The required vessel, equipment, or installation plans, as listed in this subchapter, are general in character, but include all plans which normally show the intended construction and safety features coming under the cognizance of the Coast Guard. Both Voith Drives has faulty Zero Pitch indicators that didn't set off visual alarms by the engine.

Each crew berthing area shall be equipped with information regarding vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage; and procedures and resources to report crimes, including sexual assault and sexual harassment. Accommodation Spaces (bathrooms) did not have the required information posted inside the berthing spaces. 46 USC 11101

All piping, valves, and fittings shall meet the applicable requirements of subchapter F. Port side fire main piping to fire monitor has multiple areas with temporary repairs. Prior to conducting standby service. 46 CFR 95.17-15(a), 46 CFR 50.05-10(a)

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and vetilators. Pimp room blower fan motor is inoperable and vent cover is not properly installed until new motor arrives. 46 CFR 92.15-10(a)

The aforementioned information shall be kept up to date, any alteration being recorded in the applicable medium as soon as practicable. Fire & Safety plan was not up to date based on visual inspection of plan on board vessel. 46 CFR 97.36-1(c)

At each inspection for certification, periodic inspection and at other times necessary, the inspector will determine that all fire-extingusihing requipment is in suitable condition and may require any tests necessary to determine the condition of the equipment. Portable fire extinguisher in starboard main deck locker was missing nozzle horn. 46 CFR 91.25-20(a)

Where fire detecting or extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation shall meet the requirements of this part. Fire pimp gauge and indicator light inoperable from operating station. 46 CFR 95.01-5(a)

Vessels operated solely within one hundred nautical miles of shore must be equipped with .. A single sideband radiotelephone capable of operating on all distress and safety frequencies in the medium frequency and high frequency band listed in 80.369(a) and (b), on all the ship-to-shore calling frequencies in the high frequency bands listed in 80.369(d), and on at least four of the automated mutual-assistance vessel rescue (AMVER) system HF duplex channels (this requirement may be met by the addition of such frequencies to the radiotelephone installation required by paragraph (b) of this section). Single Sideband radiotelephone was not fully operational. 47 CFR 80.880(b)(1)

Representatives from the OSV LOOP RESPONDER reported an engine room fire that orginiated on the Port M/E resulting in a loss of propulsion and damage to areas in the engine room and engine room ventilation. Vessel must affect repairs to the Port M/E and other damaged equipment to the satisfaction of the CG prior to departure.

There must be an individual holding an MMC or license endorsed as Chief Engineer or other credential authorizing service as Chief Engineer employed onboard vessels of 200 GRT and over. The Chief Engineer was limited to operations on Offshore Supply Vessels.

During preparation, launching, and recovery, each survival craft and area of water into which it is to be launched or recovered must be adequately illuminated by lighting supplied from the emergency power source. The emergency lighting did not provide illumination on the aft deck where the life-rafts would be boarded.

Hertz Gage on #2 Switchboard stuck at 61hz.

The Wire Cables that support the platform on the rescue platform is aged and splintered which will cause injury to hands.

Battery box in engine room was not secured.

Marpol V placard not current.

Oxygen acetylene tanks stored on aft deck 01 level with insufficient restraint and no barrier between bottles.

No transfer procedures or line diagram posted at fuel transfer or recovered oil controls.

Overspeed trip being serviced on starboard generator.

Batteries for SSB in crawl space under bridge were not properly installed in a battery box and secured against shifting.

Both pressure vessels exhibited evidence of pitting/corrosion in upper portions of interior of PV's. Internal inspection insufficient. Both pressure vessels to be hydrostatically tested.

Electric motor for backup lube oil pump for port voith drive being service at time of inspection. Tech report to be provided by vessel rep upon completion, installation, and test.

The STBD crane pedestal has had the mating flange removed due to corrosion. The flange was replaced and welded to the pedestal. Vessel did not call out CG for oversight. Vessel to have NDT done on welds with a CG Marine Inspector in attendance to witness testing. Prior to placing crane in service.

Due to vessel's port crane pedestal mating flange being removed and replaced the vessel must provide welder qualifications and approved welding procedures to the attending MI for review and approval prior to placing the crane back in service.

Both of the vessel's deck cranes were removed to be rebuilt and serviced. Prior to placing the cranes back into service the vessel must conduct the 60month crane testing IAW 46CFR107.260 prior to the cranes being placed back into service. Crane testing must be witnessed by an attending CG Marine Inspector.

Port generator would not trip offline from an overspeeding condition.

Repair engine room ladder rail at 01 deck.

Properly repair electrical splice and install proper junction box in pump room.

Provide 100% liferaft capacity on each side of vsl within 3 months

Provide fixed fire extinguishing systems for recovered oil tanks & pumproom in compliance with Cg MVI Policy Ltr for Oil Spill Recovery Vsls (3-92) & 46 CFR Part 95 within 3 months.

Demonstrate overspeed trips for both SSGs prior to sailing.

Demonstrate reverse power trips for both SSGs prior to sail (inop).

No record of completion of approved automation tests since 2004, engineer not using approved procedures.

Second licensed mate (in charge of nav watch) lacked GMDSS endorsement.

Provide new rescue harness for man overboard platform.

Provide dedicated auxiliary line fore line throwing gun.

Revise Automation Test Procedures and provide for CG approval.

Provide approved automation test procedures for vessel.

Install an striker plate for Stbd crane anti- to blocking device and prove proper operation of same.

Prior to operating ship's service generators in parallel - replace reverse power relay, #1 ship service generator switchboard and prove proper operation of same to attending marine inspector.

DEMONSTRATE PROPER OPERATON OF BOTH SHIP SERVICE GENERATOR OVERSPEED TRIPS TO THE SATISFACTION OF ATTENDING MARINE INSPECTOR.

INSTALL AND PROVE PROPER OPERATION OF GLOBAL MARITIME DISTRESS AND SAFETY SYSTEM IN ACCORDANCE WITH 47 CFR 80 SUBPART "W" TO THE SATISFACTION OF ATTENDING MARINE INSPECTOR.

DEMONSTRATE PROPER OPERATION OF BOTH SHIP SERVICE GENERATOR REVERSE POWER RELAYS TO THE SATISFACTION OF ATTENDING MARINE INSPECTOR.

MODIFY BALLAST MANIFOLD AND PIPING SYSTEM TO BRING INTO COMPLIANCE WITH 46CFR 56.50-50(h). APPROVED PLANS MUST ALSO BE PROVIDED.
