A vessel equipped with fixed pitch propellers must have on the navigating bridge and at the engineroom control station a propeller speed and direction indicator for each shaft. Navigating bridge indicators were inoperable during forward and aft shaft rotations. Shaft sensors were not installed after dry dock. 46 CFR 113.37-5(a).

Each fire hydrant must have a fire hose 15.2 meters (50 feet) in length, with a minimum diameter of 38 millimeters (1 1/2 inches), connected to an outlet, for use at any time. During fire main hose and pressure checks found engine room hose failing at the coupling connections. Vessel to replace fire hose. 46 CFR 132.130

No person may operate an oceangoing ship of 400 gross tons and above but less than 10,000 gross tons, excluding a ship that carries ballast water in its fuel oil tanks, unless it is fitted with approved 15 parts per million (ppm) oily-water separating equipment for the processing of oily mixtures from bilges or fuel oil tank ballast. During IOPP survey found OWS inoperable and disassembled for repair. Vessel to repair, calibrate OCM, and test system. 33 CFR 155.360(a)(1).

The owner and operator of the vessel shall ensure that when cargo is due for transfer, the person in charge of the transfer has received special training in the particular hazards associated with the cargo and in all special procedures for its handling. Crew lacked sufficient knowledge on the safe operation and transfer of liquid mud and methanol cargoes. Vessel owner/operator to provide training on the use of installed systems including emergency procedures before carrying such cargoes. 46 CFR 153.957(a)(3).

Each vessel must carry a lifejacket for each person on board. A COI endorsement was issued in 2017 which allowed the vessel to operate as an industrial vessel and carry 21 industrial personnel. COI carriage requirement stated only 20. Vessel restricted to 16 industrial personnel until objective evidence is provided to allow 21 to be carried. 46 CFR 199.70(b)(1).
