The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by table 1 to § 181.500(b) and other provisions of this section. Vessel was missing the 40-B:C Fire Extinguisher. Owner to obtain proper fire extinguisher for vessel and provide proof to MI.

Vessel did not have required 40BC portable fire extinguisher. Provide required and approved 40BC portable fire extinguisher.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. NFPA F.7.2.4 If the inspection of a rechargeable fire extinguisher model reveals any physical deficiencies such as damage, corrosion, low-pressure reading, missing parts, obstructed nozzle, illegible operating instructions, prior use, or broken tamper seals, such deficiencies indicate the need for immediate maintance and servicing of the fire extinguisher. Inspector found the two available portable fire extinguishers missing tamper seals. Service the two portable fire extinguishers with a qualified servicing organization and re-install tamper seals.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Fireboy was found to be past due annual service requirements. Service Fireboy with a qualified servicing organization.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Visual indicator at operating station did not light to the operation of the automatic bilge pump, but lit for manual operation as intended. Vessel must have visual indicator at the operating station when the automatic bilge pump is operating.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to the provision for a sea worthy hull. Operator reported striking a rock on the Lower Salmon River at Green Canyon Rapid. Vessel is to be hauled out at have satisfactory repairs conducted prior to the carriage of passengers or cargo.

Propulsion engine started when the Inspectors were testing the fume detector.

Fuel oil (gas) sprayed out of the fuel line to the starboard engine when the engines started.

A pre-engineered fixed gas fire extinguisher system must be installed in accordance with the manufacturer's instructions. Fireboy charge indicator indicates "recharge." Ensure Fireboy is installed in accordance with manufacturer's instructions to the satisfaction of the attending marine inspector.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space bilge. Vessel's engine room bilge alarm is inoperable. Ensure bilge alarm in engine room is operable to the satisfaction of the attending marine inspector.

Each small battery installation must be protected from falling objects. Both batteries in the engine room are missing covers. Protect batteries from falling objects to satisfaction of the attending marine inspector.

An emergency switch must be provided in the normally ungrounded main supply conductor from a battery. The battery switch onboard was inoperable. Vessel required to proper operation of battery switch to the satisfaction of the attending Marine Inspector.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Ring buoy was not marked with reroreflective tape. Vessel required to apply retroreflective tape to ring buoy.

A vessel must carry either a first-aid kit approved under approval series 160.041. The consumables found in the first aid kit were expired. Vessel required to place into service consumables which are equivalent and valid.

Provide Coast Guard Approved Ring Buoy.

Repair/Replace starboard fuel oil emergency cut off valve.

Provide two(02) portable flashlights.

Replace expired items in first aid kit.

Repair and prove proper operation of navigation lights.

Provide two(02) 2.5 gallon fire buckets, with lanyards.

Provide emergency check off procedures.

Provide anchor or grappling hook and line (ground tackle).

Provide one (01) 2-A fire extinguisher for the accommodation space.

Provide proof of payment of user fees.

Have both portable fire extinguishers and fixed-fire system (FireBoy) serviced.

Repair bilge pump float switch.

Provide audible alarm for vapor (gasoline) detection in the machinery space.

Owner to provide proof of enrollment in a certified DAPI program.

Replace life-ring buoy.

Replaced expired consumables in first aid kit.

Remove expanded foam from engine room hatches. (Used as a sound barrier, but not flame retardant.)
