"A" class divisions are those divisions formed by bulkheads and decks which comply with the following: they shall be so constructed as to be capable of preventing the passage of smoke and flame to the end of the one-hour standard fire test. PSCO observed a penetration through an "A" class boundary from the E/R to a workshop that was not properly insulated. The E/R to the workshop was labeled as an "A" class boundary on the Fire Control Plan. 74SOLAS(97) II-2/3.3

Resolution: Crew rectified the deficiency on the spot by adding insulation to the pentration to create a seal to prevent the passage of fire and smoke.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all rspects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO received notification per survey report of holes in the bulkhead stcture between the No.2 port fuel oil tank and No.3 port water ballast tank, that caused contamination of both tanks. Vessel is allowed to proceed to drydock to further assess damage and make repairs. 74 SOLAS (20) I/11(a)

Resolution: Class report was recieved attesting that the bulkhead holes of No. 2 pfo tank and No.3 pwb tank have been repaired with steel plate equivalent to the original. vacuum test was carried out and determined satisfactory.

PSCO received notification that during initial report above, contaminated fuel oil was transferred from the No.2 port fuel tank to No.2 starboard fuel tank. Provide documentation for proper disposal of contaminated fuel oil in No.2 starboard fuel tank. 74 SOLAS (20) I/11(a)

Improper connections were found in all three of the vessel's laundry rooms and the light outside accommodation space near the CO2 room was secured by zip ties. SOLAS 72(97)II-1/40.1.3

Means shall be provided whereby operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. vessel lost propulsion while leaving port due to a short in an electrical pane. Ensure with concurrence from class that appropriate repairs are completed.
