Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 91.25-7. The portable C02 fire extinguisher attached to the on deck flammable storage locker has not been serviced since 2023. 46 CFR 91.25-20(a)(1)

The aforementioned information described in 46 CFR 97.36-1(a) shall be kept up-to-date, any alteration being recorded in the applicable medium as soon as practicable. The General Arrangements depicted on the Fire and Safety Plan were approved in March 1996 and have not been updated to reflect subsequent alterations made onboard the vessel. Update Fire and Safety Plan during next dry dock, submit to OCMI for review/approval, and complete any necessary onboard verifications by the attending Marine Inspector. 46 CFR 97.36-1(c)

The manufacturer's instructions for onboard maintenance of lifesaving appliances must be on board the vessel. The following must be provided for each appliance: (i) checklists for use when carrying out the inspections required under paragraph (e) of this section; (ii) maintenance and repair instructions; (iii) a schedule of periodic maintenance; (iv) a diagram of lubrication points with the recommended lubricants; (v) a list of replaceable parts; (vi) a list of sources of spare parts; and (vii) a log for records of inspections and maintenance. The vessel has all Imperial immersion suits that are >5 years old. Imperial's manufacturers maintenance instructions calls for annual inspection/maintenance (leak/pressure testing) for all suits > 5 years old. At time of inspection there was no objective evidence that this has been carried out for the onboard suits. 46 CFR 199.190 (b)(1)

In accordance with subchapter I (46 CFR 96.06-1) The installation of all lifesaving appliances and arrangements must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Each cargo vessel must carry — Liferafts—(i) With an aggregate capacity sufficient to accommodate the total number of persons on board and that are stowed in a position providing for easy side-to-side transfer at a single open deck level; or(ii) With an aggregate capacity on each side sufficient to accommodate the total number of persons on board. Vessel maintains 02-ten person liferafts that cannot accommodate the maximum persons allowed of 21 as listed on the Certificate of Inspection. Limit persons allowed on board to no more than 20, until liferaft(s) aggregate capacity is installed to accommodate 21 persons.46 CFR 199.261(b)(2)

MIs found onboard breaker panels lacking circuit breaker labels. All electrical equipment and circuits must be clearly marked and identified.

MI found storage space/laundry space smoke detector did not properly activate alarm and strobe inside the space. Fire detection, manual alarm, and supervised patrol systems are not required except in special cases; but if installed, the systems must meet the applicable requirements of 46 CFR, part 76 of subchapter H.

Water tight bulkhead has electrical wiring that has not been made water tight.

Ring life buoys must be approved in accordance with 46CFR160.050 of Subchapter Q. Vessels starboard ring life buoy aft of the pilothouse was not in acceptable working condition. 46CFR180.70(b)(1)

Electrical equipment on a vessel must be installed to protect passengers, crew, or other persons from electrical hazards including fire or electric shock. Vessel had numerous frayed dead-end wires under bridge console. 46CFR183.200

Must have a detailed inspection of radio installation once every five years and be issued a safety radiotelephony certificate. Vessels certificate expired 03/24/20. Pass radio inspection and show proof of valid certificate. 47 CFR 80.903
