Vessel was due for drydock examination on November 20th, 2025 and has yet to receive a drydock exam. Vessel shall not operate with passengers until a drydock inspection has been completed. 46 CFR 176.600(c)(1)

MI observed vessel with 7 total Flares. Vessel required to have 12 Total Flares onboard (6 Day/6 Night), vessel only had 4 Night Flares and 3 Smoke Flares. Vessel Rep purchased 6 additional Night/Day Flares to be in compliance. Corrected on the spot.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry -Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. A hand red flare distress signal may be substituted for any of the hand orange smoke distress signals required under paragraph (a) or (b) of this section. Four red hand flares were found expired. The vessel has eight red hand flares onboard for distress signals. 46 CFR 180.68

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The hydrostatic release was found expired. 46 CFR 185.740

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b) and other provisions of this section. The COI requires the vessel to carry one 2-A portable fire extinguisher. One 1-A portable fire extinguisher was found onboard. 46 CFR 181.500

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Two 8 AWG conductors were found in the engine room, used as a jumper from terminal strip to stud, without overcurrent protection. The conductor must be protected IAW its current carrying capacity. 46 CFR 183.380(l)

A Certificate of Inspection is valid for 5 years. During administrative inspection, MI found vessel's COI to be expired. Vessel shall not carry passengers until MI issues renewal COI. 46CFR176.107(b)

A vessel must be provided with a main steering gear that is: Of adequate strength and capable of steering the vessel at all service speeds; Designed to operate at maximum astern speed without being damaged or jammed; and Capable of moving the rudder from 35 degrees on one side to 30 degrees on the other side in not more than 28 seconds with the vessel moving ahead at maximum service speed. The vessel lost steering and movement of the rudder while conducting a man overboard recovery drill underway during a COI inspection. A technician is scheduled to attend on February 11, 2022. 46 CFR § 182.610

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Operator failed to test the EPIRB monthly in Nov and Dec 2021 and Jan 2022. EPIRB tested on the inspection this date and logged. 46 CFR § 185.728

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The crew failed to conduct a drill every three months. The last recorded dril was on October 4, 2021. 46 CFR § 185.520

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC Safety Certificate is expired.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first aid kit onboard did not have all provisions from 160.041 and several items were expired. 46 CFR § 184.710

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. The conductors from the battery switch to the DC panel have blade automotive fuses installed instead of UL listed cartridge type fuses. 46 CFR § 183.380(L)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. An aux sail preventative maintenance plan shall be in place and completed by the crew. The PMP shall be logged by the crew upon completion. The crew established a PMP after the previous USCG Annual inspection but crew members failed to log the PMP maintenance conducted over the past year. 46 CFR § 176.840

The preventative maintenance plan (PMP) should address the following at a minimum: the maintenance schedule of the rigging and associated equipment to include detailed protocol for examinations and inventory of rigging components including age of component as well as size and replacement schedule. The vessel's PMP does not contain the age of components or replacement schedules. The vessel's PMP is missing detailed protocol for examinations to be conducted by the crew. Identified the following during a visual inspection of the deck level standing rigging: 1/2" fracture in the starboard shroud chainplate. The third party rigging inspection completed on Feb. 22, 2021 shall establish a baseline to create the vessel's PMP. Provide a PMP for this vessel. The PMP shall include at a minimum: the maintenance schedule of the rigging and associated equipment to include detailed protocol for examinations, inventory of rigging components including age of component as well as size, and replacement schedule (life cycle). A third party rigging survey does not substitute for the vessel owner/operator from implementing a PMP or conducting/documenting preventative maintenance. 46 CFR 176.840

Rigging component was found with evidence of a fracture. Starboard shroud chainplate found with 1/2" long fracture. The fracture was found 2" above the bottom of the chainplate and traveled in a horizontal direction. Due to the location of the fracture being found at the bottom of the chainplate, scantling of the chainplate (3/4"), the continued monitoring of the fracture by the master/owner, and recommendations from the attending third party surveyor, the chainplate may remain in service until the next scheduled dry dock where it shall be replaced. Evidence of the fracture continuing to grow or creating new fractures above this area will require the chainplate to be replaced immediately. It is the owner/master's responsibility to monitor this area for further damage. 46 CFR 176.600

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. VHF radio will not broadcast GPS position due to a faulty GPS. Renew GPS installation and connect to the VHF for DSC broadcast. Contact inspector for re-inspection when installed. Vessel is restricted to routes no further than 1000 feet from shore until rectified. 46CFR184.52

The mast is structurally supported by a transverse crossbeam made of FRP visible on the main deck which is fastened to each hull using a chain plate. Visually inspected the chainplates for the mast crossbeam and identified pitting corrosion under the bolts/washers used for fastening. The mast crossbeam chainplates were 3/4" thick and appeared servicable until the next dry dock in the corroded condition. The pitting corrosion depth measured up to 3/8" in some areas but the pitting diameter was less than 1/8" in these heavily pitted areas. The owner shall replace the chainplates or submit a proposal to repair and leave the chainplates in service. 46 CFR 176.600

Structural support for starboard side trampoline net was found fractured at the bolt fastener connection. Structural support is bowed out in several areas due to weight on the trampoline net and the fractured support. Passengers are not permitted to use the starboard trampoline netting. Make satisfactory repairs to the trampoline net structural support. 46 CFR 176.600

Each ring life buoy on a vessel must: be approved in accordance with 160.050 in subchapter Q of this chapter. The ring life buoy's grab line was deteriorated and no longer serviceable. Replace. 46 CFR 180.70(b)(1)
