The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. The alcohol swabs on board were expired. 46 CFR 4.06-15

A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12. The eye wash was expired. 46 CFR 199.175 (b) (10) (i)

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel COI has expired. Complete satisfactory COI renewal prior to the carriage of passengers. 46 CFR 176.100(a)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Fire/bilge belt running off forward portion of port main engine requires guard.

Gauges to indicate engine RPM, jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Lower control station indicator for starboard main engine was inoperable.

Batteries must be mounted in trays lined with, or constructed of, a material that is resistant to damage by the electrolyte. Batteries behind lower helm not stowed in tray.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Identified several nonmetallic pipes, metallic pipes, and electrical cables penetrating watertight bulkheads without maintaining watertight integrity, as required by vessel's Stability Letter. Restore watertight integrity to the satisfaction of the attending MI. 46 CFR 185.100

On a vessel of more than 65ft in length, the minimum capacity of the fire pump must be 50 gpm at a pressure of not less than 60psi. The engine driven fire pump does not meet 60psi. Vessel shall make repairs to the satisfaction of the attending marine inspector. 46 CFR 181.300(a)(iii)

Lakes, bays, and sounds, and rivers routes. A vessel on a lakes, bays, and sounds, or rivers route must carry: Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel does not have 3 red flare distress signals or equivalent flares. Vessel restricted to short runs of approximately 30 minutes away from dock until obtaining approved flares, to the satisfaction of the marine inspector. 46 CFR 180.68(b)

A fire pump must be capable of both remote operation from the operating station and local operations at the pump. The primary electric fire pump was not capable of remote operation from the operating station. 46 CFR 181.300(c)

Vessel has a power strip in the engine room that is hardwired into a breaker box. Vessel shall remove power strip to the satisfaction of the attending marine inspector. 46 CFR 183.200(b)

Vessel had several dead end wires, unused machinery, and unauthorized equipment permanently installed. Vessel shall remove dead end wires, unused equipment (with exception of out of commission diesel generator), and unauthorized equipment permanently installed to the satisfaction of the attending marine inspector. 46 CFR 183.200(b)

A grease extraction hood must be equipped with a dry or wet chemical fire extinguishing system meeting the applicable sections of NFPA 17 or NFPA17A and must be listed by an independent laboratory recognized by the commandant. Vessel shall have an approved dry or wet chemical fire extinguishing system installed on their grease extraction hood prior to using the system. 46 CFR 181.425 (b)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; (2) Perform the duties and functions of patrolmen or watchmen required by this chapter; or, (3) Are specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies. Only two crewmembers are part of random testing program. Create program to ensure pre-employment and random testing is completed for all crewmembers and test results are properly maintained, to the satisfaction of the marine inspector. 46 CFR 16.230(a)

A vessel must comply with the applicable requirements for any radio...including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission. Station license was expired. Obtain a valid station license to the satisfaction of the marine inspector. 46 CFR 184.502

A documented vessel must have onboard the original Certificate of Documentation currently in effect for that vesel. Vessel has a valid COD in the system but could not produce a physical valid COD. 46 CFR 67.313(a)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel has Out of Commission generator in forward space. Vessel shall repair generator to the satisfaction of the attending marine inspector or remove the power supply from the vessels grid and provide an electrical analysis for the safe operation of the electrical plant in its current state. 46 CFR 183.200(b)

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, must comply with this part (New T). Vessel has numerous electrical repairs, modifications, and equipment installations conducted after 1996 that are not in compliance with 46 CFR 183.115. Vessel shall make repairs to all applicable systems to include but are not limited to Fire Systems, Bilge systems, Steering, and lighting to the satisfaction of the local OCMI prior to December 1, 2023 or during next repower (whichever comes first). 46 CFR 183.115

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. Stern light was extinguished. Vessel operator replaced lightbulb and stern light is fully operated. 46 CFR 183.420

At each initial and subsequent inspection for certification of vessel, the owner or managing operator shall be prepared to conduct tests and have vessel ready for inspections of machinery. Vessels engine room generator was excessively leaking coolant and needs to be repaired to the satisfaction of the OCMI.

On a vessel mor than 65 feet in length, each hose must be lined commercial fire hose that conforms to UL 19 or hose that is listed and labeled by an independent laboratory recognized by the commandant as being equivalent in performance. Vessels FWD/AFT fire hoses were leaking through the lining and needs replaced.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Vessels visual alarm indicator was inoperable at the operating station.

Spanner wrenches must be provided where a 1.5 inch diameter firehose is required by 181.320(b). Vessel did not have spanner wrenches at their firestations.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons from electrical hazards, including fire. Dead end wires were found in the forward bilge area, the closet near the in house operating station, and in the aft bilge area in the master bedroom. Owner shall either removed or properly connect wires to mitigate potential hazards.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. Fuel shut off for port fuel tank was over actuating, therefore not fully closing. Vessel repaired valve while on board.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Vessels audible high level alarm was not working in the engine room. Vessel repaired alarm while on board.

Vessel's fire pump is not capable of providing the required throughput or of maintaining the required pressure. 46 CFR 181.300(b)

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines.Generator fuel shutoff valves did not close completely by using emergency fuel shutoff valve pull handles.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Port inboard exhaust manifold gasket failed allowing oil to accumulate on the manifold and exhaust fumes to enter the engine room.

