The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Upon boarding the vessel, the Pilot discovered the RPM displays on the bridge and in the engine room were inconsistent. Ensure RPM displays are operating properly and the vessel is safe to proceed to the satisfaction of Class and the USCG.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Auxiliary Engine # 1 has an active cooling water leak due to a crack in the piping.

Each vessel of 10,000 gross tons or more must have, in addition to the radar system under 164.35(a), a second marine radar system that operates independently of the first.

In meeting these requirements, the company should ensure that: inspections are held at appropriate intervals; any non-conformity is reported, with its possible cause, if known; appropriate corrective action is taken; and records of these activities are maintained. Objective evidence discovered during an expanded ISM exam revealed that the vessel failed to properly inspect, maintain, and report nonconformities relating to fire doors, generator oil leaks, and oily water separator equipment. An external third party SMS audit is required prior to departure from port. 74 SOLAS (2020 cons) IX/3.1 ISM Code 10.2

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCOs observed significant fuel oil leaks on the number three (3) generator causing a fire hazard. 74 SOLAS (97 cons) II-2/15.2

The fire resistance of doors shall, as far as practicable, be equivalent to that of the division in which they are fitted. “A” Class fire doors throughout the engine room are holed, missing latches, and/or have loose door handle accessories. 74 SOLAS (97 cons) II-2/47.1

Except as specified in Paragraph 3 of this Regulation, any ship of 10,000 gross tonnage and above shall be fitted with oil filtering equipment complying with Paragraph 7 of this Regulation. Vessel’s C/E reported that the OWS three way valve would not fully open and close during testing and is now being overhauled. MARPOL I/14.2

Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. PSCO observed gaskets missing or damaged in several door frames. 74 SOLAS (2001) II-2 / 47.1

Any required fire detection system shall be capable of immediate operation at all times. PSCO observed a plastic bag covering a smoke detector. 74 SOLAS (1997) II-2 / 12.1.1

Fire-extinguishing appliances shall be kept in good order and be available for immediate use at all times. PSCO observed boxes blocking a hose station near the boiler. 74 SOLAS (1997) II-2 / 21

Door fitted in boundary bulkheads in machinery spaces of Category A shall be reasonable gas tight and self closing. PSCOs found multiple class "A" fire door closing mechanisms inoperable with broken hardware, preventing both opening and proper closure. Degrading the structural fire protection of the Category A-60 spaces as indicated on the fire control plan. 74 SOLAS (1997) II-2/47.1 Code 17ac

Every ship shall be provided with fire main complying as applicable with the requirement of this regulation. PSCOs found substantial deterioration to the fire main with multiple temporary repairs made to the piping, significantly reducing the systems integrity. 74 SOLAS (1997) II-2/4.1 Code 17ac

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and ensure that inspections are held at appropriate intervals, any non-conformities are reported, appropriate corrective action taken, and records of these activities, PSCO's noted several deficiencies related to firefighting systems, structural protection, lifesaving equipment and the lack of required reporting to company, Flag and Class,which objectively demonstrates non-compliance with relevant conventions, calling into question the adequacy and implementation of the vessel's SMS under the ISM code. An external audit is required to be conducted by the Flag or RO within 30 days to determine the ship is operating IAW the ISM code. Provide a copy of the completed audit report to USCG prior to departure. 74 SOLAS(20) Xl-l/4.1 ISM 10.1 17ac

Two means of escape shall be provided from each machinery space of category A, in the lower space, the steel door shall be capable of being operated from each side to provide access to a safe escape route. PSCO's found exit door located in electricians workshop blocked with boxes and stored ship parts. Upon clearing exit path, it was discovered that door mechanism does not properly operate, preventing access to emergency escape just outside of compartment. Additionally, escape breathing device in engine room found empty. 74 SOLAS 97 cons. ll-2/45.3 17ac

The machinery shall be so installed and intended to reduce to a minimum any danger to person on board with due regard to moving parts. PSCO's observed #2 M/E seawater cooling pump missing guard in way of electric motor shaft. 74 SOLAS 97 cons. ll-1/26.1 50ac

Plans and booklets shall be kept up to date, any alteration being recorded thereon as soon as practicable. Class "A" self closing fire door leading to maneuvering and switchboard room is not reflected on the fire control plan being displayed. 74 SOLAS 97 cons. ll-2/20.1 40ac

The opening below the lowest course of the guard rails shall not exceed 230mm. While conducting deck walk, PSCO discovered missing bottom guard rail. ILLC 1966/25.3 40ac

The associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Hydraulic value operator for the #2 M/E sea water cooling discharge is inoperable and has been removed, preventing the opening and closure of value. 74 SOLAS 97 cons. ll-1/26.1 50ac

Crew members failed to test equipment properly IAW the ships proceedures of the engineering spaces heat detectors.74SOLAS(14)IX/3, ISM Code 10.

PSCO observed under-inflated rescue boat & deck material that was rotted. This caused a distortion of the shape of the rescue boat. 74SOLAS(14)I/11

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. PSCO observed load line mark painted same white color as the hull, not the required contrasting black.
