Vessel must have life rafts with an aggregate capacity on each side sufficient to accommodate the total number of persons onboard. 8 of 10 life rafts have been removed from the vessel for required maintenance. Vessel's total persons allowed onboard restricted to 50 until life rafts have been serviced and reinstalled.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Port and STBD motors suffered a reduction in RPM’s, provide tech report detailing repairs and successful testing of Port and STBD propulsion motors.

Ferrous pipe used for salt water service must be protected against corrosion. Ballast piping in the Water Heater room 2-100-4, is wasted at the valve. Temporary syntho-glass pipe patch has been installed at the skin valve to the hull. Replace in kind piping at the next dry dock scheduled in 2025. 46 CFR 56.60-3 Code 705 c

Marine Sanitation Device - Each device must be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow toxic substances to escape to the interior of the vessel. In the MSD space there are 02 temp repair syntho-glass patches. One patch is on the discharge line from the Type II treatment plant. The other is from the space bilge suction to the black water tank. Both shall be replaced in kind at the next dry dock in 2025. 33 CFR 159.95 Code 705 c

The material and arrangement of main propulsion machinery and each auxiliary directed connected shall meet the standards established by ABS. Shaft seals must ensure proper watertight integrity and maintain their functionality. Approximately 30 GPM was observed leaking at the port and starboard (combined) shaft seals at various RPM’s.

In accordance with memorandum of agreement government owned contractor operated Military Sealift Command Vessels meet applicable regulations of 46 CFR, with exceptions outlined in Annex II and III or as modified. Each Vessel of 10,000GT or more must have in addition to the radar system under 33 CFR 164.35(a) a second marine radar system that operates independently to the first. Vessel's S-Band 3Ghz radar is not operable and requires replacement. Vessel will replace to the satisfaction of the attending MI. Vessel will operate in Sector Jacksonville COTP zone on continuing LOD until replacement, if vessel is required to travel to another COTP a new LOD must be submitted. 33 CFR 164.37

Vessel’s port shaft inner seal failed/overheated during sea trials. When a defect is discovered or whenever any important repairs or renewals are made. A survey shall be such as to insure that the necessary repairs or renewals have been effectively made. Prior to departing from Port of New York/New Jersey provide/submit a copy of the cause and repair statement from the manufacture representative to the satisfaction of the OCMI.

Demonstrate operation of #2 and #4 lifeboats.

Demonstrate remote closure of all watertight doors using master switch and local opening ensuring all doors function as designed.

Demonstrate activation of smoke detector, zone 25, deck 01, IWO fire station 24.

Identify breakers labeled with tape on panel 5SRP111A2, IWO bridge across from GMDSS and install appropriate/accurate labels.

Ensure smoke/fire detector, call point list posted adjacent to alarm panel contains accurate information regarding number of devices per zone.

Locate fire damper 02-136-2, demonstrate operation and ensure it's ID'd on FCP

Renew deteriorated flex loops on local fixed CO2 systems for the #'s 1,2,3,& 4 DG's

Affect permanent repairs to address leak in overhead, port side bridge as ID'd by ABS - At next drydock

Install (provide on board) updated/approved Fire Control Plan (FCP) - At next drydock

Develop and implement a system/schedule to test smoke/heat detectors and manual call points, utilizing manufacturer's recommended procedures or safe/practical method. Incorporate process in Safety Management System. - At next drydock

Affect permanent repair to knife edge IWO intake 02-336-2, service fan room 02-324-2 - At next drydock

Identify solution to A-0 fire screen door installed at entrance to GMDSS space wrt the requirement to remain open for battery vent vs. closed for structural fire protection - At next drydock

Hydro-test sponsor generator fuel replenishment hose.

E-stop for FO purifier at upper AMR #2 228-2 not functioning properly.

Adjust exhaust blanlets on MDE's so they as designed.

Repair hydraulic leak on starbaord Palfinger winch.

Repair leaking hydraulic seal on ram to WTD 2-80-4.

Demonstrate proper operation of gooseneck vent closure on main deck, P/S aft of exterior ladder.

Demonstrate proper operation of natural vent closure of S/S aft of bridge.

Locate and repair source of hydraulic leak for the port side anchor windlass.

All recommendations pertaining to lifeboat #1 detailed in Harding Safety dtd 11JUL16 shall be corrected NLT date indicated (report scanned into MISLE documents).