At each inspection for certification, the owner shall have the vessel ready for inspection including all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Port wet exhaust found leaking water below the deck in the aft stateroom.

At each inspection for certification all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all protective devices must be in satisfactory condition. Wire connections in bilge for electric submersible bilge pumps found not to be watertight, which could result in a potentially hazardous situation should the pumps short out.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Medical certificates will be issued for a maximum period of 5 years. Vessel captain's medical certificate was found expired.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. The vessel's alcohol testing equipment was found expired.

The owner shall make sure that maintenance is carried out in accordance with the instructions required under the manufacturer's instructions. The vessel did not have an appropriate inspection/maintenance log for their lifesaving appliances onboard.

The water light attached to the life float by a 12-thread manila or equivalent synthetic lanyard, shall be at least 5.5 meters (18 feet) in length. The water light found on the vessel's life float was attached to a lanyard that was less than the required 18 feet in length. The operator replaced the lanyard on the spot with the appropriate size/material.

An Automatic Identification System (AIS) must be maintained in effective operating condition, which includes accurate input and upkeep of all AIS data fields and system updates. Vessel is missing required draft reading in its AIS profile.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. The painter for the life float installation is degraded. Replace the painter IAW 46 CFR 180.175.

Portable fire extinguishers shall be installed to the satisfaction of the [name withheld] fire extinguishers on the vessel are mounted with incorrect brackets. Replace brackets with approved type listed on fire extinguisher label. 46 CFR 181.30-10(b) "Old T"

Compressed gas cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. The carbon dioxide cylinders for the vessel's soda machine are not secured in a bracket. Stow the carbon dioxide cylinders IAW 46 CFR 147.60. 46 CFR 147.60

All marine employers must submit a Drug and Alcohol Testing MIS form. The operator failed to submit a MIS form for CY 2018. Submit an MIS form IAW 46 CFR 16.500. 46 CFR 16.500(b)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel is authorized to continue commercial operations until 10 May 2019. Beyond this date, vessel is considered overdue for its annual inspection.

ZERO DRAFT: no static draft information

Watertight Bulkhead Penetrations are not watertight. Hoses, Wires, and pipes pass through Watertight bulkheads 13, 37, and 48. Some of the penetrations are sealed with cauking and some are not sealed at all. Hoses are the type used for soda machines. Owner is to provide a plan to make all watertight bulkheads watertight. 46 CFR 171.114(a)

Unable to test Pre-Engineered system shutdowns for auxilary ssdg located in the Engine Room because it wouldn't start. SSDG is not to run with passengers until proper operation of shutdown may be demonstrated.

Determine which battery/starter cables are welding cables. Replace any that are not reccomended for use by the engine manufacturer. 46CFR183.05-40

REMOVE TEMPORARY HOUSEHOLD ELECTRICAL WIRING PENETRATING ENGINEROOM OVERHEAD AT AFT BULKHEAD TO STBD.

PLUG SMALL HOLE (1/8") IN PORT ENGINEROOM NATURAL VENTILATION DUCT BOX.

REMOVE TOWEL WRAPPED AROUND MSD DISCHARGE HOSE TO TANK (CABIN #3); PROVIDE HOSE CHAFING MATERIAL OR REPAIR HOSE AS NECESSARY.

REMOVE MISC ITEMS IN VICINITY OF STBD RUDDER ACCESS PORT IN ORDER TO PROVIDE VENTILATION TO THE COMPARTMENT (ACCESS PORT IS LOCATED IN MASTER SUITE BATHROOM).

REMOVE STANDING WATER LOCATED STBD SIDE, FORWARD E/R (4-5 IN). THERE ARE NO LIMBER HOLES IN THIS AREA TO ALLOW DECK DRAINAGE TO FLOW TO THE LOW POINT IN THE E/R.

PULL WIRE WHICH IS PRESENTLY NOT CONNECTED TO THE ELECTRICAL SYSTEM. WIRE IS LOCATED IN FORWARD JUNCTION BOX.

PROVIDE RADIO OPERATOR'S PERMIT AND RADIO TELEPHONY CERTIFICATE.

RENEW DETERIATED GASKET ON PORT HOLE IN # 2 CABIN.

STENCIL FWD AND AFT EMERGENCY ESCAPE HATCHES "EMERGENCY ESCAPE, KEEP CLEAR".

REMOVE AN REPLACE THE EXISTING CLASS I WATERTIGHT DOOR WITH ONE THAT IS A QUICK ACTING WATERTIGHT DOOR.

PROVIDE A COPY OF THE STABILITY LETTER

REMOVE ALL HARDWARE TO THE SHOWER AND TOILET SPACES FORWARD OF THE WATERTIGHT DOOR. (BEING DONE TO MAKE THE SPACE FWD OF THE WTD A MACHINERY SPACE VERSUS AN ACCOMODATION SPACE.)

REMOVE THE DOOR INTO THE FWD GENERATOR.

INSTALL AN ALARM ON THE QUICK ACTING WATERTIGHT DOOR THAT WILL ALARM AUDIBLY AND VISIBLY AT THE BRIDGE STATIONS.
