A vessel not more than 26 feet in length must carry a minimum of one USCG approved ring life buoy of not less than 20 inches in diameter. Found grab line around life ring frayed and severed when pulled by hand. Provide evidence of new life ring prior to carrying passengers.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate flooding in a machinery space bilge or other spaces subject to flooding from sea water. Observed installed alarm indicator but was unable to test the audio and visual alarm at the time of inspection. Provide evidence of working audible and visible high water alarm.

Vessel must carry a USCG approved first-aid kit. Found expired contents in the vessel's first-aid kit. Provide evidence of renewed non-expired contents.

No employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Observed no record of drug testing for 2 of 3 Captains employed. Provide proof of drug testing of the 2 other Captains prior to them operating your vessel.

On a vessel at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space bilge well, shaft alley bilge, or other spaces subject to flooding from water piping within the space.

Fuel shutoff valves must be installed to shutoff fuel flow and be operable from outside the compartment in which the valve is located. Observed remote fuel shutoff for port fuel tank was not fully operable.

The master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including the emergency instructions listed on the emergency instruction placard. Crew training shall be logged for review by the Coast Guard upon request and log entries shall include date of training and a general description of the training topics. Observed record showing vessel had conducted man overboard drill, however could not produce record of conducting fire and heavy weather drills. Provide proof of required training.

New installations of fire protective equipment on existing vessels must meet the requirements applicable after March 11, 1996. There must be one 10B:C portable fire extinguisher at the operating station and one 40B:C portable fire extinguisher located near the machinery space. Vessel has 10B:C rigged into the machinery space and 40B:C in the immediate area. Replace 10B:C with the 40B:C to protect machinery space with appropriate extinguisher. Relocate 10B:C to area within the immediate vicinity of the operation station.

Fire buckets must have an attached lanyard. Fire buckets did not have lanyards attached.

The owner or qualified servicing facility shall complete inspections and tests on portable fire extinguishers in accordance with NFPA 10. No evidence of servicing available. Provide proof of portable fire extinguisher servicing.

One ring life buoy must be fitted with a floating waterlight with a 3 foot lanyard secured around the body of the ring life buoy by a quick disconnect clip. Waterlight lanyard attached to vessel and no clip was present.

Placards containing donning and use instructions for lifejackets onboard. Multiple types of lifejackets onboard with donning instructions for only one type. Provide donning instructions for all types of lifejackets onboard.

Abandon ship, man overboard, and fire drills must be conducted at least once within a 3 month period before vessel gets underway with passengers. Drills must be logged providing dates and general description of drill scenario. Logbook unavailable for inspection. Will provide logbook and evidence of drills at all inspections moving forward.

All applicants for USCG officer endorsement must complete first aid and CPR courses. Vessel master could not provide proof of course completion. Provide evidence of course completion. 46 CFR 11.201
