and is protected by an appropriate fire-extinguishing arrangement. Additionally, boxes containing pails of “Radcolube” were found leaking. Remove flammable liquids from “Cargo Strm (Lube Oil) 01-27-1” or provide mitigation procedures until a fixed fire-extinguishing arrangement is installed. 74 SOLAS (97) II-2/18.7

A minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter's communication shall be carried on board and be explosion-proof or intrinsically safe. Radios provided for fire-fighter’s communication were not utilized during the drill and comms were unable to be verified. 74 SOLAS (20) II-2/10.4

The condition of the ship shall be maintained in all respects will remain fit to proceed to sea. Sound powered phones in Cargo Motor Rooms do not function. 74 SOLAS (20) I/11(a)

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. All bilge alarms routed through the cargo control console are currently inoperable due to continued degradation of the CPU. This is currently affecting 10 bilge alarms in the cargo pump/motor rooms.

All gearing and every shaft and coupling used for transmission of power to machinery essential for the propulsion and safety of the ship or for the safety of persons on board shall be so designed and constructed that they will withstand the maximum working stresses to which they may be subjected in all service conditions. The stbd pedestal bearing overheated while underway.

74 SOLAS (2004) Chapter I, Part B Reg. 11 (a): The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea. Unable to test vessel’s steering gear system at time of USCG’s visit due to ongoing maintenance and repairs. A test of the vessel’s steering gear per Regulation 29, must be completed to the satisfaction of the attending USCG Marine Inspector prior to vessel movement.

Title 46, Code of Federal Regulations, Part 32.25-1: Vessel’s general alarms were tested inoperable. Repair and prove operation to the satisfaction of Class. General alarms: 1-35-2 (next to 5 port tank top), 01-34-1 (station 1), 01-34-2 (station 2), 01-39-1, 01-46-2, 01-56-1 (station 7), 01-82-1, 01-110-3 (striker only), 01-110-4, 03-81-1.

74 SOLAS (2009) Chapter III, Regulation 20.3: Maintenance, testing, and inspection of life-saving appliances shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring reliability of such appliances. Incorrect battery used in each floating electric water light attached to ring lifebuoy. Replace with battery as per manufacturer’s recommendation.

74 SOLAS (2014) Chapter III, Regulation 20.3; LSA Code 1.2.2.7: Maintenance, testing, and inspection of life-saving appliances shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring reliability of such appliances.Replace deteriorated reflective tape on lifebuoys and floating light at Stations 11, 12, and 22, and 2 x adult PFDs with new SOLAS Type I approved tape

74 SOLAS (2004) Chapter I, Part B Reg. 11(a): The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to sea w/out danger to the ship or persons on board. Replace/repair vent screens in Motor Room No. 2, and replace missing explosion proof light lenses in Motor Room No. 3.

74 SOLAS (2004) Chapter II-1/Part A-1, Reg. 3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. Repair exterior overhead with opening and missing hardware as to original and make weather tight on 03 Deck between lifeboats 1 and 3.

74 SOLAS (2001) Chapter II-2, Regulation 12.1.1: Any required automatic sprinkler, fire detection and fire alarm system shall be capable of immediate operation at all times and no action by the crew shall be necessary to set it in operation. Overhead fixed heat detector in Motor Room No. 3, disconnected from mounting plate and is hanging loosely by wires. Secure sensor to mounting plate to as original.

74SOLAS (04' consolidated) Ch. III, Regulation 20.11.3: Lifeboat or rescue boat on-load release gear, including free-fall lifeboat release system shall be operationally tested under a load of 1.1 times the total mass of the boat when loaded with its full complement of persons and equipment whenever the release gear is overhauled. Such overhauling and test and shall be carried out at least once every five years. Provide proof of recent five year service report for vessel’s port side rescue boat release mechanism (Henriksen) or replace with an approved SOLAS release mechanism and tests carried out upon installation.

Classs issued OSD SC3607571-A extending requirement to have permanent repairs made to vessel's starboard forward no. 1 lifeboat canopy.

Lifeboat determined to exceed placarded "A" weight by 1,110 lbs.

Have Lifeboat #1 (Survival Systems MCBO 34) re-weighed in the presence of an attending USCG Marine Inspector to re-verify the lifeboat's Net (A) and (B) weights. The load cell, dynamometer, or weight scale used shall be a traceable certified weighing instrument. This requirement is based upon findings during an earlier Cg inspection where it was revealed that the actual A and B weights were considerably more than that which is endorsed on the lifeboat's hull plate.
