46 CFR 71.250 - At each annual inspection, the inspector must ensure that the following tests and inspections of fire detection and extinguishing equipment have been conducted: All portable fire extinguishers and semi-portable fire extinguishing systems must be maintained in accordance with NFPA 10, chapter 7; Carbon Dioxide Weigh cylinders; All fire detection and extinguishing systems, all piping controls, valves, and alarms must be checked to ascertain that the system is in operating condition. Annual servicing/inspection not completed (Exp May 2025)

46 CFR 113.40-10 - Each rudder angle indicator system must have a transmitter at the rudder head that is actuated by movement of the rudder with the angular movements of the rudder transmitted to a remote indicator or indicators. However, the indicator may be physically located on a control console, such as an integrated bridge system, if it is readily visible by the helmsman at the steering stand. Rudder angle indicators in engineering and in wheelhouse do not match and are inaccurate relative to machinery equipment.

46 CFR 78.15-1 Subdivision bulkheads. (a) All watertight doors in subdivision bulkheads shall be kept closed during navigation except when necessarily opened for working of the vessel, and in such cases they shall always be ready to be immediately closed. Discovered the door had been removed; prior to the re-inspection. Forward WTD of MDE space is being refabricated. Prior to Movement, prove proper watertightness of WTD.

Scope. The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in satisfactory condition. Port ship’s service diesel generator is inoperable and requires overhaul for repair. Rectify within 120 days.

Flexible electric cord and cables. No flexible cord may be used except: (1) As allowed under Sections 400.10 and 400.12 of NFPA 70. Flexible cords, flexible cables, flexible cord sets and power supply cords shall not be used as a substitute for the fixed wiring of a structure. Flexible cord is being used to power new freezer in main deck galley. Current power cable is too short and cannot reach dedicated receptacle. Replace power cable or move dedicated receptacle closer to the appliance.

Temporary emergency loads. The following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: At least one light in each space where a person may be maintaining, repairing, or operating equipment, stowing or drawing stores or equipment, or transiting, such as… galleys. 01 Deck Galley emergency lighting lamp is inoperable.

Accessibility and spacing. The design and arrangement of electric apparatus must afford accessibility to each part as needed to facilitate proper inspection, adjustment, maintenance, or replacement. 01 Deck Galley 120v panel door is damaged, secured with duct tape. Repair panel door to facilitate proper use.

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this sub chapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Persons designated by the owner of the vessel must conduct all tests and the Design Verification and Periodic Safety tests must be witnessed by the Coast Guard. Design Verification Test Procedures (DVTPs) have been approved by MSC but have not yet been witnessed by the Coast Guard.

Electric installations on vessels must ensure:Maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. Anchor light receptacle junction-box is inoperable. Anchor light is currently receiving power via extension cord. Repair/replace primary anchor light receptacle as designed.

The fire detection and alarm system must control and monitor input signals for all connected detectors and manual pull stations or call points. Smoke detector in storage room on main deck has been removed. Re-install new detector when received to clear fire control panel fault. Conduct operational test upon re-installation.

The following plans must be submitted to the Coast Guard for approval in accordance with §§ 50.20-5 and 50.20-10 of this chapter: (1) A general arrangement plan of control and monitoring equipment, control locations, and the systems served. (2) Control and monitoring console, panel, and enclosure layouts. (3) Schematic or logic diagrams including functional relationships, a written description of operation, and sequences of events for all modes of operation. (4) A description of control or monitoring system connections to non-vital systems. (5) A description of programmable features. (6) A description of built-in test features and diagnostics. (7) Design Verification and Periodic Safety test procedures described in subpart 61.40 of this subchapter. (8) Control system normal and emergency operating instructions. A new engineering alarm monitoring system was installed at the Engineering Control Center without prior Coast Guard approval. Submit plans to the Marine Safety Center for review within 60 days. After plans have been reviewed/approved by the Marine Safety Center, the Design Verification Testing Procedures and Periodic Safety Testing procedures shall be witnessed by the cognizant OCMI for final approval.

Witnessed operational steering test underway, all sat except center flanking rudder inoperable due to a leaking hydraulic cylinder. Parts ordered.