Repair J/W leak on outboard side of EDG.

Repair J/W leak on SSDG #1 located above AMOT valve.

Renew deteriorated S/W cooling lines on the #5 SSDG.

Replace missing SFP insulation under deck plate along fwd blkhd IWO SSDG #3.

Properly repair chafed wire under local control panel for SSDG#3 and #5.

Replace cover to J/W pump motor for SSDG #5.

Remove or enclose dead end wire located on blkhd between EOS and SSDG #1.

Make repairs to condensation leak IWO door 1-181-1.

Replace and re-test the CO2 time delay for the propulsion motor room so that the time delay triggers within the 60 seconds allotted.

Replace and re-test CO2 time delays for the paint storeroom, grease locker and the paint mix room so that the time delays trigger within the 30 seconds allotted.

Provide complete A-60 fire protection over the elevator doors located on each deck. The elevator doors do not currently fulfill the fire boundary requirements on each deck.

Provide USCG Marine Safety Center approved FMEA and DVTP's for the main diesel generator new installation local control panels & revised vessel vital system automation periodic safety test procedures to the satisfaction of the attending USCG Marine Inspector. 46CFR 61.40

Provide a summary alarm at the MCC for all main diesel generator alarms that alarm at the main diesel generator local control panels and do not alarm at the MCC. Installation and testing shall be to the satisfaction of the attending USCG Marine Inspector. 46 CFR 62

Repair the Halon release visual indicator alarm on the MCC to the satisfaction of the attending USCG Marine Inspector. 46 CFR 62.25-20

Repair the OWS/OCM 15PPM alarm prior to operation of the OWS to the satisfaction of the attending USCG Marine Inspector. 33 CFR 155.380

Replace all fire station Y-Gate valves that are not made entirely of metallic components. All Y-Gate valves onboard are currently equipped with plastic balls/handles. 46 CFR 193.10-15 & 46 CFR 56.50-95

Replace and re-test all the CO2 time delays for all generator and motor enclosures so the time delays trigger within the 30 secs allotted. If the decision is made to remove the time delays, submit plans to MSC regarding the new piping arrangement for approval. 46 CFR 193.15-10

Pump #2 (Port) inop from bridge station. ops test at local station - SAT. Provide technicians report and proof of operations prior to departure from San Diego.

Due to propulsion generator and sea water pump overheating, conduct and complete trouble-shooting of the entire cooling water system. Remote operation of the overboard valves is to be tested and verified as operating in accordance with the design specification. All overboard valve external indicators are to be checked and indicate the valve position.

Until the design verification test procedures of the engine room automation system is tested by the ship's crew and verified by the USCG, the engine room must remain manned in accordance with the COI.

The main generators are over-heating. The port MSW overboard discharge valve and sea-water pumps are not operating as designed and require maintenance. Prior to departure from port, have Class (ABS) attend and verify the machinery plant is operating within design specifications.

Properly repair or replace starting air compressor.

Prove proper operation of Diesel Generator #5 following overhaul and repairs. Demonstrate all safety shutdown in accordance with PSTP prior to operating generator set.

Provide USCG Sector Portland Inspection Office with a copy of the service report from DG International tech rep documenting the calibration of the ship's machinery automation system, including all recommended repairs and/or mods. 46 CFR Part 62

Provide proper operation of all remote tank level indicators (TLI's) and remote valve operation for the ballast system from EOS main console. 46 CFR Part 62

Replace EG-III control module for the main generator on #1 generator operating governor ([name withheld] Control). 46 CFR Part 58

Prove proper operation of the sound powered phones on the Bridge and After Steering station. 46 CFR 113.30

Renew lifting straps and prove proper operation of the rescue boat. Designate one (1) life boat that will be used until repairs are completed to the rescue boat. 46 CFR 199.160 / SOLAS 74/78 III-16

Provide, install, and prove proper operation of the following watertight doors: 2-120-1; 2-280-1; 2-280-2; 2-328-1. 46 CFR 111.97

Renew the following hoses: 4 gauge hoses on #1 MDE; 2 gauge and 2 heat exchanger hoses on #2 MDE; 6 gauge and 2 heat exchanger hoses on #3 MDE; 3 gauge and 2 heat exchanger hoses on #4 MDE; 3 gauge and 2 heat exchanger hoses on #5 MDE; and all fuel hoses on the Emergency Diesel Generator. All hoses shall meet the SAE-1942 Standard. 46 CFR 56.50

Provide temp repair proposal IWO 12 1/2" crack in the bilge keel at frame 224 and make temp repair to the satisfaction of the attending marine inspector prior to departure. Make permanent repair to the satisfaction of the attending marine inspector during the next scheduled dry-docking in MAY 2008.

