Fire extinguishers were past due for servicing.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed has extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. No vent closures were present. Operator is to show proof of vent closures and make readily available in the vicinity of the vent(s). [46CFR182.460(l)]

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Only one fire bucket on board. Require replacement of 02 missing fire buckets IAW above regulation. [46 CFR 181.610]

An adult life jacket must be provided for each person carried on board a vessel. Vessel is certified to carry 23 POB, only 20 adult life jackets on board. Vessel is restricted to carrying no more than 20 POB until missing 03 life jackets are replaced. [46 CFR 180.71(a)]

At least one ring life buoy must be fitted with a lifeline...Each lifeline on a ring life buoy must: Have a breaking strength of at least 5 kilonewtons (1,124 pounds). Lifeline is fraying and falling apart with minimal handling; require replacement. [46 CFR 180.70(c)(5)]

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Cooling water intake hose only has single clamp. Provide proof of double clamp on cooling water intake. [46 CFR 182.410(d)(1)]

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. No portable lights on board. Owner/operator must provide proof of 02 portable lights in required spaces. [46 CFR 183.430]

The master shall conduct sufficient drills to make sure that all crew members are familiar with their duties during abandoning ship, recovery of persons who have fallen overboard, and in case of a fire onboard the vessel. The training records for man overboard and firefighting were not present. Operator is to provide proof of training for abandoning ship, recovery of persons who have fallen overboard, and in case of a fire onboard the vessel. [46CFR185.520/185.524]

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of a fire, heavy weather, or man overboard conditions. No emergency check-off list was present on the vessel. Operator is to provide proof of development of emergency instructions and mounting at the operating station. 46CFR185-510(a)

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b) and other provisions of this section. Vessel is missing a 2A fire extinguisher per the Table 181.500(b). Operator is to show proof of purchasing and installing the missing fire extinguisher. 46CFR181.500(a)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. No life jacket donning placards were present. Operator is to acquire the appropriate life jacket donning placard for the life jackets onboard and post it for view of passengers. 46CFR185.516

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. The port side bilge discharge pipe was identified as damaged due to being melted by the vessel’s exhaust pipe. Operator is to replace the damaged pipe. 46CFR182.500(a)

46 CFR 176.801(a) - If during the inspection of a vessel, the vessel or its equipment is found not to conform to the requirements of law or the regulations in this subchapter, the marine inspector will point out deficiencies observed and discuss all requirements with the owner, managing operator, or a representative thereof. Fractures were found in the following locations: In the web of the frame just below fuel tanks in the machinery compartment at centerline extending from the top of the 1st stringer web attachment port side to the top of the frame web. In the gusset weld attachment to the bottom of the gunnel and forward machinery space bulkhead just above the fuel oil shutoff valve on the port side. Full length of transverse weld seam in the forward machinery space bulkhead just below the fuel oil shut off valve port side. Fractures are to be repaired to satisfaction of attending marine inspector.

All closure devices must be readily available and mounted in the vicinity of the vent. Vessel is not equipped with ventilation closures. Provide means for closing supply / exhaust scoops for the machinery space.

46 CFR 184.710 - A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. During the annual inspection, it was determined that the vessel is equipped with an "equivalent kit". The vessels first aid kit was not in a suitable watertight container, was not equipped with triangle bandages, and the analgesic meds / antiseptic preparations were expired. Provide a suitable watertight container and replace expired antiseptic preparations / analgesic medication.

46 CFR 176.810(a)/(b)(iv) - At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel did not have record of required annual servicing for portable of fixed extinguishers. A qualified servicing facility must perform required inspections / tests, and evidence of such is to be provided to the MI.

Within 30 days, to the satisfaction of the USCG renew fractured welds located at the adjoining bow and body where the structure meets the hull on the port and starboard side.

Within 30 days, to the satisfaction of the USCG, prove the Clean Agent Fire Extinguishing System (Fireboy) underwent annual inspection, servicing and testing IAW NFPA 2001 by a qualified Fire Protection Service Technician.

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with manufacturer's instructions. Xintex Fireboy requires that the system be maintained in accordance with NFPA 2001, which states, at least annually all systems shall be inspected and tested for proper operation by personnel qualified in the installation and testing of clean agent systems. The Xintex Fireboy onboard has not yet undergone an annual inspection. Have a qualified servicer conduct the appropriate servicing of the installed fire extinguishing system and notify the Marine Inspector upon completion.

Vessel owner to provide servicing report for fixed/installed fire fighting system, including the alarm.

Replace expired consumables in the first aid kit.

Replace worn life-ring buoy and attached line.

Prove proper operation of fume (gasoline) detector located in the machinery space.

Post life-jacket donning placard.

Install flame screens in the drip pans under the fuel filters.