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. Thirteen (13) inoperable fire dampers; listed in an email sent to vessel Captain and company rep. Prior to Movement, Prove proper operation of thirteen (13) dampers in the presence of attending Marine Inspector. 46 CFR 72.15-15(a), 46 CFR 72.15-20

Each general emergency alarm signal must be an electrically-operated bell, klaxon, or other warning device capable of producing a signal or tone distinct from any other audible signal on the vessel. Five (05) Alarm Bells are inoperable. All Four (04) on 02 Deck and one (01) on 01 Deck STBD side Frame 68. Prior to Movement, Prove proper operation of all five (05) alarm bells in the presence of attending Marine Inspector.

The hull, structural bulkheads, decks, and deckhouses shall be constructed of steel or other equivalent metal construction of appropriate scantlings. 03 Deck-Port Side Electrical Room has Four (04) holes in forward bulkhead and STBD Side Electrical Room has Two (02) holes in outboard bulkhead. Prior to Movement, Prove proper structural integrity/fire protection of both bulkheads in the presence of attending Marine Inspector.

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. All connecting devices must be suitable for copper stranded conductors. Each outlet or junction box must have a cover unless a fixture canopy, switch cover, receptacle cover, or other cover is used. 03 Deck-Port Side Electrical Room loose wire exposed and cover not secured on junction box. Prior to Movement, Prove in the presence of attending Marine Inspector the wire and cover are secured. 46 CFR 111.60-17(a) and 46 CFR 111.81-1(c)

There shall be at least two means of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight doors. Main Deck – Port Side Engine Room Door 44 restricted with a cart and refrigerator in passageway. Chiller Flat – Forward of Engine Room Port Side, Escape Scuttle does not dog down. Bow Thruster Space – Escape scuttle does not open. Prior to Movement, Prove proper operation of all three (03) means of escape in the presence of attending Marine Inspector.

Operational readiness. Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. 01 Deck – Port Side Aft, Two (02) Fifty (50) person IBAs are not stowed properly with HRUs. Prior to Movement, Prove proper stowage arrangement in the presence of attending Marine Inspector.

(c)Illumination of passenger and crew spaces. (2) Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Port and Starboard wing storage areas forward of main engine room have no emergency lighting. Prove proper operation of emergency lighting in the presence of the attending Marine Inspector.

d) Navigation lights. Each navigation light must meet the following: (1) Meet the technical details of the applicable navigation rules. Aft Mast light (Not Under Command) inoperable. Prior to Movement, Prove proper operation in the presence of the attending Marine Inspector.

The inspection shall include a complete inspections of structure including the outside of the vessels bottom. Vessels hull plating suffered several large, sharp insets at the bow rake. Additionally, several frames and gussets in way of the damaged hull plating are deflected. Crop out and renew damaged areas to the satisfaction of the attending Marine Inspector.

(a) Approved fire detection and alarm systems must be installed on the following vessels as set forth in subpart 76.27 of this part: (2) Any vessel of more than 150 feet (45.72 meters) in length having sleeping accommodations for passengers; Installed fire detection system is displaying a trouble alarm for the main deck galley and stores (zone 12). Conducted operational test of the system to include (zone 12). Tested smoke detectors in zone 12, all sat. Vessel was unable to test heat detectors in zone 12. Galley is currently not being used. Prior to operating any heat sources in zone 12, identify trouble alarm(s), replace faulty component(s), and test to the satisfaction of the attending Marine Inspector.

All automatic auxiliary boilers must be tested and inspected in accordance with this subpart and subpart 61.05 of this part. Crew has no records for testing of auxiliary boiler. Boiler is required to be tested during next quarterly re-inspection in April 2023. Provide boiler information to Marine inspector prior to next vessel quarterly inspection to determine boiler inspection requirements.

At each inspection the inspector shall conduct test and inspections of all hull equipment including all subdivision bulkheads to determine that their watertight integrity has not been impaired. Various sections of framing, girders, and bulkheads were found severely wasted from vessels watertight bulkhead, frame 33, to the aft machine shop bulkhead, frame 61. The attached documents of the vessels structural design drawings indicate all the areas that were found. Crop/renew to good metal indicated areas of concern to the satisfaction of an attending Marine Inspector.

