1410-Propulsion main engine-The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpos, the following functional requirrements shall be met: Means shall be provided to control leaks of flammable liquids. PSCO observed MDE had multiple fuel leaks located on high pressure fuel pumps and fuel injectors. Also observed lube oil leaks on multiple cylinder heads. SOLAS 74 (a) (2004) Cons II-2/4.1.1 60a

Special consideration shall be given to the malfunction of an air compressor and receiver for starting or control purpose. Resolution: Noted valves that were malfunctioned due to wear and tear replaced. Air compressors 1,2,3, tested on auto for low pressure to air bottles. Engine started in ahead and astern direction 6 times, all sat.

1270-Doors-All access opening in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weather-tight when closed. PSCO observed that the provision door, steering gear access doors from main deck were not weather-tight. 1099-Other-Bulk carriers shall be fitted with water level detectors: In each cargo hold, giving audible and visual alarms, one when the water level above the inner bottom in any hold reaches a hieght of 0.5m and another at a height not less than 15% of the depth of the cargo hold but not more than 2m. PSCO observed that the Water Ingress panel was powered off. When power was turned on the panel gave multiple failure alarms. A record shall be kept, to the satisfaction of the administration and as required by the radio regulations, of all incidents connected with the radio communication service which appear to be of importance to safety of life at sea. During the review of the GMDSS log it was observed by the PSCO that the crews radio personnel were not conducting tests at the frequency required and the record of MF/HF failure was not logged IAW regulations and ship policy.

If the vessels radio navigation receivers stop operating properly, the person directing the movement of the vessel must report that it is not operating properly to the nearest Captain of the Port, District Commander, if participating in VTS, to the VTC, as soon as possible. Vessel did not report the inop condition of their MF/HF radio to Sector NOLA on their NOA. Provide company clarification on reporting procedures to port state authorities.