1/4 inch hole allowing sea water to enter Oily Water Tank, 5-248-2, caused by sounding bob. A six inch diameter 5/8" doubler plate has been installed. Cofferdam was removed and all welds are holding satisfactory with approximately 9.7 psi of sea water on the outside hull plating. Make permanent repairs to bottom shell plating in oily water tank (5-248-2), during scheduled drydocking in May 2008.

Permanent repairs to the A/C chilling water line, where wasted, are to be completed to the satisfaction of the attending marine inspector at the vessels next scheduled dry docking.

Permanent repairs to the bilge suction in the lower motor room, IWO soft patch, are to be completed.

Suction line IWO #2 central fresh water pump is to be replaced.

Lighting fixture IWO provision loading chute is to be repaired.

Audible portion of the low lube/overspeed trip alarm for the #5 SSDG is to be repaired.

Flexible hose supplying jacket water cooling to the #3 and #4 SSDG's is to be renewed.

Time delay for the fixed halon system supplying AMR #1 was not operating correctly. Time delay shall be renewed and tested prior to vessel operating.

Actuation valve for the pilot cylinder on the fixed halon system supplying the #3 generator room was not functioning correctly. New valve required prior to vessel operating.

Valve handle is to be installed on the oil waste discharge line where missing.

Recent ananylsis for the AFFF fire fighting foam is to be provided.

Repair #1 generator bearing damage to the satisfaction of class.

Complete programmable system control evaluation and incorporate into Periodic Safety Test Procedures. Resubmit to cognizant OCMI for approval. 46 CFR 61.40 and 46 CFR 62.25-25

Prove proper operation of 5-10 second pre-closure alarm for all Class III watertight doors iaw ASTM 1197.

Prove red cotton jacket fire hoses meet 46 CFR 181.320 or an equivalent navy standard, or replace with appropriate hose

Properly pack bulkhead penetrations with approved fire retardant material

Repair junction box on pre-lube selonoid for #5 generator. (46 CFR 110.10)

Replace gauge on #5 generator pre-lube pump. (46 cfr 110.10)

Replace fuel oil-soaked flange shielding on emergency generator. (46 CFR 112.25)

Repair wastage in fuel tank containment vent # 5.176.2. (33 CFR 155.320)

Install retro-reflective tape on bottoms of all lifeboats. (SOLAS 74/78 III/30)

Repair saltwater leak on spool section for saltwater discharge.

Update and examine vessel documents such as Loadline upon return from drydock.

MAKE PERMANENT REPAIRS TO HULL ON STARBOARD SIDE WHERE THE HULL MEETS THE TRANSOM, SEVEN FEET ABOVE THE WATERLINE AT NEXT DRYDOCK OR NO LATER THAN 30NOV2003.

SUBMIT FOR REVIEW AND APPROVAL VESSEL'S NEW TRIM AND STABILITY BOOKLET TO MARINE SAFETY CENTER. MASTER MAY CONTINUE TO USE EXISTING TRIM AND STABILITY BOOKLET UTILIZING INTERIM STABILITY GUIDANCE PROVIDED.

PROVIDE AN UPDATED FIRE/SAFETY PLAN FOR COAST GUARD APPROVAL IAW 46 CFR 97.36.

RENEW CURRENT VENTILATION SENSORS (P/N 20-210) WITH SENSORS APPROVED FOR USE IN HAZARDOUS ATMOSPHERE IN THE FOLLOWING LOCATIONS: BATTERY RM, PAINT MIXING RM, PAINT LOCKER, GREASE LOCKER AND MSD SPACE. CURRENT SENSORS MUST BE DE-ENERGIZED AND TAGGED OUT IMMEDIATELY.

PROVIDE AN UPDATED FIRE/SAFETY PLAN FOR COAST GUARD APPROVAL IAW 46 CFR 97.36.
