The Company and the ship must comply with the requirements of the International Safety Management Code. The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the Company. Objective evidence discovered during an expanded ISM exam revealed that the vessel failed to properly inspect, maintain, and report nonconformities relating to vessel main engine leakages in accordance with ship's SMS procedures. In order to account for low cooling water temperatures in No. 6 cylinder, ship's crew changed temperature alarm set points and isolated the cylinder without any documentation or notifications. Corrective action must be taken within three months.