Fixed fire extinguishing systems must be checked as noted in table 71.25-20 (a)(2). In addition all parts of the fixed fire extinguishing system must be examined for general condition. Gaylord hood damper inoperable. Make repairs.

Where hose ports are fitted, they shall be cut in the lower corner of the door on the side opposite the hinge so that if the hose is passed through the doorway when the door is open, it may be closed over the hose. The cut for the hose port should be approximately 6 inches square. A hinged or pivoted steel or equivalent metal cover shall be fitted in the cut, equipped with a bullet catch or similar method of fastening which will permit easy and automatic operation of the hinged cover. Hose ports inoperable. Repair to requirement.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspections. Scupper pipe on starboard fwd. fuel void cut and unapproved repair installed. Make permanent repair to the satisfaction of an attending Marine Inspector.

Fire hazards shall be minimized insofar as is practicable. Excessive oil in the engine room bilge, steering room and starboard aft fuel oil tank supply valve leaking. Remove excess oil to the satisfaction of an attending Marine Inspector.

All subdivision bulkheads shall be examined to determine that their watertight integrity has not been impaired. Clean bilges and provide access to bow thruster room, engine room and steering room/aft void.

At each inspection the inspector shall conduct test and inspections of all hull equipment including all subdivision bulkheads to determine that their watertight integrity has not been impaired. Inspectors discovered severe corrosion to hull plating, internal framing, watertight subdivision bulkheads and fuel tank bulkheads. Also discovered multiple deformed gussets adjacent to fuel tank. Wasted fuel tank plating must be renewed. Vessel is required to be dry-docked and made available for US Coast Guard inspection to determine condition of suspect hull plating and internal framing. Vessel must undergo external hull examination and internal structural examination. The MISSISSIPPI must travel light boat from Memphis, TN to James Marine Inc. in Wickliffe, KY to undergo a US Coast Guard external hull examination and internal structural examination. Prior to departure the forward fuel tank must be stripped.

Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valve. All three SSDG and port MDE fuel supply remote operated valves were inoperable. Replace or service valves so they can be operated remotely.

Fixed fire extinguishing systems must be checked as noted in table 71.25-20 (a)(2). In addition all parts of the fixed fire extinguishing system must be examined for general condition. There was a cracked hose from a CO2 bottle to the discharge piping manifold on the engine room fixed CO2 system. Hose shall be replaced by authorized servicing provider.

The inspection shall include inspection structure, boilers, pressure vessels, machinery and equipment. Piping shall be inspected to insure satisfactory condition. There was fuel leak on EDG fuel fill piping.

The bulkhead deck must be water tight. Gaskets on all escape scuttles on the installed in the bulkhead deck (main deck)are deteriorated and no longer water tight.

The inspection shall include machinery and equipment to insure equipment is in satisfactory condition and fit for the service in which it is intended. One of the two MSD discharge pumps has been removed from service but has not been locked or tagged out properly. Pump isolation valves are not locked closed posing the potential for sewage leak. Circuit breaker is not locked out and pump wires have exposed conductors posing potential electrical shock hazard.

Portable conatainers use to store flammable and combustible liquids must meet requirements of UL 30 or UL 1313. Portable containers must be stored in a paint locker or open location designated by the master. Multiple portable gasoline conatainers were found stored underneath the stbd rescue boat. Containers were not secured for sea. Several containers were missing caps or were not UL approved. Remove unserviceable or unapproved containers. Store containers in paint locker or suitable open location where they can be secured for sea.

The grab line on lifebuoys shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum synthetic material having a minimum breaking strength of 1,350 pounds. The grab lines on the lifebuoys are deteriorated. Replace the lifebuoy grab lines

Power operated steering systems must have limit switches for cutting off power to the steering gear before the rudder reaches the stops. Stearing trunnions are hitting rudder stops. Adjust limit switches to proper settings.

In all ventilation systems manually operated dampers or other suitable means shall be provided outside the space served for shutting off the passage of air in event of fire. Ventilation cover gaskets are worn.

The inspection shall include inspection of machinery to insure it is in satisfactory condition and fit for service. EDG starter timer is malfuctioning causing the starter bendix to engage engine flywheel while engine is running. Engineers have to unwire starter after engine has started and operating to prevent malfunction.

Before the vessel leaves port and at all times during the voyage each lifesaving appliance must be in working order and ready for immediate use. Port rescue boat davit has significant leak on hydraulic cylinder. Vessel is equipped with two rescue boats and may operate with only one rescue boat. Port rescue boat davit shall not be used until hydraulic cylinder is repaired or replaced. The number of passengers is reduced from 113 to 107 for a total of 142 persons allowed to accommodate the rescue of the total persons allowed.

Each liferaft must be arranged for float-free launching. The arrangement must ensure that the liferaft, when released and inflated, are not dragged under by the sinking vessel. The 03 inflatable buoyant apparatuses were not arranged for float-free launching. Vessel crew made repairs on the spot.

Generator prime movers must shut down automatically upon loss of lubricating oil pressure. Starboard SSDG did not shut down upon loss of lube oil pressure.

If vessels certificate of inspection is renewed annually, the setting of the safety relief valve on pressure vessels must be checked annually. Vessel could not provide relief valve setting documentation.

Doors in main vertical zone bulkheads shall be of the self-closing type. Several fire doors would not self close. Rectify deficiency within 90 days.

The fire detection and alarm system must be so arranged and installed that the presence of a fire in any protected space will be automatically registered visually and audibly on the fire control panel. The visible notice must indicate the zone in which the alarm originated. Several fire detectors did not correlate to proper location on fire alarm panel. Rectify deficiency within 90 days.

Fire and Control plan must be approved by cognizant [name withheld] fire control plan has not been stamped "approved". Verify contents of plan and submit to OCMI for approval.

Drills must be conducted in the presence of a marine inspector. Full crew compliment was not present at time of inspection to conduct drills. Conduct drills in the presence of CG Marine Inspector within 14 days.

As an alternative to the survival craft requirements of § 199.201(b), vessels may carry inflatable buoyant apparatus having an aggregate capacity sufficient to accommodate 67 percent of the total number of persons on board, minus the capacities of any lifeboats, rescue boats and life-rafts carried on board. These inflatable buoyant apparatus must meet the arrangement requirements of § 199.630 (d)(1) through (d)(3). The number of persons accommodated in an inflatable buoyant apparatus may not exceed 150% of its rated capacity. Two (02) Inflatable Buoyant Apparatus' are on-board having a total capacity for 60 persons; not meeting 67% of total number of persons on-board. Passenger Count has been reduced to 74.

3 of the 20 flexible loop hoses connecting the activation heads to the E/R system manifold were threaded on to the activation head and not tightened. They were loose enough to unscrew by hand. One of the 20 flexible loops was only placed against the threads, but not screwed onto the threads at all and came off the fitting upon placing a hand on it. This would potentially caused a major failure of the E/R fixed CO2 system, had there been an E/R fire and the system was activated.

Battery operated e-light in MSD space did not operate when test button was pressed.

3 water tight doors below the main deck required adjustment. When doors were closed, up to 4 of the 6 dogs were loose and not engaged with the pads on the frame. At least one hand wheel was loose.

Emergency generator windings are destroyed. Temporary EDG is in place and hard wired into emergency circuit panel.

Found pin hole leak in center shaft tube, a welded doubler patch is allowed for temp repairs until permanent repairs can be made upon dry dock availability.

Provide 16 Type I child size life jackets as per COI.

Watertight door indicators must be operable on the pilot house

Starboard engine failed to shut down during emergency procedures due to faulty valve air intake.

Machinery space fire drill was conducted, observed multiple deficencies during drill. Crew must conduct machinery space fire drill to satisfaction of the [name withheld].

02 deck general alarm is inoperable, verify GE, bell & lights, PTCP.

Provide necessary markings for fire and emergency equipment missing throughout the vessel (CO2 alarms, manual alarm boxes, portable fire extinguishers, E-Lights, Etc...)
